JOB DESCRIPTION:
SUMMARY OF POSITION
As a school-based behavioral health liaison, you will be responsible for providing face to face consultation with classroom teachers to assist them in creating positive, trauma-informed classroom environments, to enhance the learning environment for children at risk of developing an emotional/behavioral problem or substance use disorder, or with serious emotional disturbance (SED). Training and education regarding childhood mental health and trauma-informed behavior responses in the classroom will also be incorporated in the consultation to assist the teachers in promoting a healthy teaching environment. Each SBBHL shall provide psycho-educational groups and other support as needed to youth identified by teachers and/or or other school officials. The SBBHL program will provide Liaison support between the family and the school and will, when invited, assist parents and youth with the Individualized Education Plan (IEP) process and with trauma-informed support plans.
This position is located at Eastside Elementary and Cartwright Elementary. School placement may be subject to change based on needs of school district.
ESSENTIAL DUTIES & RESPONSIBILITIES
Provide face-to-face consultation with classroom teachers to assist in creating a positive, trauma-informed classroom that enhances the learning environment, and to assist the classroom teacher in developing effective, trauma-informed behavior responses for the service population and other students in the class.
Provide at least one face to face training per year to LEA personnel on a variety of mental health and substance abuse prevention topics.
Assist in the development of school wide mental health initiatives and universal prevention strategies.
Provide Liaison services to include communication between the school and the child's family to build open lines of communication and to build home-school partnerships. Liaison services can include assisting in the IEP process when requested, assisting in the alternative school placement process, developing trauma-informed support plans collaboratively with the school and families of the students to meet student individual needs, etc.
Provide information and support for school personnel in navigating the local behavioral health system, including substance abuse and mental health services, and accessing mental health crisis services.
Maintain logs, tracking documents or other appropriate management tools needed to track all aspects of these services. The State will review these documents during the annual monitoring visit.
Ensure that each Liaison provides support opportunities to students at each school to include individual student consultations or interventions to assist in building positive coping and de-escalation skills, as needed; and at least three psychoeducation group activities per year that cover the following topic(s): mental health problems and disorders; the risk of substance use/abuse and substance use/abuse disorders; healthy ways to deal with stress including mindfulness, coping and de-escalation strategies; and other topics as decided and requested by each school.
Address specific mental health or behavioral issues through individual, group, and/or family therapy services. Formulate a diagnosis upon completion of intake assessment. Create and update treatment plans according to policy.
Maintain timely and accurate documentation, including treatment plans, progress notes, and report, to comply with grant and organizational requirements.
Compliance requirement for bi-annual completion of performance evaluation.

QUALIFICATIONS
Education Level
A Master's degree is required in one of the following fields: Clinical Psychology, Mental Health (MH) Counseling, Social Work, Marriage and Family Therapy, and/or a specialization in MH Counseling
License-eligible in respective discipline preferred.
OR
Applicants with a Master's degree in a related field (outside of the disciplines listed above) must meet the following criteria:
Completion of at least 24 graduate-level semester hours in psychology, social work, counseling, or other behavioral health-related coursework.
Relevant coursework and work experience in the following areas:
Clinical treatment and treatment planning
Appraisal and assessment of mental disorders
Psychopathology
Use of the DSM for diagnostic purposes within a clinical internship or practicum in a behavioral health setting
Years of Experience
A minimum of one (1) year of experience providing treatment to individuals with mental health disorders is required, and substance use disorders experience preferred.
Internship experience may be applied toward the minimum experience requirement for recent graduates.
Certification/Licensure
Must possess a valid driver's license and meet eligibility requirements to be covered under the organization's vehicle insurance policy.

SUMMARY OF POSITION
Jump start your career in the mental health field as a Psychosocial Educator. Work within a team environment to provide clinical psychoeducation to Centerstone eligible program participants. The Family Development Specialist supports clients and families by developing and implementing treatment plans, providing therapeutic interventions, and linking them to necessary services. The primary responsibilities of the Integrated Care Coordinator are to deliver comprehensive, person-centered care by planning, coordinating, and monitoring individualized treatment plans to align with behavioral health goals. They play a pivotal role in closing gaps, tracking progress, and upholding the highest standards of quality and regulatory compliance. Assist the Nurse Practitioner with clinic appointment related documentation and facilitation on site when working in the clinic. Ensure clients receive required physical exams, medication monitoring, and appropriate services.
Maintain medical record compliance and ensure timely documentation of care coordination activities.
Monitor HEDIS gaps and verify client payer and program enrollment status monthly.
Develop individualized, person-centered care plans incorporating assessment results and Division's guidelines, focusing on unmet health needs and Social Determinants of Health (SDOH).
Coordinate follow-up services for recent hospitalizations or life transitions, ensuring smooth transitions of care.
Identify and provide crisis response as necessary, participate in post-crisis debriefing, and be available for on-call support.
Communicate effectively with individuals, providers, and natural supports, providing education on services.
Establish collaborative relationships with care team members and community resources to improve resource linkage and documenting follow-up.
Support transitions between care settings and develop comprehensive discharge or transition plans.
Attend Treatment Team and supervision meetings, integrated care team meetings, and serve as a liaison with other professionals and agencies.

Qualifications

JOB DESCRIPTION:
SUMMARY OF POSITION
Implement curriculum in classroom and/or group settings within schools, community venues and/or systems of care (i.e. detention centers, group homes, etc.). Assist in the coordination and delivery of community based grant activities with children/youth, parents/caregivers and other members of identified service areas. Attend meetings and trainings. Regular and reliable attendance is required. Ability to travel overnight will be required upon occasion (expenses paid).
ESSENTIAL DUTIES & RESPONSIBILITIES

Maintain required written documentation and submit required reports within the specified period of time.
Utilize behavior-management techniques appropriately in classroom and/or group settings.
Relate to the community, (i.e. school staff, professionals, parents/caregivers, and youth) positively while demonstrating professionalism at all times.
Attend meetings, conferences, and other training sessions, as required. Maintain a team approach with peers, coordinator and manager by working cooperatively on behalf of the prevention programs, and the children/youth served.
Demonstrate competency in presenting curricula as evidenced by positive Site Reports and Evaluator/Supervisor Observations.
Adhere to program standards regarding participant contacts as dictated by Centerstone and funding organization.

Effectively communicate via written, verbal, in person and virtual methods.
Able to work independently with an “off-site” supervisor.

Provides support, assistance, psycho-social education and supervision to Adult Residential Services residents.
HOURS
This position is scheduled for Thursdays from 12:00 PM to 6:00 PM, Fridays from 9:00 AM to 7:00 PM, and Saturdays and Sundays from 7:00 AM to 7:00 PM.
ESSENTIAL DUTIES & RESPONSIBILITIES
Supervise and monitor residents' compliance with Program rules and behavioral expectations. Re-direct residents on non-compliance with rules and expectations and refer major incidents of non-compliance to the Residential Coordinator.
Supervise, educate on and monitor residents' self-medication procedures and document in residents' charts.
Responsible for keeping file and closet locked while on duty.
Review referral information as it is presented to help determine eligibility for admission and continued stay for Supported Housing and Crisis Respite where relevant Complete authorizations for stay as needed with each Managed Care Company
Supervise meal preparation and compliance of residents with dietary guidelines, providing education as needed.
Receive and dispense residents' funds from locked filing cabinet in office as needed and document all money received and given to residents in residents' charts.
Plan and supervise psycho-educational activities recreational activities during residents' free time and provide van transportation on outings.
Inform Residential Coordinator of all medical emergencies, crisis situations with residents or any disruptions in program operation.
Receive briefing at beginning of shift from Residential Coordinator or other staff and provide briefing to other staff at end of shift.
Document a progress note on each resident once per shift and at any time there is a significant incident.
Attend all Program staff meetings and all scheduled staff development activities.
Conduct room inspections as needed to monitor and educate on cleanliness of rooms, residents' compliance with hygiene/grooming and household task completion.
Each residential program within Adult Residential Services will provide a safe, comfortable supervised housing option for mental health consumers, and assist consumers in reaching and maintaining stabilization of symptoms, while encouraging consumers to assume increasing responsibility for their lives.

JOB DESCRIPTION:
SUMMARY OF POSITION
This position provides in home visitation services to enrolled participants, providing linkage and referral to other services and support to enhance family functioning by: building trusting relationships, teaching problem-solving skills, and improving the family's support system.
ESSENTIAL DUTIES & RESPONSIBILITIES
Responsible for initiating and maintaining regular and long-term (up to five years) contact and support with families. This activity will occur primarily within the family's home; with visits lasting around an hour.
The interventions should be family-centered, strength-based, and directed at establishing a trusting relationship; strengthening the parent-child relationship; promoting healthy childhood growth and development; and enhancing family well-being by reducing risk and building protective factors.
Partners with families and honors diverse family structures and parenting practices.
Activities may also include but are not limited to: administering the Family Resilience and Opportunities for Growth Scale (FROG) Scale; administering screening tools such as the ASQ, ASQ-SE, Depression Screens etc.; identifying and referring families for other supportive services, including health care services.
Responsible for assisting the family in establishing goals and supporting them throughout this process and is responsible for implementing activities outlined on the Family Service Plan to address areas of concern that were identified on the FROG scale.
Assist in preparation of required monthly, quarterly and annual statistical reports.
Maintains up-to-date and accurate records for families on GGK curriculum, HFA data, and other required records.
Participate in training according to requirements of HFA guidelines, GKI and DOH contract.
Implements GGK curriculum and HFA strategies to enrollees through weekly home visits.
Provides case management to families on caseload.
Attend training and other mandatory meetings.

SUMMARY OF POSITION

As a school-based behavioral health liaison, you will be responsible for providing face to face consultation with classroom teachers to assist them in creating positive, trauma-informed classroom environments, to enhance the learning environment for children at risk of developing an emotional/behavioral problem or substance use disorder, or with serious emotional disturbance (SED). Training and education regarding childhood mental health and trauma-informed behavior responses in the classroom will also be incorporated in the consultation to assist the teachers in promoting a healthy teaching environment. Each SBBHL shall provide psycho-educational groups and other support as needed to youth identified by teachers and/or or other school officials. ESSENTIAL DUTIES & RESPONSIBILITIES
Provide face-to-face consultation with classroom teachers to assist in creating a positive, trauma-informed classroom that enhances the learning environment, and to assist the classroom teacher in developing effective, trauma-informed behavior responses for the service population, as defined in Section A.4., and other students in the class.

Provide information and support for school personnel in navigating the local behavioral health system, including substance abuse and mental health services, and accessing mental health crisis services.
Maintain logs, tracking documents or other appropriate management tools needed to track all aspects of these services. The State will review these documents during the annual monitoring visit.
Ensure that each Liaison provides support opportunities to students at each school to include individual student consultations or interventions to assist in building positive coping and de-escalation skills, as needed; and at least one (1) group activity that covers the following topic(s): mental health problems and disorders; the risk of substance use/abuse and substance use/abuse disorders; healthy ways to deal with stress including mindfulness, coping and de-escalation strategies; and other topics as decided and requested by each school.
Administer pre- and post-tests and satisfaction surveys as prescribed by the State.

SUMMARY OF POSITION
Implement curriculum in classroom and/or group settings within schools, community venues and/or systems of care (i.e. detention centers, group homes, etc.). Assist in the coordination and delivery of community based grant activities with children/youth, parents/caregivers and other members of identified service areas. Attend meetings and trainings. Job Title: Care Manager I, Child & Youth (Manchester, TN)
Schedule: Mon-Fri, 8:00a-4:30p
Salary: $41,000 (base pay) with additional incentive for language skills and behavioral health experience. Mileage reimbursement available.
JOB SUMMARY: Provide comprehensive care management services to children and youth with SED and their families and / or to adults with SPMI. Develop relationships with consumers and assist them in overcoming challenges to live healthy, successful lives in the community. Collaborate with interdisciplinary team and community partners to provide innovative solutions to help consumers achieve their goals.
RESPONSIBILITIES:
Providing quality consumer care
Engage with consumers monthly to assess needs, develop and address care plan goals, and coordinate care.
Complete visits with consumers in the community (home, school, court, etc) to evaluate needs and connect with individuals where they are.
Address mental health, physical health, social determinants of health, gaps in care, and ED/IP utilization.
Provide side by side approach to consumers in evaluating and addressing barriers to engagement in treatment.
Assist with referral and linkage to community resources and advocate for consumers whenever necessary.
Collaborate with consumers' support system and identify / increase supports when needed.
Follow training handouts and policies to ensure consumer care is delivered with utmost quality.
Ensure timely and comprehensive documentation as informed by agency policies and procedures.
Meet productivity standards as indicated in the policies and procedures.
Collaborate with team members and interdepartmentally
Actively participate in daily team meetings, coming prepared and ready to connect with team members to share insight, offer innovative solutions, and develop action plans. Follow up with plans developed in meetings.
Participate in monthly supervision, coming prepared to discuss caseload, engage in professional development discussions, and receive feedback.
Develop and maintain collaborative relationships across departments.
Develop and maintain collaborative relationships with community partners.

REQUIREMENTS:
Bachelor's Degree in a health-related field of counseling, psychology, social work, or other behavioral health field required.
Candidates with a bachelor's degree in criminal justice must have fifteen college level semester hours of coursework in behavioral health.
