Job Description The Home Health Aide is responsible for providing personal care and other designated health-related services designed to maintain the client's physical and emotional well-being according to the physician-ordered plan of care. Specific responsibilities include: Follows the client's plan of care to help each client reach and maintain his or her highest level of performance and independence in the home environment. These services include: bathing, grooming and assisting with activities of daily living. Provides communication with client and/or family while in client's home as appropriate. Qualifications The following are the required minimum qualifications for the Home Health Aide position: High School Diploma or GED. One (1) year experience as a Home Health Aide, Medicare home health experience preferred. Certified Nursing Assistant (CNA) license. Active status in the MN Nursing Assistant Registry. Valid driver's license, current auto insurance and reliable vehicle. Ability to communicate effectively, including written and verbal skills.
