The SocialWorker role will be a tri-campus hub for triage, assessment, and evaluations to assist adult patients finding the right mental health care at the right time. This hub will unite existing staff across campuses to centralize and streamline access while decreasing redundant, unnecessary, and time-intensive work through optimization of our electronic medical record and with the use of innovative technologies. This role will support our strategic initiative to improve access to our outpatient programs, provide transparency to available services and treatment within our system, make rapid and appropriate recommendations for services to a larger outside ecosystem while supporting Ambulatory Services' mission to provide tertiary care to the most acute patients.
This is a unique opportunity for a clinically curious and diagnostically driven socialworker who thrives in a fast-paced, evaluation-focused environment. Unlike traditional roles, this position emphasizes breadth over caseload management-allowing you to engage with a high volume of new patients, sharpen your diagnostic skills, and contribute to clinical decision-making without the ongoing demands of long-term case management. You'll leverage advanced tools and technology to support timely, accurate assessments and help patients access the most appropriate care pathways. In addition to working closely with behavioral health colleagues, you will also evaluate patients referred from other NYP/Weill Cornell disciplines, offering a dynamic opportunity to collaborate across medical specialties and broaden your clinical exposure. As part of this innovative team, you will participate in daily triage and disposition huddles, collaborate with interdisciplinary colleagues, and provide psychoeducation to patients and families. This role is ideal for someone eager to deepen their expertise in psychiatric evaluation and be part of a transformative model of outpatient mental health care.
This is a full-time day shift position at NewYork-Presbyterian, based either at Westchester Behavioral Health Center.
Preferred Criteria
Knowledge of evidence-based short-term therapy such as CBT, IPT and/or trauma informed skills
Experience working in an interdisciplinary mental health evaluation and triage team
Licensed Clinical SocialWorker (MSW) by the State of New York
Minimum of 3 years related clinical experience
Required Criteria
Master's Degree in Social Work
Licensed Masters SocialWorker (LMSW) by the State of New York within 1 year of hire
Limited Permit (must supply exam date prior to start of employment)
Fieldwork or previous experience in a psychiatric setting
Demonstrated integrity and adherence to social work ethics and professional standards
Capacity to form a therapeutic relationship with a variety of patients and to maintain a neutral therapeutic stance
Knowledge base of bio-psychosocial model, Diagnostic and Statistical Manual of Mental Disorders IV (DSM-IV) diagnoses, criteria for treatment of different diagnoses in an acute care setting, behavioral risk factors and of the impact of mental illness upon individual and family functioning
Competency in individual, family and group treatment modalities and treatment techniques appropriate to an inpatient treatment setting
Ability to conduct initial evaluations and psychosocial assessments and to establish individualized and measurable treatment goals
Knowledge base of community and discharge planning resources and processes
Effective organizational skills to manage clinical, discharge planning and documentation requirements within assigned LOS of inpatient hospitalization
Ability to communicate effectively and to collaborate within an interdisciplina ry team
Demonstrated self-awareness to identify learning needs and capacity to apply supervisory feedback into practice

Job Description
An MSW, currently licensed (if applicable)and registered in the state of practice, who provides medical social services within the scope of MSW practice standards in collaboration with primary care physicians and other members of the health care team and teaches and educates patients and their families. Medical Social Services in the home is performed under the supervision of the Administrator, Director of Nursing.

Responsibilities:
Receives the intake referral information and prepares paperwork/tools necessary.
Obtains all pertinent medical history from patient, family or significant others.
Performs home safety check and environmental assessment of the patient's home environment.
Performs the socio-psychological evaluation of the support systems available to the patient and documents necessary emergency contacts etc. Assists patients and their families with personal and environmental difficulties which predispose then toward illness or interfere with obtaining maximum benefits from medical care (counseling members of the family to assisting patients with admission to a nursing home).
Performs the skilled visit and documents accordingly.
Provides supportive casework designed to restore patient's to their optimum level of social adjustment. This includes assisting patients and their families to understand, accept and follow medical recommendations.
Assists patients utilize the resources of their families and the community at large. Resource utilization may include referring the patients to community resources or acting as an intermediary on behalf of the patients with other health and welfare agencies.
Effectively communicates with patient and family. Keeps the patient informed ongoing.
Effectively communicates with other disciplines in the case (case conferencing) to effectively and appropriately problem solve as situations arise.
Communicates effectively with the Director of Nursing or Nursing Supervisor scheduled visits planned and changes to the schedule on a weekly basis. Caseload is self-scheduled but communication of the clinician's schedule is essential.
Communication with the patient's physician (verbally and/or in writing) to obtain effective treatment modalities to effect the best means to obtain the desired outcome.
Communicates in the case conferencing sessions to establish best practices for the individual patient's needs.
Submits accurate, complete paperwork at the end of every week so that all medical records are intact and up to date. Uses the drop box if the office is already closed for the weekend.
Coordinates Community Services that may be available to the patient to assist in safe home care needs.
Understands that the fiscal and clinical management of each case is directly linked to the success of his/her office. Assures that visits are not done that cannot be billed.
Participates in the QA Committee process as requested to do so.
Notifies the HCSF of emergencies, sickness, and other imminent occurrences that may affect the patient caseload as quickly as possible relative to the event's occurrence.
Submits written time requests 2 weeks or more in advance of planned time off.
