About The Role:
Mobile Permanent Supportive Housing Specialist (MPSH) - This role supports communities that that have experienced chronic homelessness in transitioning to permanent supportive housing. This role works collaboratively with members of other organizations also providing supportive housing services: EPHC (El Programa Hispano Catolico), NARA (Native American Rehabilitation Association), Coalition of Community Health Clinics, Mental Health Association of Oregon, and JOIN to provide housing retention services.
Using the evidence-based Permanent Supportive Housing model and JOIN's Housing First approach, the MPSH program offers equity-informed, participant-driven, trauma-informed, and relationship-based services. With wraparound support-including case management, mental health services, and peer support-we aim to build trust, promote strengths, and ensure successful tenancy for our participants. The collaborative provides trauma-informed tools for an individual's professional success in this role. Those in this role have access to professional counseling through coalition, training funds, and it is aimed for those who plan to obtain a mental health professional certification (QMHP).
Essential Duties:
Provide housing placement and retention support services to people referred through the adult system coordinated access case conferencing process.
Work with individuals to develop individualized housing plans and identify supportive services to promote housing stability.
Assist individuals with completing housing applications and other required paperwork.
Build and maintain collaborative relationships with resident services staff, property managers, and management companies.
Provide ongoing support to individuals, including assisting with rent payments and connecting them with necessary services.
Maintain accurate and up-to-date documentation of all participant interactions and services provided.
Attend weekly case meetings to review case files, discuss progress and challenges, and coordinate services with other team members.
