Position Summary:
The Chief Information Officer (CIO) provides strategic and operational leadership for information technology, cybersecurity, and enterprise reporting systems across the organization. This executive-level role is central to the Health Center's modernization initiatives, including the technical implementation of Epic as its new electronic health record (EHR). This is a chance to lead a dedicated team and shape the future of the Health Center.

• Assesses clients and/or family psychosocial status, social work needs and living conditions utilizing professional knowledge, skills of observations and interviewing skills.
• Establishes the social work component of the patient/family plan of care based on goals mutually acceptable to the client, family and significant others. Makes referrals to other community services, as necessary.
• Travels to patients' homes and/or other facilities with varying environments (e.g., elevated buildings, walk-ups, care facilities, single/multiple family homes, presence of pets, etc.) using approved transportation options to deliver direct service to the client.
• Provides psychosocial work services to patient and/or family, including short-term individual counseling, community resource planning, and crisis intervention. Responds to emergent psychosocial patient and family needs, as requested.
• Provides advocacy to patient and/or family, e.g., assistance in obtaining entitlements and community services.
• Provides social work services in accordance with VNS Health policies, practices, procedures and Standards of Social Work Practice, which may require standing, stooping, sitting, crouching, bending and stretching to deliver patient care, as needed.
• Initiates and maintains verbal and written communication according to VNS Home Care policy, including the preparation of clinical and progress notes, to ensure optimal quality care.
• Manages social work planning for a caseload of patients prescribed by the physician and other team members from assessment to discharge. Maintains productivity sufficient to meet program goals.
• Assists the physician and other team members in understanding the significant social and emotional factors related to the patient's health problems.
• Participates in the development of treatment plans and revises the goals as needed. Coordinates approaches to patient and/or family care with other team members.
• Consults with and educates the patient and family regarding the treatment plan, self-care techniques and prevention strategies.
• Utilizes appropriate community resources and serves as a liaison between VNS Health and other community agencies.
• Participates in discharge planning.
• Assumes responsibility for continued professional growth, such as in-service programs.
• Transports and utilizes VNS Health designated/supplied carrying case weighing up to 30 lbs. (as needed) to and from patient homes/care facilities, VNS Health offices and other locations.

Medication Administration
Provide the highest degree of quality care and services by administering medication and treatments in a safe organized manner.

Count all narcotics with another Medication Care Manager (MCM) or Lead Care Manager (LCM) each shift and report discrepancies to the RCD or Wellness Nurse.
Maintain and clean the Medication Room, medication carts, and treatment carts for neatness, cleanliness, availability of medications, and expired medications.
Follow re-fill process for medications.

Full-Time Medical Assistant - OB/GYN Reports To: Chief Nursing Officer 131-72 40th Road Queens, New York, 11354 United States Responsibilities: * Direct patient care and support * Perform nursing tasks under the direction of the registered nurse or authorized health care providers
* Interview patients and records medical history, take vital signs, and prepare patients for medical visits
* Conduct health/risk screening using standardized tools
* Collect laboratory specimens as ordered
* Assist providers in procedures using sterile or aseptic techniques
* Promote patient self-management by providing patient education, appointment reminders and support in collaboration with the care team
* Facilitate communication for patients and health care providers by providing translation or interpretation as needed
* Complete accurate EMR documentation of patient information and services provided
* Effective teamwork, communication and collaboration as part of practice-wide care team
* Demonstrate curiosity and understanding of patient registration, administrative workflows and prior authorization processes
* Collaborate with the care team to conduct pre-visit planning and post-visit follow-up
* Participate in Quality Improvement (QI) projects and other related activities
* Maintain a cooperative relationship among health care teams by communicating information; responding to requests; and participating in team problem-solving
* Adapt to changes in practice operations, workflows and patient needs as they emerge
* Perform other duties as assigned
Minimum Qualifications:
* High School Diploma and 2 years of work experience
* Associate's Degree preferred
* Work experience in the health care field preferred
* Medical Assistant Certification preferred
* Basic CPR (BCLS) Certification required (Health Center will provide if needed)
* Bilingual in English/Chinese required
* Excellent interpersonal skills, ability to work as part of an interdisciplinary team
* Good problem solving skills and interested in learning all aspects of the practice
Compensation and Benefits:
* Salary: $40,000.00 - 46,000.00 per year
