As a member of the ISSWT, you'll engage in direct practice, including: 1. Adult Protective Services (APS) Investigate reports of abuse, neglect, and exploitation of vulnerable or disabled adults. Determine if protective services are necessary and develop service plans to ensure the safety and well-being of individuals. Coordinate with law enforcement, healthcare providers, and community partners. 2. Guardianship & Legal Interventions Initiate and manage adult guardianship proceedings when individuals are deemed incapable of making decisions. Serve as guardian of the person (and sometimes estate) if appointed by the court. Prepare legal documentation and represent DSS in court proceedings. 3. In-Home Aide and Support Services Assess client eligibility and level of need for in-home care, personal assistance, or homemaker services. Develop and monitor care plans to support adults living safely in their own homes. 4. Long-Term Care and Special Assistance Assess for placement in adult care homes or nursing facilities. Determine eligibility for Special Assistance (SA) and Medicaid-related services. Conduct annual or semiannual reassessments and reviews of placement settings. 5.
