Your role involves:
Providing direct patient care to inpatients in need of hemodialysis.
Supervising care and treatments, conducting patient assessments, creating personalized care plans, and addressing patient concerns.

* Initiates the plan of care when directed and requests from prescriber any necessary revisions as patient status and needs change
* Incorporates therapeutic, preventive, and rehabilitative nursing actions based on patient diagnosis and established goals
* Intervenes as applicable in response to changes in patient/client condition and reports such changes to appropriate physician or supervisor
* Administers medications and treatments as prescribed, as well as, monitors and documents effectiveness of intervention
* Reviews plan of care and program protocols and identifies areas for healthcare teaching/education, care coordination, goal setting and discharge planning needs, as applicable
Requirements:
* Current Registered Nurse (RN) License for the state in which the nurse practices.

The Afterhours Care Manager RN is responsible for identifying and coordinating patient/family needs after hours (or during alternative scheduling) and for maintaining primary control and professional management of each patient assigned during working hours.
+ Managing patient needs includes performing routine visits, admissions, death visits, on-call or after hours visits, and/or any visit or duty assigned in accordance with the patient's plan of care.
+ Assumes after hours coverage for the designated teams and functions as an interdisciplinary (IDG) team member to provide care and liaise between physician, patient/family and hospice team.
+ Performs clinical administrative tasks including workflow completion and auditing, survey readiness tasks, and other duties as assigned.

Education/Experience:
+ Current RN licensure in the state of employment

Our Physical Therapy Assistants work under the direction of our Physical Therapists and treat patients with physical impairments through the use of physical modalities. The goal is to assist persons who are physically challenged to improve mobility and function, independent self-care, other skills necessary for functioning in daily living.
