DUTIES AND RESPONSIBILITIES:
Under the direct supervision of the EMS Program Manager, within the Emergency Medical Service (EMS) program, the Administrative Assistant will:
Provide clerical and administrative support to the EMS Program Manager and EMS team
Maintain accurate records, files, and documentation to support program compliance and reporting.
Support the coordination of meetings, training, and communication efforts to ensure program efficiency.
Support Case Manager and the Clinician with administrative tasks, scheduling, and documentation.
Coordinate student stipends, logistics for orientation and wraparound services.
Maintain confidentiality and protected health information (PHI) by following all Federal, State, County, and City laws and regulations, including the Health Insurance Portability and Accountability Act (HIPPA).
Effective communication verbally, through email, through texts, and phone calls
Adhere to and promote HYC's mission and values. Acts as a role model to others exhibiting a high degree of professionalism and ability to work with diverse populations and respect cultural diversity.
Capacity to be discreet and a demeanor to maintain confidentiality relating to client and personnel information.

Qualifications
REQUIREMENTS:
Bachelor's degree in mental health and/or human services related field preferred.
Minimum of two year of administrative assistant and/or office management experience.
Experience in administrative coordination
Superb planning and organizational skills with a proven track record for strategic planning and execution and attention to detail.
Proficient in Microsoft program applications.
Ability to succeed independently within a fast-paced, dynamic, and sometimes unpredictable work environment.
Ability to work a flexible schedule, including evenings and weekends.

Mission

HYC has an immediate opening for a full-time Prevention Coordinator for the Community Collaboration Program. Under the direct supervision of the Director of Youth Services, the Prevention Coordinator will be responsible for the growth and development of staff and will be responsible for implementing best practices for optimal quality of services to the community.
Be responsible for the overall operation of the Community Collaboration Program (CCP) including the supervision of staff, monitoring of services, contract compliance and evaluation.
Be responsible for the implementation of the County Approved Work Plan, which includes an Agency-led Community Assessment, Logic Model, Evidenced-Based Programs, and SPA-Summit/Conference.
Be responsible for the development, coordinating and maintaining of SPA-Based Coalition comprised of local community residents (youth, young adults, and adults), leaders, non-substance use focused businesses, substance use prevention providers, and other from the SPA-7 community.
Ability to implement evidence-based strategies and prevention concepts to address substance use related community problems and contributing factors.
Assist in providing comprehensive community assessments to youth, young adults, and other community members in SPA-
Lead SPA-Based Coalition and must be willing and able to provide support and technical assistance for all community partners.
Develop, organize, and collaborate to coordinate substance prevention activities and events.
Be responsible for management of community-based services at all HYC locations, which also includes management of services provided at various sites within the community.
Monitor the fulfillment of the contract, including accurate documentation of all service activities, preparation, and submission of contract reports, monitor outcome performance and compliance with all contract requirements in accordance with Federal, State, County, City and agency Quality Assurance guidelines.
Be the central point of contact for the County regarding the program and its services to the community.
Collaborate with Clinical Quality Assurance to ensure proper staff orientation, training, documentation, and audit preparedness for all clinical and mental health contracts
In coordination with finance department, provide budgetary oversight of program administration and ensure program revenue and expenses are within the adopted annual budget.
Attending and supporting all County Department and related external meetings.
Oversee training and cross training programs and ensure all associates are aware of and comply with company, government and customer regulations, policies, work procedures, instructions, and deadlines. Participate in training programs and webinars as required.
Represent the Agency in the Community by participating in collaborative, interagency coalitions, and community organizations to address critical issues related to promoting Coalition goals in Southeast Los Angeles County.
Attend assigned meetings and perform other related duties as assigned.
Qualifications
REQUIREMENTS:
Prevention Coordinator shall have, at minimum, a Bachelors degree in a human service field or equivalent combination of education and relevant work experience.
Prevention Coordinator shall have at least three (3) years of demonstrated previous experience within the last five (5) years working in the field of substance abuse prevention.
Prevention Coordinator must be able to receive certification as a Certified Prevention Specialist under the Consortium of Addiction Programs and Professionals.
Prevention Coordinator shall be able to effectively communicate, in English, both orally and in writing.
Ability to work a flexible schedule, including most evenings and weekends.
Strong relationship-building and organizational skills.
Experience developing, providing, and/or evaluating environmental prevention programs/services.

Peer Support Specialists shall use their personal lived experience of recovery to assist individuals with their recovery from a mental illness or substance use disorder (SUD)
Peer Support Services may include contact with parents/legal guardians, caregivers, family members, or other collaterals (family members or other people supporting the member) to focus on the recovery and treatment needs of the member.
Supports the delivery of educational sessions or workshops such as the Early Intervention Caregiver Guide, opioid overdose prevention and naloxone administration training, and other related workshops that equip individuals with tools and strategies to address substance use and mental health.
Will provide culturally responsive support services for TAY and families (emotional support groups to promote self-regulation and wellness, as well as outreach, advocacy and care navigation to support social needs.
The Peer Support Specialist is required to attend individual and group supervision weekly throughout annual timeframes to review and monitor clinical caseloads, team-based care integration practices, and fidelity with Project ISPACE's evidence-based practices for individual, group, family clinical and support services
The Peer Support Specialist will collaborate with community partners for the integration and delivery of services (i.e., community colleges, LA Care, LACOE, Jovenes, DMH drop in shelters and EDD access centers, etc.).
Supports community outreach and engagement efforts designed to raise awareness among families and caregivers about available early intervention and SUD treatment services.
Facilitates family and caregiver support groups, offering a supportive environment for families grappling with the effects of their loved one's substance use.

HYC is seeking a compassionate and dedicated Enhanced Care Management (ECM) Lead Case Manager. This position serves as the lead care manager for Medi-Cal members with complex needs, coordinating health, behavioral, dental, and social services in collaboration with providers, community supports, and other entities.
The ECM Case Manager's goal is to improve health outcomes, reduce health disparities, and enhance continuity of care for high-need Medi-Cal enrollees, including those with chronic health conditions, behavioral health needs, substance use disorders, or who are experiencing homelessness or frequent hospitalizations.
KEY RESPONSIBILITIES
Assessment & Care Planning
Conduct comprehensive health and psychosocial needs assessments.
Develop individualized, person-centered care plans with measurable goals.
Service Coordination

Facilitate seamless transitions of care, including hospital discharges.
Member Engagement

DUTIES AND RESPONSIBILITIES:
The Community Engagement Specialist (CES) will develop and implement community engagement strategies to strengthen family and community connections.
This position is responsible for cultivating and empowering family leadership, building and sustaining relationships between families and their communities, and working to remove barriers while creating bridges that foster community success.
The CES will facilitate parent and family workgroups at the Community Family Resource Center (CFRC) to engage and empower caregivers as active participants and critical thinkers. In addition, the CES will collaborate effectively with co-workers, partners, and community stakeholders by sharing ideas constructively, considering input from others, following through on commitments, and maintaining respectful communication.
Under the direct supervision of the CFRC Program Manager, the Community Engagement Specialist will:
Develop and implement community engagement strategies, cultivate and empower family leadership, provide support to build and strengthen relationships between families and communities.
Facilitate parent and family workgroups at the Community Family Resource Center (CFRC) to engage and empower caregivers as active participants and critical thinkers.
Plan and execute large in-person community outreach and engagement events.
Coordinate ongoing and single in-person educational and enrichment activities to promote overall well-being, this may include events, classes, training, workshops, etc.)
Conduct a community needs analysis using tools such as community mapping (e.g., ARDI Equity Map), publicly available reports, and interviews with community members.
Develop and manage a Community Advisory Committee by engaging and involving community members in Service District (SD) 4 to participate in planning, implementation, and decision-making.
Collaborate with community leaders to identify emerging needs and new resources to ensure equitable access to services and support.
The CES will support data collection efforts by administering surveys, pre- and post-tests to measure impact and outcomes.
Provide accurate and timely documentation.
Effective communication verbally, through email, through texts, and phone calls.
Attend all assigned meetings and other related duties/activities. Must represent the agency in a professional manner.
Adaptive and multi-tasking ability. The individual must easily transition from one task to the next.
Must reside in Supervisorial District (SD) 4.

Qualifications
REQUIREMENTS:

Provide appropriate resources/referrals to families to assist with court mandates and/or assist with each family's needs, as well as follow-up on progress.
Collaborate and maintain effective communication with the assigned CSW to assist in accomplishing case plan goals and outcomes for families.
Complete and submit monthly reports for all cases to the assigned CSW on the case.
Maintain an average caseload of approximately 10-12 families per week.
Maintain accurate and timely documentation of all job-related activities including weekly progress notes, assessments, monthly reports, treatment plans, within 3 business days.
Attend weekly individual and group supervision, as well as individual and team program meetings.
Conduct one-time Family Preservation Assessment Services, with parents involved with DCFS, by conducting a 2-hour Biopsychosocial Assessment and complete and submit a summary report with diagnosis and recommendation for treatment within 72 hours.

Essential Duties and Responsibilities:
Facilitate connections with local mental health providers to establish a network of resources.

Provide crisis intervention implementation, especially for clients experiencing the effects of mental health crisis.
Facilitate training among the MDT team regarding helpful practices, methods, and interventions for addressing mental health among people experiencing homelessness.

The Homeless and Housing department at HYC is seeking a Metro Peer Counselor to join our team.
The Metro Peer Counselor is part of the Intensive Case Management Services (ICMS) Street Based Engagement multi-disciplinary team at HYC.
This team is responsible for providing services to people experiencing homelessness on the Metro Rail Stations and/or Bus Routes.
The Metro Peer Counselor will have the primary responsibility of using their lived experience and knowledge to provide outreach and engagement, support advocacy, and direct services to clients.
The Peer Counselor will travel with the HYC Metro team to a variety of settings including but not limited to transit stops on the bus and rail line stops/stations, homeless encampments, shelters, drop-in centers, and other areas designated by the Los Angeles Department of Health Services (DHS).
The role involves offering emotional support, sharing experiences of personal recovery, and facilitating access to community resources.
This position will be assigned to the Metro Blue Line from Slauson to Downtown Long Beach stop.
DUTIES/RESPONSIBILITIES
Provide peer support to people experiencing homelessness that are encountered on the Metro Blue line
Provide outreach, counseling, and case management services to people experiencing homelessness
Conduct intakes, assessments, enroll individuals in LA HMIS, and document according to the timeframe provided in the contract and according to HYC policies
Meet with enrolled clients on a regular basis to review, evaluate, and support both long- and short-term goals established
Assist clients with establishing mainstream benefits and gathering necessary vital documents such as birth certificates, IDs, or social security cards
Work with local law enforcement officers and interim service providers to ensure a smooth transition to interim housing
Participate in case conferencing, internal and external meetings, trainings, and other functions as directed by Program Manager and DHS
Assist with applications for supportive and subsidized housing and build relationships with prospective landlords
Facilitate connections with community-based organizations and agencies to provide comprehensive services to clients
Perform follow-ups and wellness checks on existing clients assigned
Maintain and execute confidential information in accordance with HIPAA standards

Qualifications
REQUIREMENTS:

Review clinical documentation (progress notes, treatment plans, Up Front Assessments, etc.)

· Conduct one-time Family Preservation Assessment Services with parents involved with DCFS

Qualifications
