POSITION SUMMARY: In partnership with Woodlake High School, Dinuba HS, Orosi HS, Tulare Union HS. Provides services in the areas of outreach, assessment, vocational exploration, case management, counseling, development of customers' individual plans, job search assistance and follow-up services; assists in preparation of administrative assignment.
ESSENTIAL RESPONSIBILITIES include the following:
1. Initiates and maintains contact with youth, High School staff, employers, job services and community agencies who express interest in the In-School Youth program opportunities.
2. Provides supportive services to at-risk youth and interprets and/or makes appropriate referrals.
3. Conducts initial assessment interviews and interprets testing results as part of initial assessment.
4. Places youth in subsidized and unsubsidized employment.
5. Provides job search assistance to participants.
6. Assesses skill levels and service needs of participants, including administering and interpreting objective assessment, test results, completing formal objective assessment, to include training and employment needs of the youth, and determining supportive services needs of customers.
7. Prepares and completes Individual Service Plans.
8. Provides counseling to at-risk youth, such as job counseling and career counseling.
9. Provides case management services to participants.

EDUCATION AND EXPERIENCE: Bachelor's Degree in related field, OR four (4) years of directly related experience and/or training; OR a combination of education and experience.
COMPUTER SKILLS: Moderate computer skills required for data entry, Word, Excel, and other software programs, as required for the position.
LANGUAGE SKILLS: Ability to read, and interpret documents such as safety rules, operating and maintenance instructions, and procedures manuals. Ability to write routine reports and correspondence. TRAVEL REQUIREMENT: Will require weekly travel to local Proteus offices or events at 50% of the weekly schedule and other assigned divisional areas. Travel is primarily local during the business day, although some out-of-area and overnight travel may be expected.

The Registered SUD Counselor working under moderate supervision provides counsel and support to patients suffering from drug or alcohol dependency, educate families in the best ways to help in the recovery process, and work with both inpatient clients and those participating in outpatient programs. Under moderate supervision delivers services to substance abuse clients including client assessments, monitoring client attendance at meetings and classes, individual and group counseling, etc. Must be working toward certification.
How will I contribute
May conduct intake and program orientation for participant including, but not limited to, assessment of psychological and social functioning and the development, implementation, and ongoing review of the participants Individual Treatment Plans in accordance with the Title IX regulations and Program protocol and policies.
Deliver individual and group counseling services, case management services, education and community outreach services, and referrals to appropriate services/community resources as necessary.

Interact with the criminal justice system and/or referral sources, when necessary.

Monitor participant payments/fees and assist with fee collections. Conduct financial assessment interview in accordance with Title IX Regulations and Program protocols.
Recommend leaves of absence, dismissal/suspension, reinstatement, completion, etc. for participants in accordance with Title IX and Program policies.
Communicate openly and clearly, on a regular basis, with Clinical Supervisor and support staff to ensure Program effectiveness (appropriate caseload, work hours, scheduling, group capacity, etc.)

Alcohol and Drug Registration and actively pursuing Certification, in good standing by an agency approved and recognized by the Department of Health Care Services (DHCS) is required at time of hire.
Associate degree or the equivalent sixty college semester units with emphasis in behavioral or social sciences if required for the position. Three years abstinence if there is a history of substance abuse. Completion of 40 hours per two-year period of related training is required for continued employment for registered/certified counselors.
Must maintain counselor certification as required by State law. Knowledge and compliance with CLAS standards. Familiarity with electronic health record systems (AVATAR). In-depth knowledge of drug and alcohol testing to monitor treatment plans and medical instructions.

Prepare all assigned written documentation including progress notes, Problem List, and medical necessity notes and keep all assigned documentation in compliance with applicable standards and requirements.
Receive referrals from criminal justice system, Child Welfare Services, various providers, and community members; may involve providing crisis intervention and case managements.
Attend staff meetings and other meetings as assigned, complete special projects promptly, and maintain strict confidentiality regarding sensitive or proprietary information or materials.
Conduct gender-specific, observed drug testing to clients as mandated by the referral source and/or the individual treatment plans followed by proper documentation and reporting. Alcohol and drug testing may include patch, urine analysis (UA), and Breathalyzer following appropriate procedures.

Provides orientation to new patients to the programs' rules, policies, procedures, and patients' rights.

Recognize and utilize patients' Strengths, Needs, Ability, and Preferences (SNAP), to develop and implement individualized Treatment Plans that effectively help patients to meet their short & long-term goals. • Determine the clinical necessity of counseling services based on the clinical assessment and evaluation of the patients.
Monitor closely, and document any change of circumstances with the patients, especially as it relates to matters that may affect their recovery, identifying and addressing relapse indicators to promote relapse prevention interventions in an early stage.
Comply with and implement the Medical Q.A (Quality Assurance) and Clinical Risk Management Policies, including:
Evaluate the patients for any high-risk conditions (e.g., liver failure, pregnancy, overdose, bipolar, schizophrenia, etc.), which may endanger the patient (suicidal) or other parties (homicidal).
Administer the BAM-R, C-SSRS, GAD-7, and PHQ-9 screening tools to determine the severity of the patient's mental/emotional status.
Evaluate the risk factors concerning each such case, considering the appropriateness of, and professionalism in treating such individuals in an OTP in comparison to treatment in a residential facility or other treatment modalities.
Review and ask about other practitioners' involvement in the treatment of these high-risk conditions. Recommend immediate referral to such a specialist in case of need. Request the patient sign a waiver for release of information which will allow communication with the other practitioner in case of need (e.g., deterioration of the patient's condition)
Flag high-risk patients to the Medical Directors, Physician Extenders, and Regional Clinical Leadership (RCL) and highlight the conditions concerned.
Refer such patients to the Medical Directors, Physician Extenders or RCLs (Regional Clinical Leadership) based on the urgency, utilizing the informal Referral Policy or the scheduled Treatment Team/Case Conference session.
Review physician notes, following the Medical Directors or Physician Extenders' session with the patient, verifying that they noted and addressed the high-risk factor, or otherwise bring it immediately to the attention of the supervising counselor, as well as the clinic manager, until the matter's proper and professional handling by the physician or physician extender.
Advise, in writing, other staff members (including the clinic manager, supervising counselor, dispensing nurse and front office) of high-risk patients and their condition. Maintain and circulate minutes from treatment team/case conference sessions to the other staff members.
Provide professional counseling and referral services by:
Addressing patients' clinical and special needs by maintaining referrals to specialized (“second tier”) services rendered by physicians, physician extenders and counselors within Aegis, as well as referrals to Keys to Recovery (or other twelve-step) support groups, and other community resources (e.g., agencies and practitioners outside of Aegis), acting as a liaison to these agencies, as necessary.
Providing individual and group counseling as clinically necessary, to address the problem issues identified in the Initial Assessment and Treatment Plans. The counseling service will include protocols for the treatment of Relapse Prevention, Peer Pressure, Anger Management, Domestic Violence, Parenting, Family Preservation, Vocational Rehabilitation, etc.
Utilizing advanced counseling techniques such as Cognitive Behavioral Therapy, Motivational Interviewing, etc.
Scheduling regular counseling sessions with patients; issue patient appointment cards and record scheduled or rescheduled sessions in EHR scheduler.
Documenting counseling entries, including signature and date, as required by applicable federal and state regulations, CARF guidelines and Pinnacle policies.
Engaging in Community Relations and Outreach efforts.
Attending all recommended training including Physician Training of Counselors, RCL training, and case conferences with the program physicians and physician assistants.
Collecting research data when requested.
Observing the collection of patients' urine samples when required
Attend team meetings and complete all training courses timely as required.

Responsible for assessing the psychosocial needs of clients and families in a Residential Substance Abuse Treatment environment. Provides intervention to meet those needs utilizing the treatment team and resources provided. The incumbent may also provide crisis intervention, case consultation and collaboration services to other professionals. As part of the treatment team, this position is responsible for maintaining the well-being of clients and the integrity of the program. Provides client supervision and staff/unit support as assigned.
POSITION REQUIREMENTS:
High school diploma or equivalent is required.
Current registration and enrollment in a state-recognized chemical dependency counselor certification program, such as Registered Alcohol and Drug Technician (RADT) with CCAPP, is required.
Six months of substance abuse or related field experience or an equivalent combination of education and experience is required.
Valid CA drivers license is required and must remain current throughout employment. A driving record may be obtained through the DMV.

CPR and First Aid certification will be obtained within seven days of employment and maintained current thereafter.

Essential Duties and Responsibilities
