JOB DESCRIPTION:
SUMMARY OF POSITION
Provides services that help consumers gain access to needed medical, social, educational and other services. This includes direct assistance in gaining access to services, coordination of care, oversight of the entire case, linkage to appropriate services, community-based work and ensuring continuation of entitlements.
ESSENTIAL DUTIES & RESPONSIBILITIES
Provides direct client services. Manages case load of life skills training for clients.
Facilitates client's use of family, social and community resources, acting as an advocate when necessary and maintaining necessary communication with other involved parties.
Assesses and develops person-centered plan and monitor progress of the plan.
Collaborates with team and outside agencies. Serves as the main point of connect for communication with resources or other entities to ensure continuum of care.
Completes client assessments (ex: Ansa, PHQ9, GAD7) and develop care plans, monitor client's progress through care plan.

What You'll Do

The Chief Executive Officer (CEO) is responsible for planning, organizing, directing, monitoring and evaluating all external and internal functions of the Corporation. SUMMARY OF POSITION
In addition to working as a Managed Care Specialist, the Senior Specialist has the responsibility of coordinating the daily task within the Managed Care team assigned to lead. The Senior MCS provides weekly status update reports to the team and leadership.
Also, this role will review accounts to resolve concerns with payer information that is not correctly entered. Assists with obtaining benefit information and sets up coverage information to ensure proper billing. Responsible for processing of denials due to no authorizations received and works to resolve.
ESSENTIAL DUTIES & RESPONSIBILITIES
Timely and accurate data entry.
Maintain a collaborative relationship with Support Services team members and clinical staff.
Timely and accurate completion of authorization, referrals or payer information for set up the financial chart.
Compliance with annual completion of Relias training as required.
Coordinating the daily task within the Managed Care team assigned to lead.
The Senior MCS will also facilitate a daily team meeting to check in with the team members to review task assignment obtain project status updates as well as answer questions, concerns or view workflow processes.
Creates and reports weekly status updates to the progress of the team work load and assignments.
The Managed Care Specialist is responsible for reviewing the daily reports which pertain to the Managed Care team assigned.
These reports include authorizations, service package assignments, referral information or updating payer information.
The MCS works to set up the financial chart and reviews payer information in order to bill for services.
The MCS will review accounts to resolve concerns with payer information that is not correctly entered.
This can include review of future scheduled appointments with lapsing authorizations. Must maintain a working knowledge of payer requirements for proper scheduling and authorizations.
The MCS is Responsible for reviewing daily service activity failures with regard to authorizations. Also includes billing failures for coverage issues.
Assists with obtaining benefit information and sets up coverage information to ensure proper billing. Responsible for processing of denials due to no authorizations received and works to resolve.

The Peer Support Specialist provides compassionate, person-centered support to individuals experiencing homelessness in a day shelter environment. Provides supportive services during mobile crisis response and crisis intervention services to clients, families, community partners and referral sources. ESSENTIAL DUTIES & RESPONSIBILITIES
Assists in crisis assessments, safety planning, return to use prevention planning, overdose prevention planning of individuals at a local community hospital once discharged from hospital care, local police department, school, jail, or other community-based sites, and facilitate appropriate linkage and referral to on-going services.
Assists in emergency mental health consultations with physicians, law enforcement officials, school officials, clinical on call staff and other community and/or state agency representatives in an effort to resolve mental health and/or substance use disorder crisis regarding residents in our service area.
Assists with pre-screenings for both the state operated hospital and private psychiatric hospitals to determine the need for either voluntary or involuntary psychiatric hospital admissions. Coordinates appropriate staffing with clinical team members as indicated which may include assisting with referrals to Substance treatment level of care and/or psychiatric inpatient care
Assists individuals in making safety plans, discharge plans, overdose prevention plans, return to use prevention plans. Provides follow up services as assigned or assists with team member follow up planning/scheduling for client safety and well-being.

POSITION SUMMARY
Under the supervision of the Pharmacy Manager and CEO, ensure accuracy during the prescription filling process. The chosen candidate will be working as a pharmacist in the Columbus CHS clinic to assist when the full-time pharmacists are out of the pharmacy. The pharmacist will work with patients and providers to accurately fill prescriptions, assist providers with treatment decisions when consulted, provide immunizations, and patient consultations as needed. The candidate will also work with providers and health coaches within CIN to assist with group home and ACT team patients.
Responsibilities:
Prepare medications by reviewing and assessing orders to identify, prevent and resolve drug related problems
Provide accurate and understandable counseling and education to clients and staff
Perform drug utilization reviews and check for drug interactions when filling every prescription
Communicate effectively with prescribers, nurses and other healthcare providers
Maintain client rights and confidentiality at all times
Communicate with medical offices regarding medication issues and refills
Provide Medication Therapy Management and bubble packing of medications to clients when appropriate
Maintain accurate and confidential records and documentation
Supervise pharmacy staff and assist in scheduling, training and ongoing evaluations
Provide high quality customer service and accurate answers to medication inquiries in person and over the phone
Maintain current Indiana pharmacist license and complete all continuing education requirements as designated by the Indiana Board of Pharmacy

Minimum Job Requirements:

SUMMARY OF POSITION
Assist the physician(s) with patients. This includes preparing patients and the examination rooms; taking initial medical history for the chart, taking vital signs, making lab and x-ray appointments, and explaining the preparations to the patients. Assist physician(s) in all special procedures, including minor surgery, as required. Removes sutures, clean wounds, and apply dressings as directed by physician(s).
