The Animal Care Assistant supports keepers with the husbandry, behavioral, and welfare management of all the animals in their department (primarily birds) according to Omaha Zoological Society, AZA, and USDA guidelines. The Animal Care Assistant is expected to be proficient in operant conditioning and capable of public presentation of a variety of animal species in both formal and informal settings, as this position has a high level of public interaction. Additionally, the Meadowlark Theater Animal Care Team is involved in working with Zoo Academy students, interns, and other animal ambassador handlers. This position reports directly to the Supervisor of the Meadowlark Theater. Maintain trained animal behaviors for husbandry and show behaviors as well as natural behavior demonstration to enhance animal ambassador programs. Assist in show production (pre-show preparations, sound, post show wrap-up). Assist with student education efforts including Zoo Academy, shadows, interns, and classes. Assist with the maintenance of the departments buildings, animal exhibits, and structures including cleaning, painting, light construction, etc. along with maintenance of Zoo grounds including plant care, raking, weeding, trash collection, etc. Assist Zoo guests and facilitate their needs. Qualifications, Knowledge, Skills, and Abilities: Must be at least 18 years of age or older. High School Diploma or GED required. Pursuing B.S in biology, animal science, behavior or related field, preferred. Basic computer proficiency in Microsoft Programs: Word, Excel, and Power Point. Experience working with birds or exotics preferred. Familiarity with basic animal training techniques, theory, and application, preferred. Able to learn relevant and factual information about a diverse group of animals. Effective and engaging public speaker. The primary responsibility of an SLP is to maintain a safe and healthy home environment for the individual supported. SLPs have basic home support and related responsibilities, which include providing/supporting nutritional meals; providing transportation to all appointments, to and from day services and recreational activities; providing/supporting access to community activities, cultural and religious services, if appropriate, and providing up to 24-hour supervision and supports as outline in the individuals Individual Support Plan (ISP). The daily home routine should be based on the Individuals choice but structured as determined by the Individual and ISP team which is helpful to the Individual supported in learning to function independently in a home and community setting. The SLP is responsible for teaching and supporting the Individual's life choices, hopes and dreams, and daily documentation of that teaching. Individuals with a developmental disability are matched with trained Share Living Providers who share the same interests and hobbies as the person seeking supports. The size of the home, all who live in the home (family size/kids/animals), and the location of the home are all influences on if the individual and their family are interested in meeting with a potential candidate. Qualifications Requirements Transportation, Valid Driver license, car insurance, and current registration Ability to pass a State/Federal and APS/CPS background check (all members in the home) Experience supporting an individual with developmental disabilities Computer/laptop, printer, scanner, and internet for daily documentation Completion of all required trainings Must have detailed verbal / written communication and computer skills Must understand and comply with all agency and state policy and procedures Must attend and participate in all meetings Must maintain a clean and orderly home Must have a high school diploma or GED Must be at least 19 years of age Must be able to lift 50 pounds, kneel, twist, reach, crawl, run, etc. Shared Living has the following additional requirements and limitation, as decided by the Department of Health and Human Services in the state of Nebraska (this is not an exhaustive list, just a sampling of the requirements): A Home Study Survey must be completed by the Provider with any potential SLP contractors in the home and all adult family members. Service authorizations will not be approved until the SLP has completed the Home Study Survey, has a compliant Final Settings Rule Site Assessment, is affiliated with Maximus, has completed a Service Coordination on-site walk through and a team meeting has been held to discuss the potential placement. If the placement is denied, the agency provider, not the SLP, may request a fair hearing. Provider agency must document monthly on-site visits for Basic-High Tier participants and two times monthly on-site visits for Advanced-Risk Tier participants. No less than two visits for Basic-High Tier participants or four visits for Advanced-Risk Tier participants must be unscheduled per 90-day period. If an SLP Provider is supporting a participant on the Basic-High tier then there can only be two state-funded (CFS, DD, AD Waiver, Probation, etc) individuals in the home. No other state-funded individuals are allowed when the participant is on the Advanced-Risk Tier. If an SLP is supporting a participant on the Advanced-Risk Tier there must be two providers, living full-time in the home and available during residential service hours, on the contract. If an SLP has children under the age of 13, another adult must live full-time in the home and be noted in the Home Study Survey, to provide care and supervision to the child in the event of an emergency. Any residence that the SLP and participant reside in must have its own United States Postal Service-recognized address. No separate basement apartments, apartments over garages, or apartments not fully integrated into the home will be permitted. No SLP or adult living in the home can serve as day staff for any participant living in their home. No SLP or adult living in the home can become an Independent Provider in order to provide Day Supports to the participant living in their home.

Responsibilities: Essential Functions Perform primary health care for patients by utilizing the processes of assessment and diagnosis of health status. Obtain patient's medical and history information, data collection is prioritized according to immediate condition or needs. Perform physical exam and determine patient's mental status. Develop and implement treatment, follow-up and evaluation plans for patient outcomes. Collaborate with physicians and other disciplines as appropriate and provides regular and consistent feedback. Use appropriate assessment techniques & diagnostic information. Performs tests relevant to patient condition and record findings. Order appropriate ancillary tests. Derive diagnosis from the assessment data and diagnostic information as data becomes available. Use new or additional data to form a treatment plan. Perform telephone triage to determine patient condition and provides appropriate consultation scheduling appointments when needed. Incorporate research into clinical practice as appropriate. Implement interventions consistent with the established plan of care in a safe and appropriate manner. Apply established therapeutic practices to patient's problem. Administer medications and immunizations using the appropriate method and with physician oversight if applicable. Prescribe medications according to Controlled Substance Registration Certificate issued by the DEA. Generate referrals to appropriate services as needed to further diagnose and/or progress of care. Evaluate patient care to determine response to care/interventions for optimal patient outcome. Evaluate the patient's capabilities and/or limitations related to nursing and medical interventions. Recognize and address patient and family capabilities and limitations (physical and mental). Return patient telephone calls. Monitor outcomes and documents patient response to interventions. Seek out new treatment methods necessary to ensure patient progress. Provide patient and family teaching and education specific to the patient's health care. Proper use of medication and health/illness prevention to ensure adequate knowledge/awareness. Interview and advise specific to patient needs. Demonstrate sensitivity to family regarding need for patient information on a timely basis. Work collaboratively with physicians and other team members to provide appropriate patient education geared to the patient's needs and family's ability to provide care. Use adult learning principles for teaching purposes being careful to check for understanding and being available for follow-up questions. Recommend references and resources such as printed materials, online, community and other information sources to fulfill the family's need for education. Maintain a professional behavior in attitudes, demeanor, and interactions to achieve organization's mission. Communicate effectively with physicians, staff and patients. Is approachable and professional in working with other health professionals, patients and their family. Maintain a professional appearance, behavior, and work environment. Initiate action to correct, prevent, and/or minimize risk(s) to the patient, family or hospital/clinic employees (i.e., falls, needle sticks, patient concerns). Communicate to appropriate personnel regarding unresolved issues. Serve as a role model in the provision of emerging care. Serve as a reliable/knowledgeable resource for staff. Participate in meetings, research on special projects and professional organizations. Comply with the bylaws, rules and regulations of the organization as they pertain to Nurse Practitioners or Physician Assistants. Know and follow organization policy & procedures. Follow Medical Protocols for professional discipline. Maintain certification/licensure. Follow guidelines of regulatory bodies (FDA etc.). Maintain complete patient records. Provide appropriate care specific to the age of the patient to ensure understanding and comfort level of treatment as outlined in the "Age Specific Criteria". For Wound Care: Two years previous clinical experience required. For Wound Care: Wound experience preferred.     Integrated Life Choices is seeking Shared Living Providers (SLP) to provide residential supports in the SLPs home for individuals with developmental disabilities. The primary responsibility of an SLP is to maintain a safe and healthy home environment for the individual supported. SLPs have basic home support and related responsibilities, which include providing/supporting nutritional meals; providing transportation to all appointments, to and from day services and recreational activities; providing/supporting access to community activities, cultural and religious services, if appropriate, and providing up to 24-hour supervision and supports as outline in the individuals Individual Support Plan (ISP). The daily home routine should be based on the Individuals choice but structured as determined by the Individual and ISP team which is helpful to the Individual supported in learning to function independently in a home and community setting. The SLP is responsible for teaching and supporting the Individual's life choices, hopes and dreams, and daily documentation of that teaching. Individuals with a developmental disability are matched with trained Share Living Providers who share the same interests and hobbies as the person seeking supports. The size of the home, all who live in the home (family size/kids/animals), and the location of the home are all influences on if the individual and their family are interested in meeting with a potential candidate. Qualifications Requirements Transportation, Valid Driver license, car insurance, and current registration Ability to pass a State/Federal and APS/CPS background check (all members in the home) Experience supporting an individual with developmental disabilities Computer/laptop, printer, scanner, and internet for daily documentation Completion of all required trainings Must have detailed verbal / written communication and computer skills Must understand and comply with all agency and state policy and procedures Must attend and participate in all meetings Must maintain a clean and orderly home Must have a high school diploma or GED Must be at least 19 years of age Must be able to lift 50 pounds, kneel, twist, reach, crawl, run, etc. Shared Living has the following additional requirements and limitation, as decided by the Department of Health and Human Services in the state of Nebraska (this is not an exhaustive list, just a sampling of the requirements): A Home Study Survey must be completed by the Provider with any potential SLP contractors in the home and all adult family members. Service authorizations will not be approved until the SLP has completed the Home Study Survey, has a compliant Final Settings Rule Site Assessment, is affiliated with Maximus, has completed a Service Coordination on-site walk through and a team meeting has been held to discuss the potential placement. If the placement is denied, the agency provider, not the SLP, may request a fair hearing. Provider agency must document monthly on-site visits for Basic-High Tier participants and two times monthly on-site visits for Advanced-Risk Tier participants. No less than two visits for Basic-High Tier participants or four visits for Advanced-Risk Tier participants must be unscheduled per 90-day period. If an SLP Provider is supporting a participant on the Basic-High tier then there can only be two state-funded (CFS, DD, AD Waiver, Probation, etc) individuals in the home. No other state-funded individuals are allowed when the participant is on the Advanced-Risk Tier. If an SLP is supporting a participant on the Advanced-Risk Tier there must be two providers, living full-time in the home and available during residential service hours, on the contract. If an SLP has children under the age of 13, another adult must live full-time in the home and be noted in the Home Study Survey, to provide care and supervision to the child in the event of an emergency. Any residence that the SLP and participant reside in must have its own United States Postal Service-recognized address. No separate basement apartments, apartments over garages, or apartments not fully integrated into the home will be permitted. No SLP or adult living in the home can serve as day staff for any participant living in their home. No SLP or adult living in the home can become an Independent Provider in order to provide Day Supports to the participant living in their home.
