The pharmacist will have dedicated days each week spilt between the Endocrine and Primary Care/Population Health clinics. This pharmacist will be part of a clinical pharmacy team in the FMLH Endocrine clinic that provides chronic disease state management for type 1 and type 2 diabetes; osteoporosis and osteopenia; as well as weight loss. Within Primary Care, they will help support population health initiatives. This pharmacist will serve as a medication resource, will help with the optimization of patients' medication regimens, and may have other responsibilities, as assigned. In this role, the pharmacist will provide direct patient care via in-person office visits, virtual visits, and telephone encounters. This is a hybrid position and will require the pharmacist to travel between clinics. Using independent decision making and critical thinking skills, the Pharmacist promotes medication use that is safe, effective and considers the costs and benefits of therapy to maximize value. The Pharmacist interacts with other members of the healthcare team and with patients to proactively address medication therapy issues. The Pharmacist ensures patient care is maintained through compliance with the department's quality improvement plan, and the Joint Commission/Center for Medicare/Medicaid standards for pharmaceutical services. EXPERIENCE DESCRIPTION: A minimum of 1 year hospital or 3 years of other pharmacy experience is preferred. EDUCATION DESCRIPTION: Bachelor's degree in Pharmacy or Pharm.D. from an ACPE certified school of pharmacy is required. Successful completion of an ASHP accredited PGY1 or specialty residency is preferred. SPECIAL SKILLS DESCRIPTION: Demonstrates effective use of computers and applications (e.g. MS Word, Excel, Outlook) as appropriate to practice and communicate effectively. JOB PURPOSE AND REPORTING STRUCTURE: Under the supervision of the Director of Pharmacy, the Clinical Pharmacist is a member of the clinical interdisciplinary care team. The Clinical Pharmacist delivers pharmaceutical care in accordance with the policies and procedures of Outreach Community Health Centers. ESSENTIAL DUTIES AND RESPONSIBILITIES: * Ensure that medications reach the patient in the correct dose, form, vehicle and at the correct time. This includes tablets, liquids, drops, capsules, ointments, injections, inhalers, creams, beads, liposomes, continuous delivery systems and other forms that medications may take. * Take responsibility for the accurate, appropriate and timely use of medications administered to patients by staff in the clinic setting. * Take responsibility for the accurate, appropriate and timely dispensing of medications for patients upon presentation of valid prescriptions in the outpatient pharmacy setting. * Supervise and monitor the work of pharmacy technicians in compliance with State and Federal Requirements. * Teach and respond to queries of patients and their family's providers and other staff regarding accurate, appropriate and timely use of medications including indications, dosing regimens and side effects. * Stay current on new information and developments and educational material in Pharmacy to serve patients, providers and other staff. * Collaborate with providers and other health care staff including Community Service, Behavioral Health, Primary Care and other Medical Specialties to deliver safe, effective and economic medication use and delivery. * Counsel patients on dosage, administration, intended effects and unintended side effects of medications. * Monitor the stages of medication therapy to improve reporting of side effects and challenges with overall safe delivery. * Give special consideration to patients requiring multiple medications, complex regimens of therapy, and barriers to compliance with the goals of therapy. * Participate and complete special assignments and projects as assigned by the Director of Pharmacy. * Assists in inventory control and proper storage of medication. * Maintain professional standing and licensure in accordance with the State of Wisconsin Department of Safety and Professional Services. * Maintain technical competency and skills required for employment as a pharmacist. * Complete required documentation on a timely basis. * Comply with all Federal, State and local statutes that address discrimination. * Comply with safety rules and regulations of the organization. * Maintain the Pharmacy consistent with organizational standards. * Learn the ordering, billing and receiving system presently in use at the pharmacy. * Master the 340B Federal Drug Pricing Program. * Other job-related duties as may be assigned. Job Responsibilities: * Assist with Commercial compliance workplan oversite * PBM follow up regarding claim audits (benefit change forms, member, reimbursement/ recoupments, follow up audits to monitor effectuation of changes) * Participate in case management, disease management and poly-pharmacy for NHP patients with extraordinary high drug costs, including related services * Assist in providing physician education in areas of potential improvement i.e., create articles for pharmacy newsletter for NHP providers, speak to medical departments regarding drug issues, speak to individual NHP providers regarding drug issues including those for individual NHP members, participate in appropriate quality improvement projects, etc. * Assist with Concurrent DUR, Retrospective DUR, and MTM programs * Assist with formulary management and identifying UM criteria * Performs other duties and responsibilities as assigned Job Requirements: * Bachelor of Science in Pharmacy required * Current licensure without restriction by the State of Wisconsin Pharmacy Examining Board required * A minimum of 3 years clinical experience required * Residency/fellowship preferred * Managed care experience desirable * Prefer experience in at least one of the following areas: * Medication Therapy Management Programs * Long Term Care Pharmacy consulting * Decentralized hospital pharmacy services * Drivers license required Network Health is an Equal Opportunity Employer

Such management and coordination includes assuring qualifications for the program are met and maintained; the program is fully implemented in all areas of qualified use; related records are complete and accurate; and that it meets all primary objectives as defined by leadership. Education: Graduation from accredited technician school Required or Bachelor's Degree Preferred Experience: 2+ years experience as a pharmacy technician or in a healthcare related field with knowledge of pharmacy operations.
