The Clinical Pharmacist delivers pharmaceutical care in accordance with the policies and procedures of Outreach Community Health Centers. ESSENTIAL DUTIES AND RESPONSIBILITIES: * Ensure that medications reach the patient in the correct dose, form, vehicle and at the correct time. This includes tablets, liquids, drops, capsules, ointments, injections, inhalers, creams, beads, liposomes, continuous delivery systems and other forms that medications may take. * Take responsibility for the accurate, appropriate and timely use of medications administered to patients by staff in the clinic setting. * Take responsibility for the accurate, appropriate and timely dispensing of medications for patients upon presentation of valid prescriptions in the outpatient pharmacy setting. * Supervise and monitor the work of pharmacy technicians in compliance with State and Federal Requirements. * Teach and respond to queries of patients and their family's providers and other staff regarding accurate, appropriate and timely use of medications including indications, dosing regimens and side effects. * Stay current on new information and developments and educational material in Pharmacy to serve patients, providers and other staff. * Collaborate with providers and other health care staff including Community Service, Behavioral Health, Primary Care and other Medical Specialties to deliver safe, effective and economic medication use and delivery. * Counsel patients on dosage, administration, intended effects and unintended side effects of medications. * Monitor the stages of medication therapy to improve reporting of side effects and challenges with overall safe delivery. * Give special consideration to patients requiring multiple medications, complex regimens of therapy, and barriers to compliance with the goals of therapy. * Participate and complete special assignments and projects as assigned by the Director of Pharmacy. * Assists in inventory control and proper storage of medication. * Maintain professional standing and licensure in accordance with the State of Wisconsin Department of Safety and Professional Services. * Maintain technical competency and skills required for employment as a pharmacist. * Complete required documentation on a timely basis. * Comply with all Federal, State and local statutes that address discrimination. * Comply with safety rules and regulations of the organization. * Maintain the Pharmacy consistent with organizational standards. * Learn the ordering, billing and receiving system presently in use at the pharmacy. * Master the 340B Federal Drug Pricing Program. * Other job-related duties as may be assigned.     What Retail Pharmacy contributes to Cardinal Health Pharmacy Operations is responsible for the safe, efficient and effective coordination of Cardinal Health's pharmacy operations that service acute care hospitals, hospital retail customers, ambulatory care and alternate site facilities, oncology and cardiology practices as well as retail customers. Retail Pharmacy is responsible for ensuring the correct and safe dispensing of prescription medications to the general public, in accordance with legal, ethical and professional guidelines; may also consult with and advise patients and physicians regarding prescribed and over-the-counter medications. Cardinal Health manages the Sixteenth Street Community Health Center, and we are currently recruiting for a PRN Pharmacist to work as needed covering PTO and vacation. The facility is open Monday through Friday 8:30 AM to 6:00 PM. Responsibilities Under the direct supervision of manager, this position is responsible for dispensing and ensuring safe and efficient delivery of medications to all customers. Maintains open communication and is available to customers and practitioner sites for consultation Participates in supervision of technical staff Ensure pharmacy is compliant with best practices for 340B contract pharmacies Provide professional consultation to both patients and prescribers of partner health facility Completes pharmacy operational requirements by organizing and directing technicians' work flow; verifying their preparation and labeling of pharmaceuticals; verifying order entries, charges, and inspections. Qualifications Bachelor's degree in pharmacy required, PharmD preferred Pharmacist registration in the state of Wisconsin required Immunization Certification or willing to become a certified immunizer within 90 days of hire Ability to work a flexible schedule Interpersonal relations and customer service skills 340B experience preferred May require vendor credentialing What is expected of you and others at this level Applies working knowledge in the application of concepts, principles and technical capabilities to perform varied tasks Works on projects of moderate scope and complexity Identifies possible solutions to a variety of technical problems and takes action to resolve Applies judgment within defined parameters Receives general guidance and may receive more detailed instruction on new projects Work reviewed for sound reasoning and accuracy Anticipated hourly range: $50.10 per hour - $70.69 per hour.
