The Adult and Aging Services Unit provides citizens with the social services to support the achievement of goals of self-sufficiency. Further, it is the aim of this unit is to prevent abuse, neglect and exploitation of adults but if necessary, intervene to provide protection. The primary purpose of this position in relation to Adult and Aging Services is as follows: To provide Special Assistance In-Home Services to clients that meet eligibility requirements for the program. To manage a caseload of Adult Protective Services. This shall include completing a thorough evaluation to determine if the disabled adult has been abused, neglected or exploited. The worker will also work towards mobilizing services to assist the adult is needed .To provide At-Risk Case Management Services assist adults who are at-risk or show evidence of abuse, neglect, or exploitation in gaining access to needed medical, social, educational, and other services. To manage a caseload of guardianship cases. This shall include at the minimum a quarterly contact with the ward and his/her caregiver.
