About the Role
Care Coordination is a service model whereby all of an individual's caregivers communicate and interface so that the patient's needs are addressed in a comprehensive manner. This is done primarily through a "Care Coordinator" who oversees and provides access to all services an individual needs to assure that they receive everything necessary to stay healthy, out of the emergency room and out of the hospital.

Through the creation of an Individual Plan of Care the Care Coordinator is able to:
Coordinate the enrollee's provision of services
Support adherence to treatment recommendations
Monitor and evaluate a patient's needs, including prevention, wellness, medical, behavioral health treatment, care transitions, and social and community services where appropriate.
Meets documentation requirements in a timely and accurate manner by effectively utilizing designated Care Coordinator Portal (Medicaid Analytics Performance Portal; MAPP) and Electronic Health Records (EHRs) as needed
Maintains required contact with participant and their families and conducts face-to-face support team and/or family meetings as required
Functions as an advocate for clients within the agency and with external service providers
Promotes wellness and prevention by linking enrollees with resources and services based on their individual needs and preferences
Effectively communicates and shares information with the individual and their families and other caregivers with appropriate consideration for language, literacy and cultural preferences.
Participates in care planning meetings/conferences as an interdisciplinary team member to effectively provide/coordinate comprehensive and holistic care
Identifies available community-based resources and actively manages appropriate referrals, access, engagement, follow-up and coordination of services
In the event of hospital admissions, actively engages in the discharge planning process ensuring that the patient has all recommended post discharge services in place prior to discharge
Arranging appointments, transportation, and interpreter services when needed
Accompany the child to appointments as needed
Conducting follow-up activities to ensure appointments are kept.
Attends and participates in ongoing staff development trainings to enhance skills needed to effectively meet the demands of the Care Coordinator position

Summary Job Description:
The contracted child and adolescent psychiatrist will work in collaboration with the team to ensure high quality clinical services and provide up to 15 hours/week of trauma-informed, child-centered psychiatric evaluation and medication management for children placed in foster care, as well as providing timely documentation, participating in multidisciplinary team meetings to recommend behavioral supports, such as care management, crisis intervention, and individual and group psychotherapy. Other responsibilities may include case reviews, participating in training, and consultation with other team members.
Principal Responsibilities:
Provide comprehensive psychiatric evaluations for children and youth placed in foster care. On a case-to-case basis, optionally provide psychiatric evaluations and medication management for youth up to one-year post-discharge from foster care.
Analyze the psychosocial information shared by the team, past psychiatric and psychological evaluations, medical histories and physical examinations, and mental health screenings, as well as direct observation and interviews, and include a written report including diagnosis in the electronic health record.
Provide recommendations for appropriate treatments and supports (i.e., psychotherapy, care management, home and community based services, special rate, higher level of care, and supportive housing).
Psychiatric consultation and assessment on an emergency basis: conduct an urgent assessment of a youth when discharged from the hospital, or in crisis to assist treatment team in preventing hospitalization, or to aid in developing an action plan with the treatment team.
Provides timely paperwork in compliance with ACS, DOH, OCFS and COA requirements.
Assess whether medications are warranted.

ACADEMIC
Re-engage disconnected youth and help them re-enroll in an academic setting
Assess the fit between the young person and their current academic setting and help them transfer into a better-fit high school or high school equivalency (HSE) program, if needed
Visit the young person's school/program and build relationships with school staff, review transcripts and academic data, provide educational advocacy, and discuss progress towards graduation/HSE obtainment
Ensure young people are attending school and have the supports and resources they need to be successful, such as tutoring, individualized educational plans (IEPs), 504 Plans, or other special accommodations
Expose students to post-secondary pathways, such college and/or accredited vocational programs, starting in the 9th grade (or as early as possible)
Assist students with post-secondary planning, along with the guidance counselors and/or College Specialists and Career Development Specialists
Connect the student to the College Specialist to apply to colleges, opportunity programs, and all forms of financial aid, funding, and scholarships

Provide persistence coaching to young people enrolled in any external program by checking in with them regularly
HOUSING AND INDEPENDENT LIVING

Principal Responsibilities
Facilitate high-quality instructor-led and virtual training sessions for all staff, including onboarding, annual refresher requirements, and specialized program training.
Build learning modules grounded in adult learning theory and aligned to the Little Flower competencies.
Delivery required training (e.g., Mandated Reporter, OCFS-required modules, HIPAA/FERPA, crisis intervention refreshers, documentation standards, performance management).
Adapt content to diverse learner groups across youth care, clinical, administrative, and supervisory staff.
Develop and update training decks, facilitator guides, e-learning modules, assessments, and job aids.
Ensure all materials meet regulatory requirements and reflect current agency policy, COA standards, and OCFS directives.
Maintain the agency's learning library and work with cross-functional partners to ensure content accuracy.
Track all training attendance, completions, and certifications within ADP and internal tracking grids.
Support documentation for COA, OCFS, ACS, DOH, and internal audits, including session attestations, rosters, and test results.
Identifygaps in compliance training and escalate risks promptly.
Conduct training needs assessments with program leaders todeterminerequired learning solutions.
Collect, analyze, and report survey data and training metrics to support continuous improvement.
Use evaluation results to refine training content and delivery methods.
Partner with HR, Quality Assurance, Program Leadership, and Compliance to align training with agency initiatives, regulatory updates, and performance trends.
Participate in agency committees, workgroups, and initiatives related to workforce development and organizational culture.
Required Qualifications and Experience
Bachelor's degree in Human Resources, Education, Social Work, Psychology, or related field
Minimum three years of training, workforce development, or program facilitation experience
TRIPP-trained and familiar with all regulations related to foster parent training.
Trained in NTDC, sothey canassistin delivering training to prospective and current foster parents.
LGBT-QAI+ trained, so they canassistin delivering training to prospective and certified foster parents.
Knowledgeable about foster home regulations and the certification requirements for foster homes.
Demonstrated experience delivering content in behavioral health, child welfare, education, social services, or similar environments
Strong facilitation skills with diverse audiences and levels
Proficient in Microsoft Office, LMS or HRIS learning modules, and virtual training platforms
Strong written and verbal communication skills
Preferred Qualifications
Experience in trauma-informed care, crisis prevention, de-escalation, youth care, or foster care environments
Familiarity with OCFS, ACS, COA, and New York State regulatory training requirements
Experience creating multimedia content (videos, e-learning, microlearning)
Bilingual Spanish.
Travel Requirements
This position offers a full-time, hybrid schedule and is based in Brooklyn and requires travel to Wading River and New York City, and other locations when needed.

Family Advocate
The Family Enrichment Center is seeking a Family Advocate for a Family Enrichment Center (FEC) that will be opening in the Bedford Stuyvesant area in Brooklyn. The Family Enrichment Centers are a warm and welcoming community gathering place where any family or individual can come in for support, information, and to participate in and/or lead activities (called “offerings”). Each FEC is a unique to the community's needs, a home-like setting designed with and for community members where they can connect as neighbors, contribute to their community, and support one another. FECs are based on the premise that community members design and drive the FEC's activities or offerings.
Principle Responsibilities
The Family Enrichment Center (FEC) Family Advocate supports the FEC Director with day-to-day operation of the Family Enrichment Center, while building relationships with community members and organizations.
The Family Advocate builds connections using a warm and patient communication style and uses a strengths-based and community-driven approach to organizing and collaborating.
Responsibilities include building partnerships with community members and stakeholders, co-designing and delivering relevant offerings, engaging in advocacy, and continual collaboration with the FEC Advisory Board.
Work collaboratively with local organizations and community leaders;
Develop and nurture collaborative and effective relationships with community stakeholders;
Represent the FEC at community events and venues;
Interact regularly and frequently with the community members;
Facilitate offerings in response to community priorities, needs or interests;
Collaborate with the Advisory Board to co-create surveys to understand community interest in offerings;

Special Qualifications

Professional experience should be in community-driven-programs working directly with children, youth, and families.
Minimum five (5) years of experience living and/or working in Brooklyn Stuyvesant area;
Minimum two (2) years of experience working with children and families;
Bilingual in Spanish/English preferred.
Ability to stay calm and poised under pressure.
Knowledge and experience working in the Bedford Stuyvesant Community.
