Experience working with a military or veteran population is preferred.

Responsible for ensuring patients are transitioned to appropriate levels of care in a timely and effective manner:

POSITION SUMMARY
To provide therapeutic/emotional support, education, and resource information to patients and their families; Advocate for patient's needs/wishes with team members, family members, caregivers, and other providers.
ESSENTIAL FUNCTIONS
Assess the psychosocial needs of the patient and family members
Provide therapeutic counseling and emotional support to patients and family members and education on end of life issues
Contribute to and execute the Plan of Care with family/patient
Record daily documentation of visits and contact in accordance with Policy
Provides referrals to community resources
Performs in-office/non-clinical administrative duties
Perform other duties as assigned
POSITION QUALIFICATIONS
Bachelors of Social Work with 1 year of applicable experience required; Master's degree preferred
Texas State license required
Reliable transportation and valid driver's license.
Hospice or healthcare experience preferred.
Proficient skill using a computer is required

Interprets to other social service agencies, medical or psychological condition of patients as related to their emotional adjustment, physical limitations, employment, and financial needs, and to medical staff, those social, economic, or emotional factors which may be related to patients' medical care and physical rehabilitation.
Plans and conducts community meetings and discussion sessions relating to work of agency.
May provide working supervision to one or several students, professional, or paraprofessional social case workers of lower grade; may make assignments, review work in process and at completion, and advise with respect to resolution of work problems.
May participate in clinical and laboratory research in such areas as animal research, child development, physiological and behavioral functioning, etc.
Performs other duties as assigned.

Essential Functions:

May coordinate admissions process for new or returning patients.
Coordinates discharge planning with staff, patients, and families.
Coordinates social services "in-service" and educational programs for staff.
Completes all necessary and supportive documentation-progress notes, intervention notes, adjustment notes in accordance with state and federal regulations
May identify and promote a family council committee.
Ability to function as a Team Leader/Role Model.
Serves on various committees as requested by Administrator.
Has reviewed Cantex Continuing Care Networks Clinical Policies and Procedures or Abuse Prevention and knows the employee's responsibility to enforce it.
Responsible for assuring patient/resident safety
Performs other duties/tasks as assigned.
