The Speech Language Pathologist (SLP) provides direct rehab care in speech therapy to patients upon physician referral in accordance with all applicable laws, regulations, and Life Care standards.
Education, Experience, and Licensure Requirements
Graduate of an accredited program in speech therapy (minimum MS or MA SLP)
ASHA certified (CCCs) OR must be eligible for 9 month Clinical Fellowship Year (CFY) in order to earn ASHA certification (CCCs) and/or meeting ASHA credentialing criteria. Once achieved, must maintain ASHA credentialing.
Currently registered/licensed in applicable State. Must maintain an active license in good standing throughout employment.
One (1) year experience in post acute care or related setting preferred
CPR certification upon hire or obtain during orientation. CPR certification must remain current during employment.

Maintains confidentiality of all proprietary and/or confidential information
Understand and follow company policies including harassment and compliance procedures

As a Home Health Speech Language Pathologist, here's a big-picture view of what you'll do:
Provide home-based speech therapy to patients having communication, swallowing or cognitive difficulties due to an illness or injury
Work as part of a home health team which may include an RN, LPN, CNA, HHA, PT and OT, focused on the patient's plan of care and established goals
Observe and evaluate a patient's ability to communicate, swallow and process information
Conduct patient assessments, document progress and ensure patient goals are met
Assist with mouth, breathing and swallowing exercises, fluency, articulation and reading

PRIMARY DUTIES AND RESPONSIBILITIES
Provide appropriate therapy to eligible students, consult with teachers and other staff members, serve as a resource to faculty, and participate in Child Study Team.
Serve as a resource to school staff members in the development of a balanced program for oral communication and language development.
Provide a therapeutic program to meet individual needs of speech and hearing disabled students.
Assist and guide teachers in observing, describing, and referring suspected speech and language impairments.
Provide a thorough assessment and diagnosis of speech, voice, and language impairments.
Provide screening to identify speech disabled children in compliance with state and federal guidelines.
Assist in proper referrals of individuals to agencies and specialists in the community as appropriate.
Provide appropriate individualized and group programs of therapy to meet individual student's needs and ameliorate existing speech or language disabilities.
Collaborate with classroom teachers and other school staff members to implement therapy by suggestions from the student's daily activities.
Provide in-service education and serve as a consultant to teachers and school staff members on topics concerning speech improvement.
Provide information, support, and counseling to parents and families when appropriate.
Keep thorough ongoing records for the individual student receiving therapy or other school provided speech services.
Attend child study meetings providing appropriate input on the special needs of students.
Conduct annual IEP conferences and other IEP related meetings as needed, develop appropriate IEP goals and objectives in consultation with parents and other professionals.

What we're looking for
Applicants should have a master's degree in Speech Language Pathology that is accredited by Education Standards Board or whose practice meets ASHA certification requirements. Additional requirements include:

Speech Therapist FT Days Position Summary
Evaluates, plans and implements specific treatment programs for individual patients according to the principles and practices of speech language pathology. Functions as an interdisciplinary team member providing patient assessment/re-assessment, direct patient care, and individualized care planning/implementation. Works with neonatal through geriatric patients with clinical competency, as applicable. Completes established competencies for the position within designated introductory period.
Responsibilities
Responds to physician orders for physical therapy. Administers and interprets tests and measurements of patients. BAYADA Home Health Care is currently seeking an experienced Speech Language Pathologist, SLP, for a per diem opportunity performing home health visits for our office located in Sierra Vista.. This office services adult and geriatric patients in homes throughout Cochise County .
Prior home care experience as a Speech Language Pathologist, SLP is a plus, but not required.
As a home care Speech Language Pathologist, SLP, you will be an integral member of a multi-disciplinary health care team that provides rehabilitative care and skilled nursing to clients, affording them the opportunity to receive the medical care required to remain at home.
Speech Language Pathologist, SLP Responsibilities:
* Make home visits to clients in designated geographic territories.
* Perform thorough evaluations to assess, diagnose, and provide therapeutic interventions for clients who are experiencing speech, language, communication, or swallowing difficulties.
* Develop a treatment plan based on the evaluation, physician's orders, and recommended goals.
* Continually assess and revise the speech therapy care plan, and participate with BAYADA clinicians in the multidisciplinary care plan, as appropriate.
* Educate and instruct patients, family members, or other patient representatives, in rehabilitative care and activities necessary to promote the client's health, safety, and independent living.
* Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided, utilizing a state-of-the-art touch pad tablet.
Speech Language Pathologist, SLP Qualifications:
* A current license as a Speech Language Pathologist in Arizona
* Possess a certificate of clinical competence in speech from the American Speech and Hearing Association, or;
* Have the equivalent educational requirements, as indicated by school transcript or diploma, and work experience necessary for such a certificate, and be awaiting the opportunity to take the American Speech-Language-Hearing Association examination for certification, or;
* Have completed the necessary work and education requirements, as indicated by school transcript or diploma, and be in the process of accumulating the necessary supervised work experience required for certification.
* Solid computer skills; prior experience with electronic medical records (EMR) preferred.
