POSITION OVERVIEW:
The Director of CPI Coordinator will be responsible for overseeing the quality of care in ACS/OFCS/OMH Children Residential Programs. In addition, ensure the programs are incompliances with all regulatory bodies. The Director of CPI will updates policies and procedure for the programs.

Work with staff to ensure in-kind needs are promoted on social media, through email and in organizational newsletters

Care Coordination is a service model whereby all of an individual's caregivers communicate and interface so that the patient's needs are addressed in a comprehensive manner. * Provide comprehensive psychiatric evaluations for children and youth placed in foster care. On a case-to-case basis, optionally provide psychiatric evaluations and medication management for youth up to one-year post-discharge from foster care.
* Analyze the psychosocial information shared by the team, past psychiatric and psychological evaluations, medical histories and physical examinations, and mental health screenings, as well as direct observation and interviews, and include a written report including diagnosis in the electronic health record.
* Provide recommendations for appropriate treatments and supports (i.e., psychotherapy, care management, home and community based services, special rate, higher level of care, and supportive housing).
* Psychiatric consultation and assessment on an emergency basis: conduct an urgent assessment of a youth when discharged from the hospital, or in crisis to assist treatment team in preventing hospitalization, or to aid in developing an action plan with the treatment team.
* Provides timely paperwork in compliance with ACS, DOH, OCFS and COA requirements.
* Assess whether medications are warranted.
* Provide medication management, as needed, and continue to assess and adjust medications.
* For youth who are psychiatrically hospitalized, the psychiatrist communicates with the treatment team to consult about planning and post discharge plans.
* Meet (option of virtually) with parents to obtain informed consent, as needed.

The Client Engagement Specialist team is the front office, reception and scheduling team for The Jewish Board Outpatient Clinics and CCBHC programs. The team works closely with callers, referring agencies, and Jewish Board Clinic staff, therapists and psychiatric providers to ensure clients are getting care and services appropriate to their needs. The team is the main point of contact for existing clients and new clients receiving outpatient services at the Jewish Board, so an expertise in customer service is essential. Additionally, the team gathers sensitive information (e.g. PHI) as part of the front office, reception and scheduling process and appropriately responds to acute clinical/safety concerns that may be identified.
POSITION OVERVIEW:
The Client Engagement Specialist is responsible for answering, returning, & screening calls/requests, collecting and scanning documentation and co-pays as required, checking clients in/out as required, scheduling and confirming appointments made to the Jewish Board Main Reception Line (1-855-CLINIC-1) for outpatient Clinic and CCBHC programs and when clients call the clinic directly or visit the clinics in-person. The Permanency Support Supervisor supervises key staff assigned to the area and focuses on promoting the well-being of children in the Permanency Services Program. The supervisor participates in staff meetings and training and coordinates with staff involved in providing Permanency services.

Principal Responsibilities
Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.
Directly supervises employees as denoted on the Agency Organizational Chart in accordance with federal, state and/or COA requirements.
Provides administrative expertise as required.
Ensures the accuracy, content, and completeness of documentation and files.
Participates in planning, programming, and budgeting of the Permanency Program.
Participates in the development of short, intermediate, and long-range plans and goals for the Permanency Program.
Participates in the development of Permanency Policies and Procedures.
Assists in organizing program meetings.
Conducts program orientation and on-the-job training of staff assigned to the unit.
Coordinates community resources for the benefit of children/families served by the Permanency Program.
Coordinates with provider placement agencies and contractors on the delivery of services to children and families.
Refrains from assuming any duty that is unrelated to and/or interferes with the responsibilities of the position.
Provides regular supervision and professional development of Agency employees.
Advocates, promotes, and practices cultural sensitivity and responsiveness in all day-to-day interactions.
Develops, promotes, and practices teamwork in all activities.
Ensure compliance with child welfare, OCFS, and federal regulations by applying strong knowledge of permanency policies, ASFA requirements, and all relevant legal standards in day-to-day casework and decision-making.
Required Qualifications and Experience
Bachelor's degree in social work or related field from an accredited four-year college or university with 4 years full time child placing experience as a worker or above in a private or state agency.
Ability to define problems, collect data, establish facts, and draw valid conclusions. Familiarity with computers and their capabilities.
Knowledge of child welfare policies, especially those related to permanency.
Knowledge of OCFS policies, federal regulations, and relevant legal requirements
Understanding of ASFA regulations as they relate to foster care youth.
Ability to work collaboratively with the team to meet targets, along with a clear understanding of what it takes to accomplish those targets.
Preferred Qualifications
Master's degree in human service or related field with at least 2 years of progressively responsible supervisory experience in the administration of a Permanency Services program or the equivalent.
Travel Requirements
This position offers a full-time, hybrid/in field schedule and is based in Brooklyn and requires frequent travel across New York City.

Summary Job Description
Coaches are paired with up to 15 young people and work with each of them from 9th grade (or age 14, if they are disconnected from school) to age 21, regardless of their academic status. Through the course of their work with assigned youth, coaches' general functions include the following:
Building relationships with up to 15 young people
Provide ongoing social/emotional support to the youth on their caseloads
Work 1:1 with young people to develop individualized academic and career development goals based on their interests and strengths and take measurable steps towards their goals
Connect young people to quality schools, programs, resources, and opportunities in line with their goals
Help young people persist in these settings, navigate any challenges that arise, celebrate successes, and plan/prepare for the next step on their journey
Should a youth not be successful in any particular school/program setting, Coaches help young people reflect on the experience and work with them to set new goals and transition to the next opportunity
Collaborate with Case Planners, Tutors, Specialists, and foster/birth parents, as needed, to help youth achieve their goals and ensure their overall well-being
Connect young people to peer group supports and build positive relationships
Ensure young people who are aging out can access and maintain stable, affordable housing and build independent living skills
Principle Responsibilities:
ACADEMIC
Re-engage disconnected youth and help them re-enroll in an academic setting
Assess the fit between the young person and their current academic setting and help them transfer into a better-fit high school or high school equivalency (HSE) program, if needed
Visit the young person's school/program and build relationships with school staff, review transcripts and academic data, provide educational advocacy, and discuss progress towards graduation/HSE obtainment
Ensure young people are attending school and have the supports and resources they need to be successful, such as tutoring, individualized educational plans (IEPs), 504 Plans, or other special accommodations
Expose students to post-secondary pathways, such college and/or accredited vocational programs, starting in the 9th grade (or as early as possible)
Assist students with post-secondary planning, along with the guidance counselors and/or College Specialists and Career Development Specialists
Connect the student to the College Specialist to apply to colleges, opportunity programs, and all forms of financial aid, funding, and scholarships
Provide college persistence coaching, including ensuring students are receiving all of the financial and academic supports they need to succeed each semester
Assist students with planning for apprenticeship and vocational education paths, as appropriate
Collaborate with Case Planners, guidance counselors, Specialists, Tutors, teachers, family members, and caregivers
CAREER DEVELOPMENT
Assist young people in obtaining working papers, if needed
Conduct career exploration activities with young people
Assist ALL young people in selecting at least one career development experience each year that is in line with their interests and academic situation/background
Work with the young person and Career Development Specialist to identify potential longer-term career pathways based on the young person's interests, strengths, and academic/vocational plans
Connect the young person to the Career Development Specialist, if needed, to explore workforce development programs, vocational programs, and for assistance with job applications
Connect young people, as needed, to agency-based job readiness and/or internship programs (e.g., YA WORC or the Mentored Internship Program)
Provide persistence coaching to young people enrolled in any external program by checking in with them regularly
HOUSING AND INDEPENDENT LIVING
Ensure that the young person coordinates with a Housing Specialist (and/or Case Planner, if the agency does not have a Housing Specialist) to ensure that all young people's housing applications are submitted as early as possible (by age 19 if their goals is to age out)
Accompany the young person to visit supportive housing residences, when needed
Help young people move in and obtain furniture
Help young people maintain their housing by providing independent living and life skills supports such as budgeting, obtaining a bank account, and advocating with landlords, if needed
Minimum Requirements:
Qualified applicants for the Coach positions will be required to have a Bachelor's degree and at least three years of experience working with at-risk or system-involved young people or an Associate degree and 5+ years of working with at-risk or system-involved young people
Strong written and verbal communication skills
Strong time-management, organizational skills, and attention to detail
Ability to troubleshoot, advocate, and navigate obstacles where there is not always a clear-cut pathway
Open to learning how to use and interpret data to inform decision-making
Ability to effectively collaborate in team settings and build relationships with external partners
Ability to use Motivational Interviewing techniques
Ability to use trauma-informed techniques
Experience working with vulnerable youth
Knowledgeable about youth resources and services
Ability to facilitate therapeutic peer groups
Competence in planning, organizing, scheduling, case documentation, and strong oral and written communication abilities. Some experience in public relations preferable
Must possess strong human relations skills to communicate and work effectively within a culturally diverse work environment and community
Valid NYS Driver's License if using personal vehicle or agency vehicle to drive on agency business.
Travel Requirements:
This position is based in Brooklyn, NY and will be required to travel to all locations to service this population.
