LSF is looking for talented Youth Mentors who want to make an impact in the lives of at risk youth in our male and female group homes. We are looking for people who are willing to work with challenging behaviors while having the ability to make an impact in the youth's lives.
This schedule is 2pm-10pm. Must be able to work weekends.
Purpose & Impact:
Key responsibilities include the direct supervision of youth residents, providing quality care, ensuring a safe and secure environment and transporting and accompany/supervising residents on outside activities. This is a direct service delivery position at a primary location, but the position may work at any one of a number of regional group homes within a reasonable radius as shift needs arise.
These group homes house adolescent teens who have aggressive behaviors. The ideal candidate for this role is someone who would be comfortable working with at risk adolescent males and females. The ideal candidate would also not be intimidated by youth who have been impacted by abuse, neglect, and trauma. Take on a challenging role while having the ability to be a mentor and make an impact in the lives of youth.
Essential Functions:
Provide supportive environment for youth.
Assists children with preparing and eating meals, taking baths, maintaining hygiene, administering medication, and performing other daily routine tasks.
Transports individual and groups of children for appointments or field trips outside the facility
Provide daily security and up-keep of facility.
Develops, plans, coordinates, and participates in daily recreational and social activities, including outdoor programs and physical exercise.
Consults and communicates with shift leaders, supervisors and professional staff concerning behavior issues and the needs of children
As directed, participate in the implementation and monitoring of client case plans.
Participate in various in-service training sessions and attend staff meetings
Assign and supervise chores in addition to performing light housekeeping duties such as making beds and cleaning laundry, to ensure that the children's residential setting is neat, clean, and orderly.
Interviews and responds to critical incidents and emergency situations. Document incidents in the logbook as well as on the appropriate forms.
Assists with the initial intakes and discharge of youth.
Document in the logbook as outlined in policies and procedures.
Conducts group meetings with youth to discuss issues and address concerns.
Provides instruction on appropriate behaviors and independent living skills.
Observes and documents the behavior, development and needs of children.
Maintains daily written records and logs of observations and concerns about individual children.
Provides responsive, high-quality services to Community Based Care Lead Agencies, the Department of Children and Families, the Department of Juvenile Justice, the Courts, the School Board and other representatives of outside agencies and members of the public by providing accurate, complete and up-to-date information, in a courteous, efficient and timely manner
Other Functions:
Perform other duties as assigned by Program Director and/or House Coordinator for various shifts on an “as-needed” basis.
Physical Requirements:
Ability to remain alert and responsive during all shifts, including overnights and extended hours.
Ability to walk, stand, sit, stoop, bend, squat, kneel, and climb stairs frequently throughout the day.
Ability to lift, carry, push, and pull up to 25 pounds occasionally.
Ability to perform physical intervention or de-escalation techniques if required, consistent with agency-approved crisis management protocols.
Sufficient vision and hearing to observe and monitor client behaviors and ensure safety in indoor and outdoor settings.
Ability to operate standard household equipment (e.g., washer, dryer, kitchen appliances).
Manual dexterity to write legibly, operate a computer, and perform light cleaning and household duties.
Ability to work in a variety of environments, including indoor residential settings and outdoor recreational spaces, in all weather conditions.
Ability to drive and maintain eligibility to operate agency vehicles, if required for transporting youth.
Ability to transport youth in agency van.
Ability to accompany youth on field trips and other recreational outings.
Certified CPR/First Aid, and crisis intervention required.
Valid Florida driver's license and proof of active insurance.

Job Description:
The employee must be productive under tight timeframes, balance multiple and competing priorities and maintain goal-directed behavior and performance sometimes under stressful conditions. The employee must interact courteously with others. Attendance is an essential function of this position. This position is responsible for providing independent professional vocational rehabilitation counseling to individuals with a diversity of disabilities in order that they may attain employment; providing and coordinating complex services; and providing technical assistance on vocational rehabilitation issues to internal and external customers. The incumbent of this position will access client information, maintain case records, produce correspondence, and perform other automated functions within the scope of the position's responsibilities.Assesses medical, psychological, and vocational information to determine eligibility for services. Provides technical assistance to internal and external customers. Develops and implements with full participation of the customer with a disability, an Individual Plan for Employment to determine employment outcomes.Provides professional vocational rehabilitation counseling to individuals with a diversity of disabilities who require complex vocational rehabilitation strategies in order that they may attain appropriate employment outcomes. Provides information to assist individuals with disabilities in making informed choices regarding services, service providers and emplyment outcomes. Authorizes, coordinates and/or negotiates services leading to successful employment outcomes. Negotiates with individuals with disabilities regarding plan development. Devises job placement opportunities through job placement activities, including job analysis, job development, and labor market information to assist individuals with disabilities to obtain appropriate employment outcomes. Develops employer contacts and serves as liaison with employers. Utilizes the resources of community rehabilitation providers in securing appropriate job placement for eligible individuals with disabilities. Coordinates or collaborates with others in other placement activities. Provides technical assistance to internal and external customers. Conducts in-depth interviews of individuals with disabilities requiring complex rehabilitation strategies to determine eligibility for vocational rehabilitation services and the feasibility for achieving a vocational goal. Manages resources to ensure financial accountability according to state and federal regulations and policies. Collaborates with community partners to assure appropriate use of available resources. Provides technical assistance and specialized training to internal customers. Participates in training experiences designed to enhance professional competency in vocational rehabilitation. Performs other duties as required.

Background Screening Requirement:

What You Will Do
Perform comprehensive diagnostic assessments of your patients' needs and psychosocial status.
Develop a comprehensive, individualized treatment plan and goals for your patients.
Additionally, you will provide psychotherapy, group, and family therapy if applicable.
Collaborate with members of the facility's interdisciplinary team, as appropriate, to develop treatment goals for the patient.
Provide appropriate documentation of all pertinent information obtained in interviews, consultation with other healthcare professionals, referrals, contacts, and supportive services provided to each patient.
This position does not include any case management or discharge planning. Our billing team will handle all billed claims and authorizations. This allows you to focus on solely providing therapy to your patients and your documentation directly into our own Electronic Medical Records.

Essential Job Responsibilities
Recruitment and Screening:
Maintain assigned billable hours per week on average throughout the year.
Screen clients for mental health and substance abuse disorders as well as for available counseling interventions.
Informed Consent:
Obtain informed consent utilizing agency forms prior to providing services.
Conduct client orientation to treatment program according to applicable laws and agency procedures.
Assessment:
Conduct In-Depth Assessment to determine diagnoses and/or course of treatment for both individuals and groups.
Complete assessment requirements for various available counseling interventions and programs.
Treatment and Service Planning and Documentation:

Intervention & Crisis:
Conduct interventions with clients including individual, group and other appropriate counseling interventions.
Determine client needs for specialized services within or outside of the agency, such as medical treatment, psychiatric care, psychological testing as well as with other health-related organizations in accordance with the client's treatment plan and communicate all such actions to the psychosocial team as applicable.
Assess clients in crisis for appropriateness of Baker Act proceedings, utilize Baker Act documentation when necessary and coordinate with police, family, and other staff as required to place client when necessary.
Supervision:
Provide daily documentation of work performed (e.g., Informed Consents, Assessments, Treatment Plans & Reviews, Case notes, Discharge Summaries)
Attend at least weekly supervision with supervisor to review work, obtain signed documentation, discuss clinical issues and staff critical cases until such time as licensure under FS 491 is achieved.
Provide your supervisor with productivity reports weekly.
Manage Resources:
Coordinate with client case manager for clients support services as needed.
Coordinate court referred programs by writing reports and progress notes as required.
Input client information using specific software as required.
Provide clients with information about bill-coverage, services and procedures as required.
Perform quality assurance reviews bimonthly achieving a minimum score of 90%.
Participate in staff training sessions as required by the health center.
