Responsibilities
* Assists the patient and family with health related financial, social and emotional concerns and assists with effective coping skills.
Provides social work services in accordance with the plan of care.
Ensures that changes in the patient's clinical status are communicated to other healthcare team members and the care center office staff in a timely manner.
Educates the patient and family members and other team members on medical social work interventions and treatment plan.
Performs care planning, transfer and discharge functions.
Identifies risks or threats to the patient's health in the patient's environment and promptly implements appropriate interventions to reduce risks. Communicates identification of risks/ threats to the patient to the physician and other members of the healthcare team to coordinate care effectively.

Qualifications
* Holds a masters or doctorate degree from a school of social work, accredited by the Council on Social Work Education
* Has one year of social work experience in a health care setting.
* Meets state and federal requirements for this position.
* Current CPR certification.

Consult with nursing or other medical staff as needed if pt has urgent medical questions or concerns outside scope of practice.
Track billing codes used for follow-up visits. Collaborate with OSU billing dept as needed to notify of incorrect billing/billing codes and/or to clarify if a visit is eligible to be billed as TCM.
Assist in providing education to providers surrounding criteria for billing TCM visits. This may include presenting at provider meetings, sending EPIC messages, or discussing face to face.
Socialworkers who accept this position would be expected to assist clinic staff and patients in managing mental health crises or social crises (ex: domestic violence) as needed. This could include, but is not limited to initiating wellness checks, assessing for safety/safety planning, mandated reporting (CPS/APS), and de-escalation.
Socialworkers who accept this position may be expected to provide brief psychosocial intervention/support to patients experiencing grief and/or fetal/infant loss.

Conducts and prepares psychosocial assessments for patients receiving services at LCOH. During the assessment process, Social Work will identify and acknowledge emotional, social, and environmental strengths and challenges while addressing the needs of patients, families, and responsible parties relating to diagnosis, illness, treatment, and life situations.
Will participate in the LCOH clinical team which contributes to the formulation, development, and implementation of the patient's treatment plan utilizing appropriate social work interventions. Interventions may include crisis intervention, brief therapy, cognitive-behavioral, and dialectical-behavioral methods.
Will document in the Electronic Medical Record and paper chart. Documentation is timely, legible, concise, and pertinent and meets the professions and department's documentation standards. Oral reporting is clear, concise, relevant, and timely. Perform all required data collection for the department accurately and within the expected time.
Will address discharge planning needs across a multidisciplinary care continuum which includes interacting with consulting physicians and other health care providers.
