EDUCATION

Northwestern University Mesulam Center for Cognitive Neurology and Alzheimer's Disease (Mesulam Center) is seeking an exceptional socialworker who demonstrates a pronounced interest in working with persons with neurocognitive disorders and their families in a diversity of roles. The SocialWorker will work as a team member to provide clinical care for persons living with dementia and their families and achieve the goals and objectives of the Outreach, Recruitment and Community Engagement (ORE) Core, the Miller Family Buddy Program, in addition to other psychosocial initiatives.
Specifically, the SocialWorker will provide clinicalsocial work services for persons living with dementia and their caregiving families in the Neurobehavior and Memory Clinic of Northwestern Medical Group; contribute to the recruitment and retention of research participants that meet Center research priorities; manage and facilitate monthly care partner support groups; and participate in the coordination and planning of an annual scientific conference. The Mesulam Center is a multi-departmental component of the Feinberg School of Medicine. Areas of emphasis include age-related memory, language-based and behavioral dementias. We are focused on Alzheimer's disease, Primary Progressive Aphasia and Frontotemporal Dementia. Job Description

Daily Responsibilities:
• These people will be working in an office calling members to coordinate services, set up provider appointments, and talk to them about non-medical concerns and community resources.
• They will be working with the Medicaid population (birth-65 years old).
• Conduct screening and risk assessment interventions per program guidelines. Assist with nonmedical aspects of the member's care, including referrals to community resources.
• Identify special needs members through the completion of health screens and other resources
• Work with community outreach/member advocates to coordinate member care
• Educate providers and community resources on program components and available support services
• Educate members with special needs to foster compliance with program and positively impact outcomes
• Conduct site visits as appropriate for programs and provide support to other special programs
• Develop and modify care plans in conjunction with member, member's family and managing physician
• Development of plan specific literature and education materials in conjunction with medical director and corporate oversight
Hours for this Position:
• 8:00AM-4:30PM
• Required to work 8:30 to 5:00 one day a week
• Pay rate: $25-$26 per hour
