Provide resident care services by following Resident's Plan of Care Monitor residents' health status and nursing needs. Implement and evaluate plan of action. Evaluate health and resident care services: Consult with physician and other health professional as needed to meet the needs of the residents. Transcribe MD orders and follow through with informing all areas appropriately of new order. Provide leadership for nursing team members: Direct and monitor the day-to-day functions of the NA/R's in accordance with current rules, regulations, and guidelines that govern the long-term care facility. Assist nursing personnel to learn new nursing care policies, procedures, and concepts of care. Collaborate with the Director of Health Care, Clinical Managers, resident's families, and community agencies to coordinate care from admission through discharge. Develop and maintain effective communication & cooperation with inter-departmental personnel, as well as other departments within the facility to ensure that nursing services and activities can be adequately maintained to meet the needs of the residents. Make daily rounds of your unit/shifts to ensure that nursing service personnel are performing their work assignments in accordance with acceptable nursing standards. Report problem areas to the Clinical Manager. Create and maintain an atmosphere of warmth, personal interest, and positive emphasis, as well as a calm environment throughout the unit and shift. Provide other nursing leadership functions as delegated by the Director of Nursing. Assist with the evaluation of nursing personnel to assure that quality care is provided to all residents. Maintain safe methods of drug distribution to residents on nursing station where she/he is assigned. Monitor nursing care for each resident consistent with the standards of nursing service. Maintain good personal hygiene, grooming, and appropriate dress. Follow established bowel and bladder program. Monitor and promote skin care procedures. Administer treatments and special tests, as directed by medical plan. Administer medications as ordered in the medical plan. Admit, discharge, transfer of residents. Maintain accurate and complete medical records. Report all discrepancies noted concerning residents care to case manager. Develop knowledge and understanding of the philosophy of rehabilitative nursing and demonstrate and implement special techniques used in rehabilitation. Promote activity and exercise for the resident. Prevent hazards of immobility. Assist resident to meet psychological and sociological needs. Understand attitudes needed to work with the chronically ill and geriatric persons. Understand and implement Resident's Bill of Rights, ensure staff follow Bill of Rights. Understand and perform nursing functions in relationship to environmental care. Maintain a clean environment for the resident. Follow proper infection control practices in all aspects of work. Recognize safety responsibilities of employee for the resident. Report occupational hazards and/or incidents of injury, to appropriate personnel, in accordance with facility policy and procedures. Recognize employee's responsibility for his / her own safety while on the job and for the safety of others. Recognize and remove environmental dangers. Complete maintenance service request slips for equipment in need of repairs. Include date, time and signature on your request. Apply principles of good body mechanics when lifting and moving objects. Use transfer techniques consistent with proper body mechanics. Use safety devices and transfer belts, when appropriate, and implement their use. Develop knowledge of actions to be implemented in emergency situations. Practice fire safety and accident prevention procedures. Participate in disaster planning drills (fire, tornado, severe weather, etc.) when on duty. Maintain calm manner in tense situations. Maintain awareness and adhere to policy and procedures governing the operation of Glenwood Village Care Center. Attend orientation and receive instruction regarding nursing department duties. Attend general orientation and assigned in service education programs Understand use of nursing procedure manual. Demonstrate dependable professional work habits. Adhere to requirements set forth in personnel policy manual. Report absences, according to policy. Come to work on time and when scheduled. Make suggestions, complaints, or grievances in a constructive manner. Handle work related situations regarding staff with confidentiality. Maintain good personal hygiene and grooming. Come to work clean and neat. Wear proper uniform. Scrub uniforms should be worn. Jeans may be worn on Fridays per personnel policy manual. No open toes shoes are allowed for direct care givers. Keep updated on new developments in geriatrics and gerontological nursing through continuing education and individual study. Attend and participate in continuing education programs designed to keep you abreast of changes in your profession, as well as to maintain your license on a current status.
