Position Summary
Provides Physical Therapy services to residents by assisting in the treatment, program planning and implementation, related documentation and communication while functioning under the supervision of the Physical Therapist.
Essential Duties and Responsibilities include the following:

Provides Physical Therapy services to patients in a safe and effective manner under the supervision of a Physical Therapist in accordance with state regulations. Provides treatment in accordance with established standards of practice, department procedures and productivity standards.
Assists the Physical Therapist in the evaluation process as needed or directed.
Assists the Physical Therapist in identifying a problem list, short-term goals, long term goals and treatment plan.
Conducts both individual and group treatments as assigned by the Physical Therapist and outlined in the treatment plan that includes but is not limited to areas of muscle strength, coordination, mobility, sensory awareness, sitting and standing, transfer training, balance activities, gait training, range of motion activities and modalities.
Assists in selection of Physical Therapy techniques and in the sequence of activities, adapts techniques as indicated by patients' performance and following consultation with the Physical Therapist. No major modification to the treatment plan is made without prior consultation with the Physical Therapist.
Monitors patient's response to intervention and consults with Physical Therapist if modification is required.
Assists in establishing and modifying individual patient schedules in conjunction with the Physical Therapist and other team members.
Adheres to department and patient schedule and modifies as appropriate for treatment regimen.
Implements and conducts treatment as outlined in treatment plan.
Provides/recommends/fabricates adaptive devices, orthotics. Trains patient, family and appropriate staff in the use or application of the above.
Consistently demonstrates sound judgement in the implementation and follow-up of patient therapy.
Integrates Bethany Home Health standards of care into patient management.
Essential Communication

Maintains positive relationships and rapport with coworkers, patients, family members and facility personnel.
Adheres to establishing confidentiality standards including all applicable HIPAA rules and regulations.

Essential Processes
Documents results of patient's assessments, treatment, follow-up, and termination of services. Adheres to Physical Therapy department procedures regarding documentation of Physical Therapy services.
Accurately and efficiently utilizes documentation and billing software to record patient treatment, labor and CPT codes according to Bethany procedures.
Complies with all Infection Control, Universal Precautions, and OSHA standards for the healthcare professional and adheres to facility/company safety standards.
Reports, as appropriate, actual or suspicious activity related to fraud and/or operational abuse of laws, regulations and standards.
Reports to work on time and coordinates schedules to achieve maximum productivity and efficiency during their shifts. Maintains a regular schedule ensuring that patients get proper therapy.
Assists with and participates in Quality Assurance Program as required.
Other Responsibilities
Attends in-service meetings and facility meetings as required.
Assumes responsibility for ongoing continuing education and professional development.
Performs other duties as assigned.

Associate degree in Physical Therapy from an accredited program

The role of the Chaplain for Bethany of the Northwest is envisioned as a Rostered Pastor position with responsibility for Word and Sacrament Ministry among the residents, providing (or providing for through other spiritual care resources) the spiritual care of residents, clients and staff throughout the Bethany system. The Chaplain will provide spiritual support through the Bethany facilities and care for our hospice clients in their own homes. The Chaplain will preside at worship, plan, coordinate, counsel and provide spiritual care services in accordance with the teachings and guidelines of the Church and current applicable federal, state and local standards and regulations which govern Bethany of the Northwest. The Chaplain must have completed Clinical Pastoral Education by ACPE.

Spiritual Care Functions:
Provide direct spiritual care to residents/patients, their families, and Bethany staff at various facilities, units of Bethany of the Northwest and in clients' homes where Bethany Hospice provides services and support, as assigned.
Share Sunday Worship leadership duties by rotation; preach the Word and administer the Sacraments for residents, their family members, and employees.
Lead spiritual enrichment such as (but not limited to) religious worship or devotional services, memorial services, prayer groups, etc.
Personnel Functions:
1. Collaborate with Spiritual care team to create a comprehensive spiritual care program to fully support the residents and staff of all Bethany facilities.

Staff Development:

Specific Requirements:
Candidates and their national endorsing body must understand and agree to this as a ministry on an ecumenical ministry team, where pulpit and table fellowship between every ordained minister on the Chaplain team is a requirement of the position.
Must have a minimum of three (3) years' experience in spiritual care ministry or related work.
Must be a Graduate of an accredited seminary or school of theology (BD, MDiv or equivalent theological degree). Have experience working with death and dying, individuals/family/caregivers.
Must be oriented to health care practices and procedures.
Clinical Pastoral Education by ACPE - Associate 2 units, Staff/Sr Staff 4 units

Job DescriptionDescriptionTitle: Nutritionist Reports to: Director of SNAP-Education Position Type: Full-time YMCA of Greater Boston Nutritionist The YMCA is a lead partner for the Greater Boston Food Bank and operates a warehouse Grocery Bag Distribution Program that packages and delivers 4,500+ grocery bags weekly to more than 45 partners through the city of Boston. A new Mobile Market will be operational in 2023 and will coordinate services in the city of Boston. The goal is to serve communities daily with food choice and individual selection. The YMCA of Greater Boston is also a partner to the Department of Transitional Assistance (DTA) and delivers SNAP-Education/ Nutritional Education to all distribution Centers in the city of Boston, partners with the Massachusetts Alliance of Ys and works directly with JSI and independent consultant to measure the impact of all Distribution Center Operations and Activities. Annually pantry meals will deliver more than 3 million meals.
The YMCA is a year-round food sponsor that provides meals to different sites throughout the city. We are a United States Department of Agriculture (USDA) sponsor of the Summer Food Service Program (SFSP) and the Child and Adult Care Food Program (CACFP). To assist us in meeting our mission and goals, the YMCA is seeking a Nutritionist to join our team.
Qualifications:
Bachelor's Degree from a four-year accredited college
1-2 years experience in nutrition education
Ability to build strong relationships and work effectively with a wide variety of people.
Excellent communication skills (written and verbal).
Experience and comfort with public speaking and presentations.
Interest and willingness to collaborate with exercise professionals to teach participants about the connections between nutrition and exercise.
Must be able to lift up to 50lbs and stand for most of the workday.
Must be motivated, energetic, punctual, responsible, and exhibit a positive attitude to be an excellent role model for staff members and partners.
Considerable skill to build external partnerships.
Demonstrated respect for the teaching, learning, and evaluation process.

Key ResponsibilitiesEssential Responsibilities:
The Nutritionist role is responsible for connecting the menus for the SFSP and CACFP with SNAP-Ed approved curriculum. The Nutritionist will create and implement SNAP-Ed nutrition lessons to youth utilizing the food program menus.
The Nutritionist will work closely with partner sites, youth and children, and families. Strong communication and coordination skills with partners and community members are integral in this role.
This position reports to the Director of SNAP-Education.
Administration & Management:
• Design and implement direct education services garnered towards youth in the Greater Boston Area, other YMCA programs, and partners.
• Implement education curriculum and programming at after school programs, early education centers, summer food sites throughout the Greater Boston area.
• Support at Grocery Bag Distribution Center and Mobile Market Food Truck sites.
• Select curriculum and make adjustments that align with communities served.
• Collect survey data for curriculums implemented.
• Create and administer education curriculum for adults and families.
• Oversee menus for SFSP and CACFP program and ensure menus are meeting DESE and USDA guidelines. Coordinate with meal vendors if menu adjustments need to be made.
• Coordinate with partners to host educational trainings for youth, partner site staff, and families.
