Role:
The Account Development Liaison is responsible for all activities designated to increase referrals/admissions and enhancing relationships with referral sources. These activities may include, but are not limited to; education, marketing, public relations, customer service, and assistance in patient referral functions to ensure timely access to services
Qualifications:
Bachelor's degree in related field preferred or an equivalent combination of education and experience
Minimum of two (2) years of professional medical sales experience preferred (other sales experience may be considered); background in healthcare considered
Ability to promote all service lines with potential referral sources in competitive areas
Technically proficient with Outlook, Word, Excel, Power Point and other job-related software
Professional presence with excellent communication skills (written and verbal), including public speaking
Bi-lingual in Spanish preferred

Job Responsibilities:
Cultivates, expands, and maintains quality professional relationships with new and existing referral sources such as hospitals, physicians, skilled nursing facilities, assisted living facilities, home health and companion care agencies, and other referral sources.
Provides a consistent professional presence in the healthcare community with all identified referral sources within assigned territory; includes participation in appropriate clinical groups and interaction with referral sources during business hours, as well as weekends, evenings and holidays in order to meet required admission goals. Activities resulting in 90% customer facing time/travel and less than 10% administrative duties.
Responsible for ongoing territory management ensuring that assigned accounts reflect up-to-date, accurate profile information. Completes all sales call documentation in the CRM timely and with accuracy.
Responsible for the development of data driven, comprehensive quarterly sales plans that focus on strategies that result in admission and program growth.
Demonstrates creativity, innovation, and desire to create continued growth in assigned territory and accounts.
Develops and maintains a structured call rotation with all assigned accounts, preventing accounts from becoming neglected. Consistently averages greater than 50 sales calls per week.
Develops new referral sources and businesses within assigned territory.
Provides ongoing education and data updates to referral sources routinely. These include patient updates, referral/admission outcomes, educational material, and statistical analysis. Is a professional representative for the Organization and performs as an active, motivated, productive, professional and positive member of the sales team.
Represents Chapters professionally during all work-related activities, ensuring that activities are aligned with the program initiatives and goals, as determined by the program business development leader.
Provides consistent, frequent communication with referral sources including, but not limited to, organizational quality, outcomes, competencies, and clinical criteria and differentiators.
Performs at a high level as the account manager, providing exceptional customer service for all account related needs. Facilitates communication and resolution between the customer and team members.
Communicates effectively with the Director of Business Development on identified customer concerns.
Independently monitors trends related to account growth and loss and the development of new business. Quickly adjusts to self-correct where the need is identified, resulting in year-over-year growth for the territory.
Constantly strives to improve the image of the Organization in the healthcare community.

Compensation Pay Range:

The Grief Counselor provides short-term counseling support to individuals, children, and families who are coping with grief and loss issues due to a death. This may include anticipatory grief support. Services are available to hospice families as well as the larger community.
Essential Duties and Responsibilities:
Quality Provision of Services:
1. Aligns all professional practice with Capital Caring Health's mission to provide patients and their families with advanced illness care of the highest quality and in accordance with the core values of patients and families first, always do the right thing, and respect everyone.
2. Develops the Bereavement Plan of Care based upon the IDT assessment after the time of patient death, and includes the following:
a. Assesses family/caregiver grief and loss issues related to death of patient
b. Offers short-term individual/family counseling, and access to support groups, and workshops
c. Offers phone calls, condolence cards, grief literature, and special programs to meet the needs of the grieving population
d. Provides grief interventions tailored to meet the needs of the client, factoring in age and any other factors which impacts services
e. Assesses the impact of social, spiritual, and cultural issues on the grief reaction
f. Completes community-based referrals as indicated
g. Assesses for complicated grief reactions
3. Provides community based short-term individual/family grief counseling due to issues related to death, as requested.
4. Participates in the provision of education, support, and supervision to CCH intern programs.
5. Actively seeks to provide grief support/education to residents and staff of area partner facilities (ALF/SNF/INF and ICF/IMR).
6. Develops and facilitates grief support groups, workshops and special programs to meet the needs of and within assigned regional area. Collaborates these activities with staff in other regions as well as IDT.
7. Completes all Family Service On-Call shift requirements in accordance with their regional expectations.
Organizational/Regulatory Compliance:
1. Participates in all QUAPI and organizationally sponsored in-service trainings and webinars.
2. Provides Bereavement orientation and training of new staff, volunteers, and interns.
3. Maintains clinical records of clients served within electronic medical record and in accordance with CHAP/organizational compliance standards.
4. Effectively utilizes organizational documentation and tracking tools for data collection.
People/Communications:
1. Communicates effectively through both oral and written communications across settings.
2. Protects client rights to confidentiality in accordance with Capital Caring Health and HIPAA policies and regulations.

Supervisory Responsibilities:
The Grief Counselor will only perform supervisory duties when recommended and educated to do so by organizational leadership.
Experience Requirements:
The Grief Counselor must have at least 3 years of clinical counseling experience in the areas of death/dying grief, loss and survivorship. Related clinical experience in varied multi-disciplinary settings and with short-term grief counseling with individuals, couples, children, families, and groups.
Education Requirements:
The Grief Counselor must have at least a Master's degree in the areas of Psychology, Social Work, Thanatology, Expressive Arts, Mental Health Counseling or Pastoral Counseling.
Required Certificates and/or Licenses:
License eligible or Board Certified based upon degree and according to jurisdiction of practice.

The Clinical Manager is responsible for the day to day triage of the Team activities of accepting referrals, assuring timely admissions, assigning patients to the appropriate team members and monitoring patient care as ordered on the interdisciplinary plan of care. The Clinical Manager conducts the IDT meetings and supervises and supports clinical staff through evaluation and field supervisory visits and assures ongoing development of the teams' clinical skills. The Clinical Manager will assure quality care and compliance with state and federal regulations, CHAP accreditation standards, and the organization's policies and procedures.
The Clinical Manager will be responsible for supervising applicable Case Managers, RNs, LPNs, CNA/HHAs, Therapists, Social Workers, Grief and Bereavement Coordinators, and Chaplains.
The Clinical Manager must have at lease one year of Hospice/Home Health experience. A minimum of two years of management experience and at least four years of medical survival and/or oncology is preferred.
The Clinical Manager must have at least a RN from a NLN accredited nursing program. A BSN is preferred.
The Clinical Manager must have licensure from the governing body for the District of Columbia, State of Maryland, and/or the Commonwealth of Virginia.
CPR certification is required.
Essential Duties and Responsibilities:
Receives/triages inquiries from the Referral Center, staff, patients, caregivers, referral sources and the community. Performs telephone assessment to determine needs of the caller. Develops and implements a plan to address the needs by (but not limited to): instruction, dispatching appropriate staff to make a patient visit, or contacting a physician.
Assures that members of the interdisciplinary team have demonstrated the skills competencies to care for assigned patients.
Assists the Executive Director as a liaison between Hospice and other professionals, including the AMD, community physicians, hospitals, nursing homes, and agencies.
Enhances the integrity and clinical practice of the team by providing them with a consistent role model, mentor, instructor, and resource person.
Assumes responsibility for own professional growth and development in order to maintain and improve competence.
Evaluates own needs for support and uses identified system(s) to meet the need.
Develops new skills by participating in ongoing education and maintaining knowledge of current nursing practice through journals, literature review, etc.
Participates in quality improvement activities, QUAPI program and hospice sponsored inservice training. Serves on committees or teams as approved by the Executive Director.
Organizational/Regulatory Compliance:
Ensures compliance with all areas of federal, state and local laws and regulations as well as CHAP accreditation standards, Conditions of Participation, and the organization's standards of care, policies and procedures.
Communicates with the appropriate physician(s) regarding the patient's condition, incorporating hospice expertise. Implements physician's orders by documenting and communicating to staff, patient/caregiver or facility. Utilizes appropriate resource services (ambulance, IV therapy, DME, inpatient facilities, and appropriate agencies) to implement orders.
Performs the introductory (90 day) and annual performance review of assigned team members in accordance with the organization's policy and timelines.
Assures that licensed practical nurses, nursing assistants and other care assistants work under the direct supervision of a registered nurse.
Follows all rules and regulations of the facility.
People/Communications:
Assigns and coordinates daily schedules of clinical staff, including PRN staff. Facilitates the provision of patient care services on a day to day basis, according to the plan of care. Works closely with team members to meet agency productivity standards.
Coordinates all interdisciplinary team services, including on call personnel, inconjunction with the after hours department in order to assure 24 hour availability of patient care services.

Qualifications
Your Qualifications:
Bachelor's Degree or equivalent combination of education and experience, in marketing, nursing, healthcare administration, social work, business administration or related field, required; a successful track record in outside sales/marketing highly desirable.

Qualifications
QUALIFICATIONS
Active State of Maryland Nurse Practitioner License in good standing.
Active Controlled Substance and DEA license in good standing required.
Minimum of two (2) years' experience as an independent nurse practitioner in hospice, palliative, geriatrics, oncology, critical care, or another related field. Inpatient palliative or hospice experience preferred.
Board certification in Hospice and Supportive Care required within 2 years of employment.
Clean or nearly clean medical malpractice history.
Proficient EMR and Microsoft Office technology.
Eligible for provider enrollment in Medicare and Medicaid.
Demonstrated understanding of and a commitment to the philosophy of Hospice and Supportive Care, Familiarity with hospice, home care and adult social services.
Fluent in the ethical and legal issues surrounding end of life care and prescribing controlled substances.
Excellent interpersonal, organizational, written and verbal communication skills required.

We are currently seeking a talented, compassionate LPN to support and maintain communication between patients, families, case management nurses, field teams, and the Rapid Response team.
This position is Full-time, 3 12-hour shifts, with a schedule of Friday, Saturday and Sunday 8am - 8:30pm.
This position is Non-Exempt, and eligible for Shift Differential.
Main Responsibilities:
Triages all calls and routes the caller to the appropriate resource.
Applies critical thinking and clinical judgment to discern urgent vs non-urgent needs and requests and prioritizes accordingly, with all requests being addressed within the same shift.
Receives verbal orders from Physicians (i.e., medications, DNR status and EMS forms), and provides education to patients and families relative to order changes, as necessary.
Places orders for medication and DME (durable medical equipment) when needed based on patient needs and physician's orders.
Instruct families on how to manage patient's symptoms using the medications listed on the patient's medication list until the field nurse can be contacted or a Skilled Nursing Visit (SNV) can be arranged if necessary.
Conducts timely and clinically appropriate virtual visits using telehealth platforms to assess, manage, and support symptom management and education needs.
Manages deployment of Rapid Response staff, as necessary, to support urgent patient needs in the event that a field nurse is not available.
Maintains documentation for patient encounters in triage platform, EMR, and Rapid Response call log.
May be required to go out into the field to conduct Skilled Nursing Visits, as needed. Field visits may include visits to patients in facilities, hospitals and home settings, including visits to our pediatric population.
Other duties as assigned or requested by the Supervisor, Support Services or the case managers on a case-by-case basis (e.g. lab supplies and wound care supplies).

Qualifications
Your Qualifications:
Possess a current Maryland practical nursing license (LPN) and thorough knowledge of nursing principles with a minimum of 2 years of experience.
Must have excellent communication skills and be able to assess patients' needs (i.e. physical, teaching, mental and spiritual) and give direction after speaking with the caregiver on the phone.
Ability to recognize safety issues to self and others and be able to assess and implement use of lifts or other personnel to lift and move patients of all sizes and weights in a safe manner.
Ability to perform consistently in stressful situations.

Role:
Working in collaboration with other members of the pediatric interdisciplinary team/group, the PRN Pediatric Social Worker provides support within the scope of social work practice for hospice and concurrent care pediatric patients regionally and provides assistance in completing initial, routine, and emergent visits as needed.
