Essential Functions & Responsibilities: Performs the following duties of the unit dose delivery system: * Processes a first dose on the robotic dispensing system. * Checks dispensing * Checks medications for the automated dispensing machines. * Checks unit dose packages. * Charges medications to patients accurately. * Operates the pneumatic tube delivery device. * Checks medication boxes and crash carts. * Operates the controlled substance dispensing machine and handles controlled substances in accordance with the department's policies. * Prepares or supervises the preparation of sterile products using aseptic technique and in compliance with the departments policies and procedures. * Prepares or supervises the preparation of total parenteral nutrition (TPN) solutions. * Prepares or supervises the preparation of cytotoxic agents in compliance with the department's safety procedures. * Documents medication events, adverse drug events, controlled substance variances, quality assurance activities, and clinical interventions. * Documents in the patient medical record in a manner that is clear and legible. * Reviews, verifies, and enters medication orders including timely discontinuation of therapies as ordered accurately into the pharmacy computer system. * Facilitates medication availability and distribution on the units. * Handles hazardous material in compliance with the department's safety procedures. * Assist the Inventory Control Coordinator in stocking medications and maintaining inventory. * Responsible for IV to PO streamlining and conversion when clinically appropriate. * Responsible for Automatic Therapeutic Interchange for approved medications. * Responsible for renal dosing Responsible for kinetic and clinical dosing. * Responsible for poly-pharmacy screening and intervention when clinically appropriate. * Participates in assigned multidisciplinary patient care or discharge rounds when appropriate. Assist pharmacy liaison with PA submission, appeal letters, insurance troubleshooting, etc. Participate in clinics, case conferences, tumor boards, and multidisciplinary rounds as appropriate. Provide evidence-based pharmacotherapy recommendations aligned with pediatric oncology protocols and guidelines. Support specialty pharmacy dispensing workflows for hem/onc therapies, including limited distribution drugs and REMS programs. Facilitate medication access by assisting with prior authorizations, appeals, manufacturer programs, and patient assistance resources. Coordinate with payers, manufacturers, and specialty distributors to prevent delays in therapy initiation or continuation. EDUCATION: A PharmD or bachelor's degree from an accredited College of Pharmacy is required. Completion of PGY-1 Pharmacy Residency or 3+ years working as a clinical pharmacist in direct patient care is required. Completion of a PGY-2 in oncology/hematology, transplant, or pediatrics is preferred. CERTIFICATES, LICENSES, REGISTRATIONS REQUIRED: An active Pharmacist licensure (in good standing) to practice pharmacy in DC or MD with the eligibility to be licensed in both within 90 days of hire Board Certified Oncology Pharmacist (BCOP), preferred Board Certified Pediatric Pharmacist (BCPP), preferred Must possess or be able to obtain a State Board of Pharmacy Preceptor's license by the current state you are applying to within one year of hire. EXPERIENCE: 2+ years of work experience in ambulatory care pharmacy setting post PGY1 training or 3+ years post PharmD is preferred. KNOWLEDGE AND SKILLS: Demonstrated expertise in medications, drug interactions, medical procedures, therapies, diagnoses, and disease states is essential. Exceptional interpersonal skills for effective communication with diverse stakeholders including hospital staff, patients, families, physicians, and colleagues. Ability to effectively manage multiple tasks, prioritize duties, and adhere to deadlines. Organized work approach with a proficiency in maintaining accurate documentation. Strong verbal and written communication skills; proficiency in additional languages is advantageous. Self-directed with strong self-management abilities; capable of identifying and addressing issues independently. Proven track record in process improvement and a keen eye for detail and precision. Proficiency in Microsoft Office programs (e.g., Word, Excel, Outlook) and web browsers. Special Working Conditions (On-call, travel, shift, coverage): This position includes a remote, weekend on-call rotation once every four weeks. Minimal travel required for this position.
