Key Responsibilities:
Conduct individual and group psychotherapy sessions for patients in a subacute or skilled nursing facility.
Assess, diagnose, and treat a wide range of emotional and behavioral health conditions, including depression, anxiety, adjustment disorders, and cognitive decline.

**Experience Requirements** : One to three years experience in the Behavioral Services field a prerequisite. Experience with insurance/managed care organizations a plus. Familiarity with PC basics necessary with the ability to learn new programs. Strong writing and communication skills are required, as well as knowledge of working with community agencies and managed care representatives. Pay range: $24.60-$35.70

Position Overview
The SocialWorker is responsible for identifying and assisting students, families, and school systems in overcoming barriers that interfere with learning through assessment, counseling, consultation, and school and community resources coordination.

OVERVIEW
The SocialWorker will be supporting the Home Study (HS) and Post Release Services (PRS) department. This position is remote, located in New York/New Jersey and requires up to 50% travel.

DUTIES AND RESPONSIBILITIES
Provide culturally and linguistically appropriate clinical case management services to UC with significant needs.
Carry a caseload of 5 to 10 EPRS cases.

Licensed ClinicalSocialWorker (LCSW) with all state certification and licensing requirements met.
Minimum of three (3) years of social work experience, with at least two (2) years in a healthcare setting (e.g., physician's office, PCMH, hospital, clinic, home care, or nursing home).
Knowledge of Medicaid and Medicare, as well as managed care organizations.
Strong communication skills, both oral and written, with the ability to collaborate effectively with multidisciplinary teams.
Experience in home care or field case management is preferred.
Working knowledge of local and state resources to support patient needs.
Ability to assess and address Social Determinants of Health (SDOH).
Ability to manage patient crises and provide focused care interventions.
Valid state driver's license and access to an automobile.

Duties

• Be confident in engaging with people who may be under physical and/or emotional stress.
• Employ graduated harm reduction strategies while supervising youth. This includes correcting, and in some cases, verbally or physically intervening when a youth displays unsafe behaviors toward themselves or other youth and staff. Harm reduction requires that a YSS learn to recognize personal or situational flags that could lead to conflict or
harm, provide positive mentorship to youth, and help youth work on their own interpersonal skills and use of self-regulation strategies.
• Take opportun ities to improve the well-being of the facility community by being collaborative and informed.
• Supervise and/or assist youth during recreation and education activities, as well as take on various, as-needed assignments
• Establish and maintain positive, professional, and appropriate relationships with youths' families and loved ones.
• Assist youth in achieving education, work readiness, and community transition goals.
