Physical Therapist, PRN, Outpatient Rehab at John St., Kalamazoo
Evaluates plans and administers medically prescribed physical therapy treatment for patients suffering from injuries, or muscle, nerve, joint and bone diseases, to improve/restore function, increase strength, relieve pain, and enhance functional independence. Employees providing direct patient care must demonstrate competencies specific to the population served.

For Bronson at Home driving your own vehicle on a regularly scheduled basis to perform business on behalf of Bronson is required and will require successfully pass of MVR check after an offer of employment has been made but prior to the employee's start date. Proof of valid auto insurance on that vehicle that will be driven while on duty for Bronson is required. It is the employee's responsibility to submit auto insurance renewals before current coverage expires to their supervisor. Must work as a team member and be able to effectively communicate with patients, visitors, and all members of the care delivery team

Pay will be set in accordance with the Compensation Plan and Wisconsin Administrative Code in effect at the time of hire. A probationary period is required.
Candidates with no prior permanent employment with the State of Wisconsin in the past 5 years may be eligible to accept a sign-on bonus of $3,000. The sign-on bonus will be paid in two installments: $1500 on the first paycheck and $1500 with the paycheck for the pay period in which the employee attains permanent status (usually 1 year). (UW System employment isn't disqualifying.)
To receive the bonus, a candidate must sign an agreement indicating they will remain in that occupational area for a year. If the candidate leaves their occupational area prior to completing the one year, they may be required to reimburse the initial payment.

These positions will normally be on site, but certain sites may have occasional remote work available. You can ask about a specific location if you are offered an interview.

This position is not anticipated to travel on a regular basis, but most DOC positions occasionally travel for training, meetings, conferences, or similar events.

If hospice care is appropriate actively provides emotional support, grief work, education and funeral/financial planning referral. Facilitates hospice or nursing home placement as needed. Initiate referrals to external resources with community agencies such as Adult Protective Services, Housing Authority, or public utility companies. Advocates with these entities for purposes of maintaining community stability.

* Provides information and referral related to community resources for patient/families/caregivers based on needs identified in the initial/ongoing assessment and Plan of Care.
* Provides education and assistance with Advance Directives as needed.
