Optimal Care Hospice Medical SocialWorkers are trained professional SocialWorkers who assist patients and families living with terminal illnesses. The Medical SocialWorker coordinates care to enhance the patient's quality of life by handling the day-to-day details of the client's medical care, establishing, implementing, and evaluating goals, and counseling the patient and their family/caregiver as they journey through the end stage of life.
In this role you will be responsible for:
Completing the patient's initial evaluation within 5 days of hospice admission and re-evaluating the patient's social work needs during each subsequent visit
Assisting patients and caregivers in planning for funeral arrangements, financial, legal, and health care decisions
Educating and assisting patients and caregivers in the preparation of advanced directives
Providing information to patients and caregivers regarding community agencies and referral services
Carrying out social evaluations, including family dynamics, caregiver abilities, communication patterns, high risks for suicide, neglect, or abuse, and planning intervention based on evaluation findings
Assessing the psychosocial status of patients and caregivers related to the patient's terminal illness
Counseling patients and caregivers as needed concerning stress, and other identified coping difficulties
Assessing for and educating staff on any special needs related to the culture of patients and their caregivers. This includes communication, the role of family, space, and any special traditions
Providing bereavement support to patients, caregivers, and hospice personnel
Required Qualifications
Master's degree inSocial Work from an accredited college or university
Current state licensing insocial work
Current and valid driver's license
Reliable transportation and valid automobile insurance coverage
Minimum of one year experience in healthcare
Medical Social Work skills are as defined by accepted MSW standards

Position Overview:
The Counselor will:
Facilitate patient participation in treatment and education within a supportive outpatient milieu
Conduct individual, family, and group counseling sessions
Perform all functions related to utilization review and discharge planning to ensure smooth, safe care transitions
Collaborate closely with healthcare teams, community agencies, law enforcement, schools, and protective services to coordinate resources and services
Interpret and communicate complex psychosocial needs of patients and families, modeling caring, professional practices
Assess psychosocial risk factors via evaluation of prior functioning, support systems, reactions, and coping mechanisms
Counsel patients and families on emotional, social, and financial impacts of illness or disability
Mobilize community and family resources to meet patient needs effectively
Conduct mental health assessments upon physician referral
Support complex discharge planning and crisis intervention efforts
Serve as a key point person for cases involving child abuse/neglect, elder abuse/neglect, institutional abuse, and domestic violence
Act as liaison between IU Health and local protective services (APS/CPS)
Provide intervention in guardianship, foster care, adoption, and mental health Level II placements
Qualifications & Requirements:
Education: Master's Degree from an accredited college or university inSocial Work, Counseling, Psychology, or a related mental health field is required.
Experience: 3-5 years of relevant healthcare, mental health, or community experience preferred.
Licensure: Current Indiana license as an LCSW, LMHC, LMFT, LCAC, or HSPP required.
Certifications:
Crisis intervention certification (required within 90 days of hire)
BLS certification (required within 30 days of hire)
Knowledge, Skills & Abilities:
Deep understanding of disease impact, human behavior, and psychosocial assessment techniques
Proficiency in evidence-based practices (CBT, MI, DBT)
Knowledge of legal mandates for child and adult protective services at state and local levels

$47,028-$71,000 per year + Car (Personal Use Option Available) *
Starting rates vary based on education, experience, and licensure earned.
Therapists holding an LCSW, LMHC, or LMFT will earn a base minimum of $58,000 annually (with potential increase based on experience).
Therapists holding an LSW, LMHC-a, or LMFT-a will earn a base minimum of $52,500 annually (with potential increase based on experience).
