Basic Qualifications

Job Summary
The ClinicalPharmacist plays a key role in advancing population health initiatives and optimizing performance across value-based care (VBC) programs. This role will support community health centers (CHCs) and contribute to clinical programming, quality improvement, and risk adjustment initiatives within the Clinically Integrated Network (CIN), Medicare Shared Savings Program, Medicare Advantage, and other VBC contracts.

This role provides clinical oversight and strategic direction for medication optimization, adherence interventions, and pharmacy technician workflows. The ClinicalPharmacist also plays a key role in supporting Medicare Advantage (MA) contracts, including performance on Stars and HEDIS measures, through evidence-based medication management and quality improvement initiatives.
What You'll Do
Chronic Disease Management:
Provide clinical oversight for medication management for patients with diabetes and hypertension.
Optimize medication regimens based on clinical guidelines, patient-specific factors, and barriers to adherence.
Conduct comprehensive medication reviews, including identification of polypharmacy risks, drug interactions, and therapeutic duplication.
Support clinical teams in medication titration protocols under collaborative practice agreements, where applicable.
Consult with providers on complex medication regimens and serve as a clinical resource on chronic disease pharmacotherapy.
Remote Patient Monitoring:
Actively manage patients enrolled in RPM by reviewing blood pressure and glucose readings, conducting patient outreach to address abnormal values, providing education and coaching, and supporting medication changes as needed.
Establish medication adjustment workflows aligned with RPM data and escalation protocols.
Review RPM dashboards and trends to proactively identify patients needing intervention.
Collaborate with care teams and providers to act on RPM alerts and abnormal readings.
Support documentation and billing workflows related to RPM services.
Medication Adherence and Pharmacy Technician Collaboration:
Oversee pharmacy technician workflows supporting medication adherence calls, refill alignment, outreach, and education.
Design, implement, and refine adherence strategies (e.g., refill sync, cost mitigation, side-effect counseling).
Prioritize patients for adherence support using pharmacy claims data, risk stratification lists, and payer adherence thresholds.

Medicare Advantage, Stars & HEDIS Quality Program Support:
Understand medication-related quality measures (i.e., Statin Use in Persons with Diabetes, Diabetes Control, Controlling Blood Pressure).
Identify performance gaps and partner with clinical and network teams to design pharmacist-led interventions that improve quality metrics.
Support documentation improvement and close care gaps through chart review, patient outreach, and provider education.
Participate in payer quality meetings, program evaluations, and reporting cycles as needed.
Data, Reporting, & Quality Improvement:
Use clinical, claims, and RPM data to identify trends, gaps, and opportunities for improvement.
Develop and track outcome metrics related to medication optimization, adherence improvements, and chronic disease control.
Contribute to dashboards, scorecards, performance reviews, and reporting for internal and external stakeholders.
Participate in quality improvement (QI) projects and support the design of scalable pharmacist-driven interventions.
Interprofessional Collaboration:
Work closely with physicians, advanced practice providers, care managers, and medical assistants to coordinate patient care.
Provide education on pharmacotherapy best practices, new medications, and guideline updates.
Participate in provider engagement meetings.

Position Summary
The staff pharmacist processes prescriptions, oversees and directs pharmacy technician workflow, provides pharmacological information to multidisciplinary health care teams and patients, and identifies and solves day-to-day operational issues.
Essential Duties
Reviews, interprets, clarifies, and enters prescriber orders/prescriptions.
Prepares all forms of medications using accurate calculations and proper procedures.
Maintains and improves operational efficiency and accuracy by training, organizing and directing pharmacy technician workflow. Verifies the accuracy of their order entry, preparation, and labeling of prescriptions.
Listens attentively and responds promptly to customers' needs, requests and concerns in a respectful manner.
Utilizes available information to track and understand each patient's medication history and make recommendations to prescribers as the situation requires.
Provides pharmacological information by answering questions and requests of health care professionals and counseling customers on drug therapies.
Maintains safe, clean and organized work environment.
Collaborates with research staff on investigational drug research. Maintains accurate records, dispenses and labels per research protocol.
Performs competencies and assigned training prior to deadline. Completes professional continuing education as required by rules and regulations of governing body.
Provides after hours services to hospital personnel as requested.
Assists Director of Pharmacy in implementation of department initiatives.
Other Duties
Checks accuracy and expiration dates, and stocks Pyxis machines as needed.
Checks customers out at register efficiently and accurately.
Inventories, stocks and places orders for the pharmacy stock as needed.

As a Pharmacist you will:
Evaluate physician medication orders for drug appropriateness, correct dosage, potential drug interactions, and proper route of administration.
Provide clinical recommendations to physicians and facility staff regarding medication therapy changes and optimizations.
Maintain up-to-date patient/resident profiles, including medical history, lab results, and current medications (prescription, OTC, and home remedies).
Ensure laboratory tests required for monitoring high-risk medications are appropriately ordered, reviewed, and communicated to the healthcare team.
Collaborate with physicians, nurses, and other healthcare professionals to provide clinical and technical support.
Document all communications and clinical interventions in the patient's medical record, including interactions with families and caregivers.
Implement and enforce policies and procedures for the safe procurement, storage, administration, and disposal of medications and biologicals.
Ensure compliance with all state and federal regulatory requirements related to pharmacy practice and medication management.
Report medication-related errors and adverse drug events to the Pharmacy Manager as required.
Educate and support facility/agency staff on safe and effective medication handling procedures.

Required:
Bachelor's Degree in Pharmacy, Pharm D preferred
One year experience in long term care
License/Certification(s): current and valid pharmacist license in the state of practice

Proficiency in electronic medical records and pharmacy information systems
Strong clinical judgement, and effective written and verbal communication

About the Role:
The PRN Pharmacist is tasked with ensuring the safe and effective preparation, administration, and oversight of home infusion therapy services based out of our Longmont, CO Pharmacy. This role involves collaborating closely with patients, healthcare providers, and infusion nursing teams to deliver exceptional pharmaceutical care in a home and infusion center setting. The pharmacist will manage medications appropriately, educate patients and caregivers, and supervise the delivery of specialized treatments such as antibiotics and nutritional support.
Patient Care & Consultation:
Perform comprehensive medication reviews and offer patient consultations to optimize drug therapy management.
Assess patients' clinical condition by reviewing lab results and vital signs to determine the suitability of home infusion therapy.
Collaborate with physicians and healthcare providers to develop individualized therapy plans.
Educate patients and caregivers on correct medication administration, storage, potential side effects, and possible complications.
Continuously monitor and evaluate patient progress, making necessary adjustments to treatment protocols in partnership with the healthcare team.
Infusion Medication Management:
Supervise the preparation, dispensing, and delivery of home infusion medications, ensuring adherence to all legal and regulatory standards.
Accurately compound and label sterile products as needed, following strict safety and quality protocols.

Clinical Monitoring & Documentation:
Track patient outcomes and monitor side effects to achieve optimal therapeutic results.

Collaboration & Communication:

Experience:
At least 2 years of experience working as a pharmacist.
Experience with sterile compounding is preferred but not required.
