Prepare and present monthly reports to supervisor outlining the Center's productivity and performance improvement adherence. Provide an effective communication link between patients and providers. Performs studies, analysis and documentation to determine impact for patient care: Initial assessments, evaluation, diagnosis, treatment follow-up, and education relevant to the client care Change in treatment or symptoms Interventions and strategies in response to changes Accuracy and appropriate documentations Policy change positives or negatives. This position entails providing treatment services to women with a substance use disorder. JOB DESCRIPTION This is a professional counseling position that involves the treatment of individuals with substance use disorders and other co-occurring conditions. Treatment involves facilitation of individual, group, and family counseling sessions, assessments, treatment planning, care coordination, case management services, discharge planning, and documentation. The clinician must possess expertise in counseling principles and techniques. Case reviews may be conducted by telephone, video or in person. * Track patient follow up and clinical outcomes using a registry. Document in-person and telephone encounters. * Patient Records and Billing * Ensure accurate and timely billing for therapeutic services provided. Qualifications * 2 years of experience as a Licensed Clinical Social Worker with knowledge and experience in mental health therapeutic processes and counseling techniques. * Master of Social Work * Working knowledge of evidence-based psychosocial treatments and brief behavioral interventions for common mental health disorders (e.g., motivational interviewing, problem-solving treatment, behavioral activation). * Licensed Clinical Social Worker (LCSW)

The Social Worker LMSW conducts psychosocial evaluations, provides individual and group psychotherapy, care management services and discharge planning. Develops and implements patient treatment plans and participates in treatment team conferences. Relies on education, experience and judgment to accomplish job. Works under supervision required by regulation and licensing board. Creativity and some latitude is expected to complete responsibilities. * Clinical Practice and Care Management * Serves as the case manager for assigned patients.
