Position Summary:
The Social Worker is responsible to facilitate care along a continuum through effective resource coordination to help patients achieve optimal health, access to care, and appropriate utilization of resources, balanced with the patient's resources and right to self-determination. The individual in this position has overall responsibility for to assess the patient for transition needs including identifying and assessing patients at risk for readmission. Conducts complex psycho-social assessment and intervention to promote timely throughput, safe discharge, and prevent avoidable readmissions. This position integrates national standards for case management scope of services including: Transition Management promoting appropriate length of stay, readmission prevention, and patient satisfaction; Care Coordination by demonstrating throughput efficiency while assuring care is the right sequence and at appropriate level of care; Compliance with state and federal regulatory requirements, TJC accreditation standards, and Tenet policy; Education provided to physicians, patients, families, and caregivers; and Leads a population of patients by service line and/or leads the team by being a resource to Tenet performance standards.

The Social Worker is a member of a multi-disciplinary team that provides evaluations and interventions to children and families in the EI Program. The Social Worker assesses risk factors such as parental mental illness, substance abuse, domestic and/or community violence. They also provide training, education and resource support that responds to the child's special needs and the development of family relationships and routines. This position participates in evaluations, interventions, and development of the Individual Family Service plan (IFSP) for children and their families.
Requirements:
Must hold MA current licensure as a LCSW, LICSW, LMFT, LMHC, LMFT or LMHC license-eligible with proof of license eligibility.
Education commensurate with holding current licensure as listed above.
Minimum of 1-2 years of experience in a pediatric setting working with children under the age of 5. Experience working with children with developmental delays or disabilities preferred.
Must hold current, valid driver's license and the ability to provide proof of insurance coverage for travel between home visits.
Must hold and maintain current CPR and First Aid training.
Demonstrated knowledge of developmental milestones and delays in children, as well as the impact of emotional, behavioral, and neurological disabilities on the child's functioning and development. Must be flexible and adaptable in administering therapy.
Excellent interpersonal, written, and verbal communication skills with sufficient computer and technical skills to complete position required documentation.

The social work consultant shall have had prior experience with elders especially regarding placement and adjustment issues in nursing facilities. Position Summary:
The Social Worker is responsible to facilitate care along a continuum through effective resource coordination to help patients achieve optimal health, access to care, and appropriate utilization of resources, balanced with the patient's resources and right to self-determination. The individual in this position has overall responsibility for to assess the patient for transition needs including identifying and assessing patients at risk for readmission. Conducts complex psycho-social assessment and intervention to promote timely throughput, safe discharge, and prevent avoidable readmissions. The Social Worker is a member of a multi-disciplinary team that provides evaluations and interventions to children and families in the EI Program. The Social Worker provides therapeutic support to address social, emotional, and behavioral challenges for the child and family. The Social Worker assesses risk factors such as parental mental illness, substance abuse, domestic and/or community violence. They also provide training, education and resource support that responds to the child's special needs and the development of family relationships and routines. This position participates in evaluations, interventions, and development of the Individual Family Service plan (IFSP) for children and their families.
Requirements:
Must hold MA current licensure as a LCSW, LICSW, LMFT, LMHC, LMFT or LMHC license-eligible with proof of license eligibility.
Education commensurate with holding current licensure as listed above.
Minimum of 1-2 years of experience in a pediatric setting working with children under the age of 5. Experience working with children with developmental delays or disabilities preferred.
Must hold current, valid driver's license and the ability to provide proof of insurance coverage for travel between home visits.
Must hold and maintain current CPR and First Aid training.
Demonstrated knowledge of developmental milestones and delays in children, as well as the impact of emotional, behavioral, and neurological disabilities on the child's functioning and development. Must be flexible and adaptable in administering therapy.
