**Job Summary and Qualifications**
We are seeking a Pharmacist with critical care or infectious disease training and background to join our healthcare team.
The Advanced ClinicalPharmacist - Critical Care practices the profession of pharmacy with the utmost code of ethics demonstrating the organization's mission and values. They are responsible for the safe, effective, evidence-based, and cost-effective selection and utilization of medications for the patients in the ICU. They use advanced knowledge, skills, and professional judgment in all aspects of their work. They serve as a patient advocate when planning and implementing pharmaceutical care within a multidisciplinary rounding team, collaborating with other healthcare professionals in a knowledgeable and professional manner. In addition, they promote pharmacy services within the hospital by serving or chairing hospital committees, presenting at conferences and contributing to publications, and providing regular, formal education to pharmacy and interdisciplinary teams. Adherence to all regulatory and medication safety guidelines are at the forefront of all actions and decisions made within the practice of the ClinicalPharmacist
In this role you will:
+ Participate in facility committees responsible for establishing and implementing medication-related policies and procedures within the ICU.
+ Primarily responsible for the direction of the critical care service line within the hospital.
+ Identify how drug costs may be minimized through appropriate use of drugs in the ICU and through implementation of cost-containment measures.
+ Participate in nursing, physician, or ancillary team onboarding and staff meetings with the ICU as appropriate to explain clinical pharmacy services and educate on new drugs or updated clinical guidelines
+ Interpret orders and transcribe or verify them accurately and efficiently into the pharmacy computer system.
+ Accurately compound and dispense pharmaceuticals.
+ Monitor drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies and appropriateness of drug and dose.
+ Sustain the hospital formulary, minimizing nonformulary procurements, utilizing therapeutic interchanges and promotes rational drug therapy selection.
+ Provide accurate and timely medication information to healthcare staff and patients.
+ Assist with clinical services such as drug consults, nutrition support, pharmacokinetics, anticoagulant monitoring, pain management, IV to PO, renal dosing and other programs as defined by Pharmacy Leadership.
+ Document all clinical activities and interventions accurately and completely.
+ Complete notification reports regarding medication errors and adverse drug reactions.
+ Effectively supervise pharmacy operation activities and provide support to the whole pharmacy team.
**Job Requirements:**
+ B.S. or PharmD from and ACPE accredited School of Pharmacy required
+ Minimum of two years of hospital experience is required or Resident trained preferred
+ Active pharmacist license in good standing with the appropriate State Board of Pharmacy
