ESSENTIAL DUTIES AND RESPONSIBILITIES Under the supervision and direction of Outpatient Program Manager the Substance Abuse Disorder (SUD) Counselor-II provides responsible individual and group counseling services to clients. trains, orients; provides instruction to clients and community on treatment of substance abuse and conducts orientations about substance abuse programs; and performs related work as required. Ensures proper and ethical documentation for the alcohol and drug treatment program according to Champions standards. Facilitate client's treatment issues and progress while guiding them through each phase of treatment, conducting process groups, education, behavioral interventions, and working in coordination with shared agencies (i.e., medical, mental health, probation, and parole, etc.) Summary of essential job functions/responsibilities Assists in the coordination of program activities to ensure effective service delivery. Assists in assigning individual cases to Substance Abuse Counselors and Trainees. Provides instruction and orientation to subordinate staff in program procedures and basic substance abuse and counseling theories. Conducts counseling sessions for individuals, groups and family members. Interviews and assesses people applying for substance abuse counseling services to obtain personal, social, and emotional history. Consults with professional staff of public and private agencies involved in client's case history or treatment. Provides information about the substance abuse program to individuals, the public and community groups and conducts outreach to promote program services. Perform and apply the techniques used in individual and group counseling. Write clear, accurate and concise reports and interpret data. Speak clearly and concisely before large groups of people. Provide and promote excellence in customer service for both internal and external customers. Acknowledge and respect cultural and linguistic differences of the Champions diverse population. Maintains a caseload of clientele and provides active and comprehensive case management services. Ensuring all area needs are attended to (mental, substance abuse, social, physical, and family). Ensures appropriate and comprehensive documentation within an EHR system. Comply with Drug Medi-Cal requirements in documentations and services. Provide comprehensive assessment using assessment tools assigned by the funding source including ASAM. Provide initial orientation and develop Problem Lists as required. Facilitate ODF and/or IOP day treatment group sessions covering topics including relapse prevention, sober living skills, alcohol and drug pharmacology, stress/anger management, family violence, job readiness and related issues. Conduct individual sessions and collateral services to assist clients' recovery process. Provide appropriate referrals, linkages and case management to other resources and services. Serve as a member of an interdisciplinary team, participate in program planning, treatment and goal development, and discharge planning. Utilize evidence-based practices including Motivational Interviewing techniques and relapse prevention model. Prepare all assigned written documentation including progress notes, Problem List, and medical necessity notes and keep all assigned documentation in compliance with applicable standards and requirements. Receive referrals from criminal justice system, Child Welfare Services, various providers, and community members; may involve providing crisis intervention and case managements. Attend staff meetings and other meetings as assigned, complete special projects promptly, and maintain strict confidentiality regarding sensitive or proprietary information or materials. Conduct gender-specific, observed drug testing to clients as mandated by the referral source and/or the individual treatment plans followed by proper documentation and reporting. Alcohol and drug testing may include patch, urine analysis (UA), and Breathalyzer following appropriate procedures. Provides guidance and orientation to new staff and interns in program procedures and basic substance abuse and counseling theories. Adheres to all client confidentiality requirements and standards, including updates of the shared Release of Information (ROI) to ensure that information is current and accurate. Employee Requisition Form Adult Substance Abuse Counselor Department: Tribal Social Services •High School Diploma or General Education Degree (GED) Three years of experience working in a substance abuse setting; Plus, one of the following certificates: SUDCC, CAO DC, CADC, CAC, CATC, RAS, and/or RADT. •Valid First Aid and CPR certification and maintain throughout employment or able to obtain within 90-days of hire •Valid California Driver License •Must be insurable by company's insurance carrier and maintain throughout employment •Knowledge of Tachi-Yokut culture and established Tribal/Departmental policies and procedures. •Knowledge and practice of Substance Abuse Treatment modalities per local, state, and federal practices. •Knowledge of applicable laws, codes, regulations, policies, and procedures. •Assist with crisis interventions. •On-call to address emergencies on a 24-hour basis. •Assist in coordinating and developing targeted activities/programs for identified at-risk populations.•Adhere to and maintain client/Tribal Member records according to HIPPA and 42 CFR standards. •Maintain Department case files and contact notes per departmental guidelines. •Update case status reports daily and provide weekly reports and specialized reports as needed. •Prepare client progress reports as requested and/or needed. •Complete screenings, intakes, orientations, assessments, treatment planning, counseling, client education, referrals, and case management services. •Facilitate Garden Group, Anger Management, Batterer Intervention Program (BIP), Domestic Violence (DV), and SUD groups. •Facilitate other groups as needed. •Develop supportive relationships and rapport with clientele. •Provide assistance and intervention within departmental guidelines. •Provide information to the client as appropriate to determine mutually agreed-upon goals and objectives. •Perform regular assessments and reassessments to determine the need for continued services at the appropriate level. •Conduct regular case reviews to ensure compliance and overall case management accuracy. •Meet with individuals and family members in the office, school, and home if necessary. •Communicate with network providers or insurance providers as the participant moves between different levels of care to support a successful transition. •Work collaboratively with substance abuse counselors and assigned therapists to engage, educate, communicate, and coordinate care with client, their families, referring parties, community agencies, and all others to ensure that all services agreed upon in the individual treatment plan are implemented. •Create and develop relationships with housing, employment, education, food assistance, childcare support, rehabilitation placement, primary care, mental health treatment, and other supportive services that will benefit the client upon re-entry into the community. •Attend group meetings to provide information and gather needs for program development and outside agency services. •May attend court hearings as needed at the director's discretion. •Stay current with counseling practices, modalities, and certifications by attending needed training, and workshops and submitting a request for certification or re-certification as needed. •Assist clients with transportation, resources, referrals, appointment scheduling, filling out documents, and other tasks as needed. •Drive company vehicles or personal vehicles to conduct business on behalf of TSSD. •Complete appropriate documentation requesting per-diem for travel when necessary. •Provide Treatment Program Placement and transportation for Youth and Adults as needed. •Attend weekly consultation with direct supervisor. •Participate in staff meetings, training, conferences, and other meetings as assigned. •Perform other duties as assigned. Effectively implement scheduled program curriculum. Leads and participates in face-to-face group and individual meetings utilizing motivational interviewing, cognitive behavioral therapy and other therapeutic processes. Engage participants daily, reinforce group rules and expectation for participation and completion. Works directly with participants to develop and implement treatment/action plans and educate participants about supportive networks as they return to the community. Review treatment plans, evaluate and record treatment progress, records visible changes in appearance, behavior, and demeanor of program participants. Maintains accurate, timely, and confidential participant records. Evaluates and documents the progress of participants assigned to their groups. Completes all delivery documentation requirements. Summarizes participants' background, treatment plan, and recovery process outcomes. Prepares accurate, concise, informative reports, records and plans that are consistent with Amity standards, and complies with applicable clinical and administrative rules. Liaise with transition/reentry staff, plans, and prepares transitional documents as directed by CDCR for continued community program activities. Works collaboratively with CDCR, DRP, CCHCS and institution staff. Supports and collaborates co-faculty, including but not limited to Supervisors, Directors, and Regional Directors. Participate in monthly, block, cross-training, curriculum training, technical assistance training, or other training as appropriate. Other duties as assigned. What You Will Bring: Minimum 6 months of experience providing SUDT services as a certified AOD Counselor. Current AOD Certification with organization recognized by the DHCS OR Registration with BBS or AOD certifying body, depending on education level. Understanding of causes, symptoms, and effects of substance abuse; appreciation for the behavioral, psychological, physical, and social effects of drug abuse. Excellent interpersonal, written and communication skills. Operational knowledge of MS Office Suite and ARMS (preferred). Ability to maintain sensitivity and objectivity under pressures generated by association with substance abuse treatment participants. Demonstrate ethical behavior in order to maintain professional standards and safeguard the participants. Protect and maintain program participants' health, safety, welfare, and confidentiality of records and participant information.
