Work schedule will be Monday - Friday from 9:00am - 6:00pm, with the ability to stay later based on business needs. This position will work 5 days a week in the Lenexa, KS Home Infusion branch. Will be required to participate in the On-call Pharmacist rotation.
Primary Responsibilities:
Reviews medication orders and evaluates appropriateness and completeness of orders with consideration given to the special needs and behaviors of the specific age group served. Obtains patient prescription and assists physician, as necessary, in formulating prescriptions and monitoring regimen. Enters medication orders into the computer system and dispenses medications in an accurate and timely manner in appropriate quantities based on stability, compatibility of the drug and age specific needs of the patient. Verifies all prescriptions not directly received from the physician or the physician's agent
Assesses the patient at the initiation of drug therapy, develops a patient care plan and drug monitoring regimen and monitors the patient according to the care plan throughout the course of therapy
Communicates with physicians in suggesting alternate or modified drug therapy programs that are the most cost effective. Communicates with the patient, physician, nursing staff, pharmacists and other members of the health care team regarding patient status, response to therapy and clinical objectives to ensure clinical outcomes is met
Coordinates with nursing staff to assure the most cost-effective method of delivery is utilized to achieve expected outcome
Information is input into the computerized software system to ensure accurate and timely dispensing and billing
Coordinates the drug preparation work of pharmacy technician and checks the accuracy of that work prior to releasing the medications for distribution. Accurately checks medications prepared by pharmacy technicians
Where applicable, compounds pharmaceuticals in an aseptic fashion according to literature guidelines. Compounds specialty drug products. Uses aseptic technique when compounding sterile drug products. Completes accurate documentation of items compounded and repackaged. Is familiar with, and strives to meet, various guidelines and recommendations as put forth by various governing bodies
Be knowledgeable with regards to the proper use and handling of sterile and hazardous medications. Compounds and handles sterile and hazardous medications in accordance with established guidelines and regulations (i.e. USP )
Ensures compliance with all FDA, DEA, State Board of Pharmacy, OSHA and other regulatory standards. Follows all department policies and procedures. Participates in department staff meetings and other functions required in the department, maintains license, continuing education and competencies as required by the appropriate regulatory agency and department
Complies with all applicable Federal, State pharmacy rules and regulations related to dispensing, controlled substances, etc.
Ensures that all manufacturer requirements for REMS and/or other requirements are met when dispensing and/or shipping drugs
Provide oversight of the safety; appropriateness, and effectiveness of the pharmaceutical care provide to patients (consumers) as it relates to specialty products dispensed

Bachelor in Science of Pharmacy or Doctor of Pharmacy degree
Current and active KS Pharmacist License, without any restrictions
Ability to participate in the on-call rotation
Reside within a commutable distance to the Lenexa, KS branch
Preferred Qualifications:
1+ years of infusion Pharmacist experience
Acute Pharmacy Compounding experience
Home Infusion and/or Hospital experience
Proven verbal and written communication skills, interpersonal relationship skills and problem-solving skills

Basic Qualifications

Overview
The Clinical Pharmacy Specialists provides pharmaceutical care to all patients consistent with the goals and objectives of the hospital and the pharmacy department. The **Clinical Pharmacist** is responsible for managing the selection and utilization of pharmaceuticals and supports core clinical programs such as DUR, DIS and formulary management.
**Primary duties may include, but are not limited to:**

+ Serves as a clinical resource to other pharmacists on areas such as prospective, inpatient and retrospective DURs and provides dosage conversion and clinical support for therapeutic interventions.
+ Prepares information for network physicians.
**Minimum Requirements:**
+ Requires BA/BS in Pharmacy.
+ Minimum of 2 years of managed care pharmacy (PBM) experience or residency in lieu of work experience; or any combination of education and experience, which would provide an equivalent background.
+ Requires a registered pharmacist.
+ Must possess an active unrestricted state license to practice pharmacy as a Registered Pharmacist (RPh).
+ For associates working within Puerto Rico who are member or patient facing either in a clinical setting or in the Best Transportation unit, a current PR health certificate and a current PR Law 300 certificate are required for this position.

* Draw and prepare radiopharmaceutical doses, including low-energy isotopes, in accordance with customer prescriptions and regulatory standards.
* Compound radiopharmaceuticals by eluting generators, combining materials, and ensuring accurate formulation and dosing.
* Conduct constancy checks on dose calibrators and single-channel analyzers (SCA) to ensure accurate measurements.
* Perform routine equipment checks and calibrations on survey meters, SCAs, and dose calibrators to maintain operational integrity.

Project Profile
The CMOP program provides prescription mailings for veterans and other patients. Services being sought will be utilized to assist in supporting CMOP's mission by preparing prescriptions and packages in an accurate, timely, and cost-effective manner without disruption in services or production.
The Candidate
ProSidian seeks a Registered Pharmacist to support engagement with The Veteran's Affairs Consolidated Mail Outpatient Pharmacy (CMOP). This position serves as a key member of the facility leadership team providing services that include, but are not limited to, pharmaceutical verification of prescription orders of legend, over the counter (OTC), medical-surgical (Med/Surg), and controlled substances to patients requiring care. In addition, the candidate shall provide patient consultation, quality improvement, and supply control duties.
Furthermore, the candidate shall direct the delivery of pharmacy services, efficient and effective drug distribution, and education to improve patient quality of life, minimize therapeutic risk, and enhance clinical outcomes. The candidate's leadership attributes in Pharmacy management shall ensure the delivery and attainment of patient care quality metrics, evaluate the performance of Pharmacy Technicians duties, and drive client satisfaction outcomes.
This is a Contract Contingent position and the Registered Pharmacist services shall be performed at the: Veteran's Affairs Consolidated Mail Outpatient Pharmacy (CMOP) facility at: 5000 S 13th St, Leavenworth, KS 66048. Qualifications
Education:
Bachelor of Science in Pharmacy degree that is recognized by the Accreditation Council for Pharmacy Education (ACPE) or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained.
Licensure:
A current, active, valid, and unrestricted license to practice pharmacy in the State of Kansas or in possession of an active license from another state that enjoys reciprocity with the state of Kansas.
The candidate must have passed the North American Pharmacist Licensure Examination (NAPLEX) and the Multistate Pharmacy Jurisprudence Examination (MPJE).
Experience:
Demonstrable proficiency in the compounding of prescriptions, drugs, chemicals, and pharmaceutical supplies required in the operation of a pharmacy;
Dispense medications prescribed by physicians and other health practitioners and providing information to health practitioners and patients about proper usage of medications and side effects;
Conduct drug utilization reviews, counsel patients on the proper use of medication while evaluating medication use patterns and outcomes for patients leveraging adherence best practices;
Apply knowledge of state and federal laws and regulations pertaining to the legal operation of a pharmacy to comply with laws, regulations, and policies governing pharmacy operations;
Provide oversight of CMOP facility by developing and implementing operational procedures, maintain accurate records and document actions taken while safeguarding the confidentiality of medical records;
Apply inventory control expertise in establishing medication-handling procedures for the storage and preservation of medications;
