The Speech Therapist in Home Health is responsible for the assessment and evaluation of patient care needs related to treating speech and language disorders and functional training in communication, swallowing, and cognitive impairments.
Provides services within the scope of practice as defined by the state laws governing the practice of speech therapy, in accordance with the plan of care, and in coordination with other members of the health care team.
Evaluates the patient's level of function by applying diagnostic and prognostic functional ability tests. Assists the physician in the development of the therapy plan of care.
Treats patients to communicate effectively by expressing thoughts according to the patient's condition using acceptable standards of practice.
Instructs the patient, the family and/or caregiver and other members of the health care team in areas of speech therapy in which they can participate.

This Speech Therapist opportunity is per diem and offers a convenient schedule. This dynamic individual will be responsible for administration and direct application of speech/language pathology treatment modalities as prescribed by a referring physician. Evaluates, plans, directs and treats all patients referred to speech/language pathology treatments to restore function and prevent disability following disease or injury. Assists patients to obtain maximum performance and function in society while learning to live a routine “normal” life within the constraints of their disability. Participates in performance improvement activities within the department and participates in CQI activities. Assures personal competency. Provides information related to departmental budget considerations in relationship to speech pathology needs for optimum patient care and service. Assists in scheduling of patient visits for speech pathology evaluation and treatment. Performs some administrative duties at the delegation of the director of rehabilitation services. Participates in infection control, safety and educational programs, both in the department and on an organizational basis.
Job Duties/Responsibilities:
Upon physician referral, performs patient evaluations to determine problems and goals.
Able to assess patient pain interfering with optimal level of function or participation in rehabilitation - makes appropriate physician contact for intervention.
Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
Formulates a teaching plan based upon identified learning needs and evaluates effectiveness of learning, family is included in teaching as appropriate.

* ASHA certified (CCCs) OR must be eligible for 9 month Clinical Fellowship Year (CFY) in order to earn ASHA certification (CCCs) and/or meeting ASHA credentialing criteria. Once achieved, must maintain ASHA credentialing.
* Currently registered/licensed in applicable State. Must maintain an active license in good standing throughout employment.

* CPR certification upon hire or obtain during orientation. CPR certification must remain current during employment.

Primary Therapy Source is looking for a Speech Language Pathologist to join our team. We are an out-patient organization that offers Physical, Occupational, Speech, and Behavioral therapy to patients of all ages. We are the leading pediatric therapy organization in Southern Idaho and the only company to offer all four therapies in one place. We are contracted with several local school districts to provide high quality therapy to children, adolescents, and young adults in these school districts. We currently have two clinic locations. Qualified candidates will have the following skills/requirements:
The Speech Language Pathologist will assist patients of all ages in both learning and/or regaining proper speech patters and functional use of communication.
Minimum Requirements and Qualifications:
Master'sdegree in Speech Language Pathology
Must possess a valid Idaholicense as a Speech/Language Pathology
Valid driver's license
Speaks Spanish (preferred, not required)
Employer conducted background check
Duties and Responsibilities may include, but are not limited to:
Provides direct treatment
Follows and implements documented treatment plans
Documents progress toward meeting established objectives and reports the information to the therapist
Adapts/modifies instructional materials as determined by patients needs and abilities
Must be able to interact and communicate effectively with children and adults
Must be able to work independently with minimal supervision
Must be able to react with flexibility and sensitivity to changing situations
Transportation, as needed
