The Certified Nursing Assistant is responsible for providing care in accordance with Family First Homecare (FFHC) and Universal Pediatrics policy, Aide Practice Acts, and State/Local/Federal regulations.
Additional responsibilities include representing Family First Homecare + Universal Pediatrics and its services in a competent, professional, and responsive manner while maintaining standards of high-quality patient care and customer service in accordance with all state, local and federal requirements, rules, and regulations. Performs other duties as assigned
KEY RESPONSIBILITIES
Accountable for providing care that is based on licensure/certification within the scope of practice.
Provides care in compliance with the patient's established Plan of Care such as:
Helps to maintain optimal hygiene and assists in maintaining a safe environment
May plan and prepare nutritious meals
Assists the patient with ambulation as tolerated
Encourage patient independence
Performs routine housekeeping tasks
Reports any change in the patients mental or physical condition to the Clinical Manger or Director of Nursing
Maintains an updated clinical record on each patient at all times, meeting required deadlines for documentation
Communicates with caregivers as needed for continuity of patient care
Participates in FFHC Quality Assurance, Performance Improvement program (QAPI) as requested
Continually strives to improve his/her skills
Attends and/or completes all mandatory training/ in-services as assigned
Submits required personnel file documentation in a timely manner
Adheres to FFHC compliance and ethics expectations
Maintains patient and family confidentiality
Maintains professional, supportive, and responsive interpersonal communication skills
Performs other duties as assigned
ROLE QUALIFICATIONS
EDUCATION & EXPERIENCE - REQUIRED
Certified Nursing Aide (CNA) with a State issued certification
1 year of demonstrated experience
Must provide valid and current CPR card/certificate that includes in person instructor led hands-on skills check.
Valid driver license with proof of insurance in applicable states
Must have a clear background screening
Clear Hep B Screening in applicable states
Clear TB Screening in applicable states
KEY COMPETENCIES
The ability to read and communicate in English both verbally and in writing
WORKING CONDITIONS/EQUIPMENT USE
Work is performed in the patients home - not substantially exposed to adverse environmental conditions. Training may be performed in an office setting.
