Job Summary:
Responsible for providing intervention and assistance to the Social Work Department which is consistent with basic generalist social work practice. This includes: resource assessment and planning, intervention, evaluation, case management, information and referral, supportive presence/supportive counseling, case management, advocacy, community resource and client. Also included is long-term care planning to consist of LTC placement, completion of appropriate documentation i.e. FL-2s and obtaining PASRRs. Must be able to travel to, enter, and function in the homes of any patient/caregiver within the service area.

Provide crisis assessments, stabilization, and service coordination in order to de-escalate crisis and potential crisis situations in an effort to maintain lowest level of care that promotes safety.
Provide coverage or back-up for Emergency Services during clinical hours and on-call coverage after hours and on weekends as needed.
Maintain licensure as required by licensing board and attend educational trainings/seminars to remain up-to-date on current best practices.
Maintain positive working relationships within the communities served, including with individuals, families, staff, monitoring and licensing agencies, organizations, funding resources, and other human service agencies.

We are seeking a Licensed ClinicalSocialWorker (LCSW) to join our integrated clinical team. In this role, you will work onsite in one or more long-term care facilities within your local community, providing therapy, assessment, care planning, and behavioral health support. What You'll Do
As a key member of the interdisciplinary team, you will:
Clinical Care
Provide individual, family, and group therapy to LTC residents.
Conduct clinical assessments, psychosocial evaluations, screenings, and treatment planning.
Support residents with depression, anxiety, adjustment to illness, grief, trauma, substance use, behavioral disturbances, and other conditions common in LTC settings.
Deliver behavioral management and brief, goal-oriented interventions aligned with best practices in geriatric care.
Interdisciplinary Collaboration
Work closely with nursing, social services, rehabilitation, and medical providers to coordinate treatment and improve resident outcomes.
Participate in care plan discussions, facility meetings, and case collaborations as needed.
Documentation & Compliance
Maintain complete and timely documentation using our Geriatric Notes Tool (GNT).
Follow ethical, professional, and regulatory guidelines for clinical documentation, confidentiality, and care delivery.
Facility Partnership
Build strong, professional relationships with facility staff and leadership.
Serve as a behavioral health resource and trusted partner in supporting residents' mental and emotional needs.

AuthoraCare Collective is currently seeking a SocialWorker - Kids Path. This is a Full-time hybrid position Monday - Friday 8:00am - 5:00pm.
Responsible for providing comprehensive assessment of the social, emotional and practical needs of the pediatric patient and family in the business areas of perinatal services, hospice, counseling, and home health when needed. Facilitates resolution of psychosocial/emotional conflicts which may interfere with a patient and family obtaining maximum benefit from Authoracare services. Provides educational programs to the community. Functions as a member of an interdisciplinary team of care. Position covers both Greensboro and Burlington campuses and serves the entire catchment area of Authoracare.

Education/Qualifications:
* Master's degree in social work
* LCSW or LCSWA (or in the process of obtaining) required.
* One year of experience in social work experience in a mental health or medical setting is required.
* Must be able to travel to, enter, and function in the homes of any patient/caregiver within the service area
* Other: Valid state-issued driver's license required. Must carry automobile liability insurance at limits required by agency. Must have own transportation
Key Responsibilities:
Delivery of Care
* Prepares psychosocial assessments.
* Develops and carries out individualized plans of care related to the grief and loss, adjustment, care, and end of life planning needs of patients, families, and caregivers.
* Provides counseling including individual, family or group counseling for pediatric patients/caregivers/family.
* Identifies appropriate resources to meet patient/caregiver/family needs.
* Serves as an advocate for patients and families.
Interdisciplinary Group Process
* Actively participates with the interdisciplinary group (IDG).
* Works effectively with other IDG members with a focus on collaboration in problem-solving patient needs and working with the patients' plan of care.
* Provides effective report to the IDG based on regulatory requirements for patient care planning evaluation.
* Maintains effective relationships and partnerships with community healthcare providers, agencies, and faith communities.
