Responsibilities
POSITION SUMMARY: The Speech Language Pathologist (SLP) - Journey Traveler must be able to travel nationwide for 13 week assignments. This position provides flexibility to travel the country and work in various settings. On assignment the SLP organizes and provides speech-language services for the location to facilitate rehabilitation. They direct patient participation in selected tasks to restore, reinforce, and enhance performance. Also, the Speech Language Pathologist facilitates the learning of those skills and functions essential for communication and swallowing, to diminish or remediate disorders or deficits. They direct activities to promote and maintain health, within the scope of practice and consistent with the Code of Ethics of the profession.
RESPONSIBILITIES/ACCOUNTABILITIES:
As necessary or applicable, supervision of CFYs temporary licensees, students and support personnel in accordance with state licensure requirements and professional standards.
Responsible for the effective and efficient delivery of rehab services; adherence to an established plan of care, department and patient(s)' schedule, and timely documentation of services.
Attends and contributes to patient care, staffing conferences and other related meetings.
Makes referrals to other disciplines, agencies in order to address the comprehensive needs of the patient.
Consults with other members of the rehabilitation team to select the most appropriate therapeutic regimen consistent with the needs and capabilities of each patient.
Adapts program and/or plan of care according to the needs of the individual patient.
Promotes a safe environment and enforces the elimination of fire and safety hazards.
Orders supplies and equipment as necessary.
Cleans, maintains, identifies, and reports the need for repair of equipment, as necessary.
Puts Patient Service first, ensuring that residents and families receive the highest quality of service in a caring and compassionate atmosphere which recognizes the individual's needs and rights.
Screens, examines and evaluates patients, including history, systems review, and application of appropriate tests and measures, synthesis information and interprets examination findings in order to establish a diagnosis, identify impairments, determine the predicted level of improvement and the time required to achieve it, identify precautions/contraindications and design and plan of care. Develops appropriate treatment goals and methods in collaboration with the patient and caregivers, implements the speech language pathology treatment plan, and completes all related documentation and record keeping regarding these services. Services include but are not limited to application of therapeutic techniques in areas such as:
cognitive-communication
speech intelligibility
oral motor skills
swallowing disorders/dysphagia
perceptual abilities
orientation
memory
pragmatics
psychosocial expression
functional communication
SLPs who have received specialized training that has resulted in competence through ASHA guidance in flexible/nasal endoscopy in order to use the tools for the purpose of evaluating and treating disorders of swallowing function may be required to perform Fiberoptic Endoscopic Evaluation of Swallowing (FEES).
Coordinates the acquisition of adaptive communication devices and instructs patients in their use.
Facilitates identification of hearing deficits and appropriately refers for assessment, diagnosis rehabilitation and/or personal amplification.
Selects appropriate activities suited to the individual's physical and mental capacity, intelligence level and interest.
Instructs patients in the skills and techniques required for participation in therapeutic activities, and evaluates patient progress, attitudes and behavior as related to rehabilitative potential.
Performs routine reexamination as needed/required to modify/progress plan of treatment.
Performs other related duties as required.
Qualifications
They must possess a master's degree in Speech-Language Pathology, Communication Disorders, Communicative Disorders or similarly titled area that is consistent and acceptable to the American Speech-Language-Hearing Association.

BAYADA Home Health Care is currently seeking an experienced Speech Language Pathologist, SLP, for a per diem opportunity performing home health visits for our office. This office services adult and geriatric patients in senior living communities (ALF/ILF) throughout Charlottesville, VA.
Prior home care experience as a Speech Language Pathologist, SLP is a plus, but not required.
As a home care Speech Language Pathologist, SLP, you will be an integral member of a multi-disciplinary health care team that provides rehabilitative care and skilled nursing to clients, affording them the opportunity to receive the medical care required to remain at home.
Speech Language Pathologist, SLP Responsibilities:
Make home visits to clients in designated geographic territories.
Perform thorough evaluations to assess, diagnose, and provide therapeutic interventions for clients who are experiencing speech, language, communication, or swallowing difficulties.
Develop a treatment plan based on the evaluation, physician's orders, and recommended goals.
Continually assess and revise the speech therapy care plan, and participate with BAYADA clinicians in the multidisciplinary care plan, as appropriate.
Educate and instruct patients, family members, or other patient representatives, in rehabilitative care and activities necessary to promote the client's health, safety, and independent living.
Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided, utilizing a state-of-the-art touch pad tablet.
Speech Language Pathologist, SLP Qualifications:
A current license as a Speech Language Pathologist in VA.
Possess a certificate of clinical competence in speech from the American Speech and Hearing Association, or;
Have the equivalent educational requirements, as indicated by school transcript or diploma, and work experience necessary for such a certificate, and be awaiting the opportunity to take the American Speech-Language-Hearing Association examination for certification, or;
Have completed the necessary work and education requirements, as indicated by school transcript or diploma, and be in the process of accumulating the necessary supervised work experience required for certification.

As a Speech Therapist, here's a big-picture view of what you'll do:
Provide in house speech therapy to patients having communication, swallowing or cognitive difficulties due to an illness or injury

The Speech Language Pathologist is expected to work independently, assess and write objective and attainable IEP goals and to develop individualized communication systems within a multi-disciplinary team. Examples of Essential Functions
- Screen, identify, assess and develop recommendations for students with communication disorders and deficits in functional communication, speech, language, fluency, voice, feeding/dysphagia and pragmatics.
- Provide individual and group treatment to students with varying speech and language abilities and needs.
- Provide appropriate and effective interventions for students with a wide range of socio-economic and cultural backgrounds and with varying intellectual, social and behavioral abilities.
- Develop and create appropriate augmentative/alternative communication (AAC) systems; train student, staff and parents on use of AAC (low-tech and hi-tech); ensure consistent use and incorporation of AAC into student's program.
- Advise and ensure the follow-through of adaptations, modifications and accommodations in order to maximize student learning and accessibility to the program.
- Conduct comprehensive assessments and evaluations using a variety of assessment tools and resources; develop complete written evaluations.
- May assist in the planning and developing of training programs; present in-service programs about various communication topics to staff and parents; instruct in techniques and procedures in speech language therapy; and prepare reports as required by the various jurisdictions.
- Collaborate and actively participate in IEP meetings and conferences.
- Notify teachers and administrators of identified speech and language students and develop an efficient schedule in which to work with them.
- Research new information on intervention/treatment techniques, methods, assessment materials, and available resources; share information with teachers and parents as appropriate.

Responsibilities
Speech-Language Pathologists (SLP) evaluate and provide skilled speech-language therapy to individuals in a home setting, under the care of an attending physician. GENERAL DEFINITION OF WORK:
Responsible for diagnosing speech and language disorders and for providing professional services to students through assessment, planning, goal development and provision of appropriate intervention services designed to enable students to access the general education curriculum.

Qualifications:

Licensure/Certifications:

Technical Skills: Proficiency in diagnostic assessments and treatment planning..
Soft Skills: Strong interpersonal communication, adaptability, and patient-centered care.
Key Responsibilities:
Conduct evaluations to diagnose speech, language, voice, and swallowing disorders.
Develop and implement individualized treatment plans for rehab patients.
Provide therapy to improve communication, cognitive-linguistic skills, and swallowing function.
Collaborate with physicians, nurses, and rehab teams to coordinate care.
Educate patients and families on home exercise programs and adaptive strategies.
Maintain accurate documentation of evaluations, progress notes, and discharge summaries.
Ensure compliance with facility protocols and regulatory standards.
