Position Summary
Provides Physical Therapy services to residents by assisting in the treatment, program planning and implementation, related documentation and communication while functioning under the supervision of the Physical Therapist.
Essential Duties and Responsibilities include the following:

Provides Physical Therapy services to patients in a safe and effective manner under the supervision of a Physical Therapist in accordance with state regulations. Provides treatment in accordance with established standards of practice, department procedures and productivity standards.
Assists the Physical Therapist in the evaluation process as needed or directed.
Assists the Physical Therapist in identifying a problem list, short-term goals, long term goals and treatment plan.
Conducts both individual and group treatments as assigned by the Physical Therapist and outlined in the treatment plan that includes but is not limited to areas of muscle strength, coordination, mobility, sensory awareness, sitting and standing, transfer training, balance activities, gait training, range of motion activities and modalities.
Assists in selection of Physical Therapy techniques and in the sequence of activities, adapts techniques as indicated by patients' performance and following consultation with the Physical Therapist. No major modification to the treatment plan is made without prior consultation with the Physical Therapist.
Monitors patient's response to intervention and consults with Physical Therapist if modification is required.
Assists in establishing and modifying individual patient schedules in conjunction with the Physical Therapist and other team members.
Adheres to department and patient schedule and modifies as appropriate for treatment regimen.
Implements and conducts treatment as outlined in treatment plan.
Provides/recommends/fabricates adaptive devices, orthotics. Trains patient, family and appropriate staff in the use or application of the above.
Consistently demonstrates sound judgement in the implementation and follow-up of patient therapy.
Integrates Bethany Home Health standards of care into patient management.
Essential Communication
Participates in patient, family, and staff education.
Maintains positive relationships and rapport with coworkers, patients, family members and facility personnel.
Adheres to establishing confidentiality standards including all applicable HIPAA rules and regulations.
Projects a positive and professional image at all times.
Essential Processes
Documents results of patient's assessments, treatment, follow-up, and termination of services. Adheres to Physical Therapy department procedures regarding documentation of Physical Therapy services.
Accurately and efficiently utilizes documentation and billing software to record patient treatment, labor and CPT codes according to Bethany procedures.
Complies with all Infection Control, Universal Precautions, and OSHA standards for the healthcare professional and adheres to facility/company safety standards.
Reports, as appropriate, actual or suspicious activity related to fraud and/or operational abuse of laws, regulations and standards.
Reports to work on time and coordinates schedules to achieve maximum productivity and efficiency during their shifts. Maintains a regular schedule ensuring that patients get proper therapy.
Assists with and participates in Quality Assurance Program as required.
Other Responsibilities
Attends in-service meetings and facility meetings as required.
Assumes responsibility for ongoing continuing education and professional development.
Performs other duties as assigned.
Qualifications
Minimum Position Qualifications
Associate degree in Physical Therapy from an accredited program
Current state licensure
Current CPR certification
Must have a current Driver's License and vehicle insurance, and access to a dependable vehicle.

STAFF DEVELOPMENT COORDINATOR
Develop, implement, and maintain an effective training program for all new and existing staff, contracted staff, and volunteers, consistent with their expected roles.
Major Duties and Responsibilities
Participates in the completion of the facility assessment at least annually for determining the knowledge and skills required among staff to meet residents' needs.
Identifies and prioritizes the facility's educational needs through the completion of an educational needs assessment, based on the facility assessment.
Works closely with the facility's Compliance Officer and other leaders in developing a training plan that addresses all regulatory requirements and any other topics identified in the facility assessment and/or needs assessment. Develop educational calendars in accordance with the plan.
Plans, schedules, conducts, and oversees orientation programs for all personnel.
Develops, schedules, and directs refresher training, as necessary, for all personnel. Tracks attendance/completion of training to ensure that all required training is received by all personnel in a timely manner.
Reviews educational content to ensure the information reflects current standards of practice and facility policies and procedures. Develops or revises content as needed. Utilizes outside resources for content development, such as consultants, content experts, or electronic learning systems.
Collaborates with department managers for developing competencies required for meeting residents' needs. Assists with evaluating competencies, depending on the knowledge-based, technical, or behavioral nature of the competency.
Cooperates with the Administrator during routine evaluation of the training program's effectiveness.
Schedules training with flexibility to ensure limited disruption of resident care, and ample opportunity for staff to attend. Posts notices of training in advance.
Maintains all training records, reports, instructional materials, and speaker qualifications related to each educational program.
Provides opportunities for staff to obtain continuing education credits for maintaining certifications/licensure. Maintains all documentation related to continuing education.
Maintains attendance records and individual training records in accordance with facility policy.

A nursing degree from an accredited college or university or be a graduate of an approved LPN/RN program.

Purpose of Position:
