Facility: Dayton Children's - Main Campus Department: Social Work Schedule: Full time Hours: 40 Job Details: The Medical Social Worker is responsible for working in a team structure with the RN Case Manager and other Integrated Care department staff, to effect overall care coordination for the transition of care and complex discharge planning process. Through psychosocial assessment and intervention strategies, is responsible for assisting patients and their families in understanding and learning to cope with illness and disability. Assists families in resolving complex personal and environmental difficulties which interfere with effective social functioning and obtaining maximum benefit from medical care.
Participates as a professional team member in patient care planning along with the hospital and community agency personnel. Provides education and consultation to health care professionals and the community regarding the impact of psychosocial factors. Responsible for the delivery of social work services which are available to the patient, the patient's family, and other persons significant to the patient, are well organized and directed, and integrated with other units and departments of the hospital and community. This position will collaborate interdepartmentally with all divisions of the hospital, inpatient, ambulatory, and Emergency.
Social Workers assume additional leadership duties to help with onboarding, training, coaching and job shadowing for newer staff, participate in the department's peer mentoring program for new staff around developing social work skills and completed additional special projects as requested. LISW Social Workers are eligible to volunteer work time to provide group supervision for independent licensure preparation for dependent licensed social workers and students, as well as attend other leadership opportunities as able.

COMPENSATION:
Hourly pay ranges are competitive, based on region/geography pay scale, and are based on employee qualifications and certifications, experience, skill specializations, positive employment & job tenure history, positive professional references, and overall client & team fit.
HCN pays weekly, on Fridays, via direct deposit into your bank account.
REQUIREMENTS & QUALIFICATIONS:
1+ year of verifiable experience in home care or caregiving setting.
Successful completion of HCN's written skills test and/or competency evaluation.
Meet/Completion of all training and/or certifications required by the state in which the home health aide services are being provided. This includes specifications of contracted agencies for which services are being provided.
CPR and First Aide Certifications (active/current certs preferred but may also be obtained prior to hire date through HCN or accredited third party).
Ability to perform all main physical tasks (listed above) on a regular basis, and willingness to do so safely.
Willingness and aptitude to perform light housekeeping in addition to patient care services as needed.
Strong track record of attendance, dependability, and employment retention with previous employers.
Good interpersonal skills and compassion for helping others live their best lives.
Demonstrated ability to read, write, and follow a written Plan of Care, incorporating attention to detail in following instructions, documenting notes, and communicating with our agency.
Professional character traits: reliable, honest beyond reproach, punctual, articulate, compassionate, detail-oriented (in care + documentation), accountable, friendly and accommodating, respectful, self-driven, problem-solver, compliant, and confidential.
Be at lease eighteen (18) years of age.
Valid driver's license/ID and reliable transportation to/from client care locations.
Consent and ability to pass a State and/or Federal Background Check.
PREFERRED BUT NOT REQUIRED:
3-5 years of paid experience in a home health or caregiving setting.
Paid experience with a Home Health Agency.
Formal certification or training program, such as CHHA, CNA, STNA, or equivalent, however HCN will train non-certified caregivers committed to building a career with our agency.
Specialized certification or training program, such as DODD-DSP, Med Certification, EMR/EHR, etc.
EMR/EHR experience and proficiency (ContinuLink highly desirable).
Willingness to positively and frequently promoting Home Care Network throughout your community, social and local networks, and regional industry partners - in exchange for generous bonuses and cash incentives.
GENERAL ROLE SUMMARYA successful Home Healthcare Aide provides personal care services under the direction of the Registered Nurse or Therapist in their region. You will be assigned to specific clients by the Registered Nurse or other appropriate professionals and perform services for clients as necessary to maintain personal comfort and safety in their home.
As a Home Health Aide, you will:
Deliver direct patient care and support, following the established plan of care, to pediatric, adult, and geriatric patients.
Measure and record vital signs and other specified metrics as instructed.
Assist with personal care tasks and activities of daily living (e.g., bathing, oral hygiene, nail & skin care, shaving, dressing).
Perform household tasks essential to health care and living at home (e.g., meal preparation/feeding, laundry, light housekeeping).
Assists patients in taking medications under the direction and supervision of the Registered Nurse.
This includes opening a container or pill caddy & handing it to the patient, but does not include administration of the medications.
Identify and report any abnormal findings or changes in the patient's condition.
Help with therapy exercises under supervision of our therapists (e.g., range of motion exercises, ambulation assistance).
