Key Responsibilities:
Conduct individual and group psychotherapy sessions for patients in a subacute or skilled nursing facility.
Assess, diagnose, and treat a wide range of emotional and behavioral health conditions, including depression, anxiety, adjustment disorders, and cognitive decline.
Develop and implement evidence-based treatment plans tailored to the needs of each patient.
Collaborate with facility staff, interdisciplinary teams, and family members to support patient care.

Requirements:
Active and unrestricted license to practice as a Licensed Marriage and Family Therapist in State applying for.

Ability to provide in-person services at assigned facilities, up to 45 minutes.
Ability to walk, stand, and move between patient rooms and offices within the facility throughout the day.

Preferred Qualifications:
Previous clinical experience in a subacute, long-term care, or geriatric healthcare setting.

Center For Family Services is seeking a Full Time Overnight Residential Counselor for our Juntos Program located in Bridgewater, Somerset County, NJ. Juntos provides important services to unaccompanied minor children who immigrated into the United States. Services include; temporary shelter and support with family unification and home study and post release services. Family Reunification is the Juntos programs primary goal. The Overnight Residential Counselor will be responsible for assisting youth with their daily life skills, education, and chores in the program and providing a direct level of support and supervision. Duties and Responsibilities including but not limited to;
Ensures that program structure is consistent with stated goals and objectives and that a Nurtured Heart, trauma-informed program is followed.
Reports to and serves on the treatment team with Therapists and other designated staff.
Provides life skills education.
Coordinates recreation and physical education activities scheduled during their shift.
Ensures that daily living needs of youth are met through delegation of responsibilities to residential counseling staff.
Coordinates appointments with medical consultants, ensures all over-the-counter and prescription supplies and medication is on hand and correctly administered.

Qualifications
High School Diploma or Equivalent with a minimum of 1 year experience in the child welfare field, working with children and/or adolescents in a social service setting
1 - 3 years experience working with the program population or relevant direct service delivery in a residential setting,

Responsibilities Include:
Conduct individual psychotherapy sessions and psychiatric diagnostic evaluations for children and adolescents.

Required Qualifications and Skills:
Candidates are required to have a masters degree in one of the behavioral sciences or related fields from an accredited college or university and on track to obtain licensure in their designated field.
Minimum of 1-year clinical experience in providing psychotherapy and counseling to individuals with mental health presenting problems.
Candidates should have clinical licensure (LCSW, LSW, LPC, LAC.)
Required experience with completing psychiatric diagnostic evaluations/intake assessments, individualized treatment plans, and clinical follow-up sessions.
Effective written and verbal communication skills.
Ability to demonstrate and model stable, appropriate boundaries with clients.
Ability to complete and submit documentation of services and other documents in a timely manner.
Comfort and familiarity working with a diverse client base.
Proficient in the use of Office 365 and Electronic Health Record systems (i.e. Valant experience is a plus).
Fully Licensed Clinicians will ideally be credentialed with insurance panels.

The FPS program is mandated to provide services to families whose children are at imminent risk of placement, or whose children are being prepared for reunification. The FPS program works to ensure the safety of all family members, prevent the out-of-home placement of children considered to be at imminent risk of removal from their home, improve family functioning, reduce the recurrence of maltreatment, help families utilize community resources more effectively, and assist families with the process of reunification where separation or removal has already occurred.
Key Responsibilities:
Provide short-term, intensive, in-home counseling to 2 families at a time, each receiving a minimum of 5 hours of weekly face-to-face contact.
Teach, coach and model parenting skills and effective parent-child interactions and advocate and link families to concrete services and supports.

Starting at $57,300 MAJOR FUNCTION Under the direct supervision of the Senior Director of Behavioral Health and Clinical Supervisor of Behavioral Health, the Behavioral Health Counselor (BHC) provides behavioral health services in an integrated care and trauma informed manner. The BHC will work in a patient center and collaborative manner providing services to patients which includes but is not limited to brief intervention counseling, identification of social determinants of health, referrals to mental health and substance use services as appropriate, and follow up services to patients via face to face or through telemedicine. Is responsible to work independently, managing a diverse caseload; does related work as required.
ESSENTIAL FUNCTIONS
Direct Service:
Provide comprehensive assessment and diagnosis of behavioral health clients.
Provide effective treatment planning and assisting clients in successfully achieving goals.
Evaluate crisis situations and apply appropriate interventions.
Actively participate in meetings that support the integrated health care model to provide comprehensive care for clients.
Assist in the detection of “at risk” patients and development of plans to prevent further psychological or physical deterioration.
Assist the primary care team in developing care management processes such as the use of guidelines, disease management techniques, case management, and patient education to improve self-management of chronic disease.
Provide assessment, consultation, and brief intervention for psychological/psychiatric problems and/or disorders.
Teach patients, families, and staff care, prevention, and treatment enhancement techniques.
Monitor the site's behavioral health program, identifying problems related to patient services and making recommendations for improvement.
Complies with and meets the required standards for Joint Commission and HRSA accreditations.
Collaborates with all members of the Integrated Care Treatment team in service of the patient.

Program Development and Maintenance:
Develops and maintains liaison relationships with community resources as able, specifically other behavioral health and substance abuse agencies. Works to establish collaborative services and to lessen duplication of care.
In conjunction with other staff utilizes the electronic community resource directory provided by the Trenton Health Team.
Participates in the development, implementation, and evaluation of special projects and/or pilot programs.
Interprets Center behavioral health policies and procedures to providers, institutions, agencies, recipients, and other concerned individuals.
Establishes and maintains necessary records and files. Drafts correspondence in the course of official duties.
Collects, records, and analyzes significant program-related data; develops monthly statistical and programmatic reports.
Staff Development:
Serves as a full member of the HJAHC clinical staff team.
Participates fully in Behavioral Health departmental meetings and other agency meetings as warranted.
Participates in professional and related meetings, conferences, and continuing education training as available; prepares reports as requested.
Keeps appraised of developing areas of behavioral health and provides colleagues with updates as appropriate.
Assists in behavioral health departmental tasks, including programmatic and infrastructure design, implementation, and evaluation.
Learns and implements various types of electronic and/or manual recording and information systems used by the Center or its related units.
Assists with onboarding of new staff as supervising university interns as requested.

School Counselor Position Overview
The school counselor assists the Director in strengthening the alliance between home, school, and community while promoting emotional, social, behavioral wellness and academic success via various interventions and initiatives. Under the direction of the Chief Academic Officer/Director and Assistant Academic Director School Counselors will function as child advocates who believe students are first. Qualifications
