Responsibilities
* Responsible for screening visitors to the appropriate location ensuring security to the building. Ensures/informs all visitors sign in and wear a visitor's pass when entering the building supplied by the receptionist.
* Responsible for daily coverage for the admissions coordinator during breaks and various times when required. Connects/transfers calls to the appropriate party and performs overhead paging.

**Responsibilities**
**Position Summary**
In this highly visible, challenging and rewarding position, you will assume a leadership role, developing and nurturing professional relationships with case managers, physicians and clinicians in acute care hospitals; educating staff, patients and families regarding rehabilitation services provided by our rehabilitation hospital. **Qualifications**
**Minimum Qualifications**
+ Current and valid state licensure or eligibility for state licensure as a Registered Nurse, Physical Therapist, Occupational Therapist, or Speech Language Pathologist required. Multi-state licensure required based on geographical territory.
**Preferred Experience**
+ Two years related experience working in a clinical healthcare setting or previous experience working in a clinical liaison role preferred.
+ Demonstrated competency with the preadmission process including assessments and relationship building in a rehabilitation setting or in another post-acute care setting preferred.

Responsibilities
What an Admissions Coordinator Does
The Admissions Coordinator serves as a member of the Business Development Team through managing the referral intake and admission of all patients. You will stay in close communication with all departments to prepare for the day of admission.
A Day in the Life
You will be a champion of The Select Medical Way, which includes: putting the patient first, helping to improve quality of life for the community in which you live and work, developing and exploring new ideas, providing high-quality care and doing well by doing what is right.
* Oversees the pull-through process for the hospital under the direction of the Director of Business Development - providing support, customer service and timely resolution of all aspects for admissions and seeking assistance from the DBD or CEO as needed
* Ensures that all deficiencies remain at 0% to eliminate holds, working with CBO and HIM to ensure any errors are resolved.
* Ensures every referral is timely and accurately entered in Transitions of Care
* Processes timely insurance verifications
* Assists in pre-cert/authorization process - tracks for follow up and timely processing
* Works with DBD, CEO and other leaders to ensure timely admissions from time of referral
* Works with team to resolve occasional challenges and overcome obstacles
* Is admissions focused
* For Case Manager: Provides appropriate documents to Case Management team timely
* Enters patient data in HMS/MedHost when pre-admitting, and enters details of patient's benefits and billing instructions.
* Completes admissions paperwork and reviews all patient forms/demographics with patient/responsible party. Secures signatures on all necessary documents. Scans completed documents, including insurance cards and photo ID.
* Leads the morning meeting with census, budgeted numbers, etc.
* Communicates admissions time, bed number, physician, etc. to the hospital when there is a confirmed admission.
* Provides tours of hospital.
* Provides customer service by accurately answering questions, for internal and external customers.
* Manages LOA's in HMS/MedHost, ensuring accurate entries for all LOA patients. Completes forms as necessary for corrections.
* Review and enter daily census via Select Portal by 11am EST. Ensure accuracy on a daily basis, as well as initiating any changes/updates that might be necessary.
* Performs Other duties as assigned.

Evaluates patient and recommends a treatment plan.
Explains purpose and procedure of treatments, tests, and equipment to the patients, and provides necessary education upon discharging to home.
Conducts physician prescribed treatments of a routine nature operating various ventilatory equipment, both invasive and non-invasive, various oxygen and aerosol devices, and various nebulizer devices in order to administer prescribed medications, humidification, ventilation and oxygenation to the patient.
Performs chest physiotherapy by operating various equipment to promote secretion clearance, suctioning of the patient by various methods (oropharyngeal, nasal and tracheal), oximetry and capnography monitoring according to policy.
Performs routine review of patient's chest x-rays to determine need for further evaluation.
Performs 12-Lead EKG and basic arrhythmia recognition.
Performs pulmonary function studies according to policy (i.e. NIF, VC etc.)
Assists with procedures relating to respiratory care according to policy (i.e. Bronchoscopy, Perc Trachs, Chest Tube placement, etc.), where applicable.
Performs adequate drainage of chest via PleurX catheter or similar device where applicable.
Performs tracheostomy care according to policy, including insertion and removal of tracheostomy tubes (emergent and non-emergent).
Performs arterial puncture according to policy and is able to determine efficiency of ventilation, as well as monitoring, recording and reporting of test results, where applicable.
Performs cardiopulmonary resuscitation according to BLS and ACLS guidelines.
Performs insertion and removal of endotracheal tubes via free hand technique and/or video laryngoscope system, where applicable.
Records all pertinent information of treatments, and patient response to treatments on respiratory care records and patient chart (including pre and post-assessment according to policy).
Communicates with attending and pulmonary physician's concerning the efficiency of therapy administered to the patient.

+ Enters patient data in HMS/MedHost when pre-admitting, and enters details of patient's benefits and billing instructions.
+ Completes admissions paperwork and reviews all patient forms/demographics with patient/responsible party. Secures signatures on all necessary documents. Scans completed documents, including insurance cards and photo ID.
+ Leads the morning meeting with census, budgeted numbers, etc.
+ Communicates admissions time, bed number, physician, etc. to the hospital when there is a confirmed admission.
+ Provides tours of hospital.
+ Provides customer service by accurately answering questions, for internal and external customers.
+ Manages LOA's in HMS/MedHost, ensuring accurate entries for all LOA patients. Completes forms as necessary for corrections.
+ Review and enter daily census via Select Portal by 11am EST. Ensure accuracy on a daily basis, as well as initiating any changes/updates that might be necessary.
+ Performs Other duties as assigned.

Responsibilities
The Case Manager is responsible for the coordination of health care decisions by using a systematic approach to assure treatment plans that improve quality and outcomes, coordination of care across the continuum; promotion of cost-effective care within the allotted time frame; assuring payments of hospital-based services meeting patient-related utilization management criteria, and implementation of safe and appropriate discharge plans. The Case Manager assesses the psychosocial needs of the patient and provides intervention as part of the discharge planning process.

Qualifications
Minimum Qualifications
* Current Licensure per state guidelines in clinical or related discipline OR a Bachelor's or Master's in a health or human services discipline.

* CCM Certification Preferred.

Responsibilities
Under the general supervision of a Registered Nurse, the Patient Care Technician performs routine tasks in patient care/assistance and documentation of care provided.
Transports patients to and from various treatment areas when necessary; assists in moving patients in and out of stretchers and wheelchairs.
Performs services for the patient involving dressing, showering, bathing and oral feeding.
Answers patient lights and performs services in a timely manner that adds to the physical well-being of the patient. Participates in patient turning/repositioning, performs purposeful hourly rounds.
Performs other duties as assigned.
Qualifications
Minimum Qualifications
High school diploma or equivalent
Requires hospital experience as a patient care technician, nursing assistant or experience in a related field such as a medical assistant or as an actively enrolled nursing student.
Certified BLS or completion within first 90 days of employment required.
