Job Description: Evaluates, plans, treats and implements care for patients in the areas of speech, language, cognition and swallowing in accordance with professional standards of the American Speech Language and Hearing Association (ASHA) and Essentia Health's policies and procedures. Education Qualifications:
Master's degree from accredited speech and language pathology program
Key Responsibilities:
Provide therapy evaluation, develop individualized treatment plans, implement evidence-based interventions, provide patient/caregiver education, and discharge planning for a full patient caseload.
Provide all ancillary/administrative requirements for a full caseload of patients, including documentation, billing, scheduling management, and other administrative duties
Demonstrate appropriate communication , professionalism and supervision of support staff (licensed assistants, aides , volunteers)
Collaborate with a multidisciplinary care team to ensure optimal clinical and financial outcomes.
Work closely with Essentia Health leadership and coordinate with referring physicians to ensure cohesive patient care.

This role includes a weekend rotation once every 4 weeks (minimum), Saturday at the hospital, and Sunday on-call. Shifts may range from a half day to a full day, depending on census. This is a casual position intended to provide coverage for open shifts due to PTO, leaves of absence, and other staffing needs.

• Direct Care Activities
Evaluate clients; obtain and interpret data necessary for treatment from medical records, observation of client in natural setting, interviews, and administration of standardized tests and measurements. Develop treatment plans and programs, including short and long-term anticipated functional outcomes, and administer speech-language therapy treatment. Obtain approval for services from physicians or designees; and provide treatments per physician's orders. Provide quality care that meets client's goals and needs by performing periodic client re-evaluations to determine effectiveness of treatments; then adjusting treatment plan and program appropriately to reach functional outcome. Work closely with RN case managers and intake personnel for client intake, care planning, and therapy discharge planning of clients. Maintain same day admission philosophy and communicate when this is unachievable and coordinate with RN case managers when admission can occur. Communicate to case manager when admission cannot occur within 48 hours.
• Client Records and Billing
Provide accurate and timely documentation of services rendered utilizing specific home health software. Demonstrate understanding and knowledge of all payer sources. Review and sign documentation of the Physical Therapist Assistant as required, complete appropriate visits for PTA supervision if applicable. Complete and/or check accuracy of completion and timely submission of billing information and appropriately account for all services rendered. Keep schedule on EMR as accurate and up to date as possible.
• Communication
Demonstrate exceptional verbal and written communication skills for purpose of work tasks and patient care. Able to clearly communicate with patients, families, staff and others to meet job requirements. Modify and adjust communications as needed to reach common understanding. Must have and know how to use a cellular phone.
• Follow-up Programs
Optimize successful discharge from therapy for patients with highest function and least amount of risk possible allowed per entitlement. Educate client and family/caregivers about transition to restorative, wellness or nursing programs. Educate client in appropriate home exercise program or other health management program. Recommend and train in use of appropriate assistive equipment and environmental adaptations to maximize function. Professionally connect with physicians, outside vendors and other healthcare professionals of services when appropriate to ensure continuum of care.
