Responsibilities:
The socialworker is responsible for providing primary therapy to patients 18 years and older admitted to the Adult Partial Hospital Program (PHP). The socialworker conducts psychosocial assessment, treatment and discharge planning of patients assigned to his/her caseload. The socialworker is responsible for crisis intervention, maintenance of the therapeutic milieu, and assessment and management of patient safety. The socialworker may be responsible for leading therapeutic groups and conducting intake assessments. The socialworker develops and maintains relationships with community agencies and providers who refer to and receive patients from the PHP. The socialworker participates on a multidisciplinary team for the ongoing review of cases.
RESPONSIBILITIES:
Provides coverage for therapists in clinic per diem including performing psychosocial assessments of patients, behavioral health treatment and education to patient and families. This may be in the following modalities; individual, family and/or group therapy. Coordinates activities related to the comprehensive treatment and discharge planning for the patients assigned to his/her caseload.
Functions as a member of the interdisciplinary treatment team and contributes to the psychosocial assessment, treatment, and continuity of care for identified patients. Requests and coordinates psychiatric consultation when indicated. Secures authorization for treatment from patients' insurers, as indicated.
Collaborates with appropriate collateral contacts and provides linkages to community resources. Provides back-up as requested and is able to cover other units/programs during the absence of certain co-workers.
Documents treatment in patient's electronic medical record in compliance with Division, Department, and Hospital Policy.
Attends Social Work Division, Social Work in Psychiatry and Program staff meetings. Participates in activities of the Social Work and Psychiatry Division Psychiatry Division including, but not limited to, ensuring policies and procedures are implemented with regard to professional practice, discharge planning and patient care, timely completion of time reporting and other Hospital or Division forms; Hospital and Division mandatory in-service training programs and UHS reviews. Participates in routine committee process of the Social Work Division, and facilitates Division and Hospital quality assurance and staff development activities. Subscribes to the departmental goal of creating a culturally competent environment by treating patient, families, trainees, and co-workers in a sensitive manner with appropriate attention to cultural differences. Participates in available cultural competency events and training appropriate to job duties.

QUALIFICATIONS:
Master's degree from an accredited school of social work (CSWE) required
2-3 years professional social work practice beyond the Master's degree, at least one of which must include psychiatric emergency, crisis or outreach work; or an equivalent combination of education and experience and working with adult populations. Prior experience in providing individual or group therapy in an acute clinical setting preferred.
Knowledge about and ability to provide behavioral health assessments/evaluations and evidenced based interventions preferred;
Commitment to and routine demonstration of biopsychosocial approach to care of individuals and families preferred;

Master's degree required from an accredited school of social work. Actively participates in community events, agency meetings, staff trainings, and program development initiatives.
Maintains accurate, timely, and confidential case records; prepares required reports and statistics; and ensures documentation complies with program policies and procedures.
Participates in ongoing professional development through supervision, in-service and external trainings, conferences, and engagement with current, relevant professional literature.
Communicates significant client-related events to supervision in a timely manner and engages in regular supervision to support professional growth and quality service delivery.

SUPERVISOR: Preventive Program Supervisor
Under the general direction of the Preventive Program Supervisor, the Preventive SocialWorker provides home-based clinical counseling and case management services to youth and families who have been identified at risk of foster care placement or replacement. The Preventive SocialWorker carries out all job responsibilities with fidelity and accountability to the Agency's Mission, Vision, Values, Guiding Principles and Strategic Plan.
COMPETENECIES and ESSENTIAL FUNCTIONS:
Exercises full compliance with the Agency's Code of conduct, all Agency policies and procedures and Federal and State laws and regulations
Exercises full compliance with the Justice Center's Code of Conduct for Custodians of People with Special Needs
Employs approved crisis intervention / prevention techniques utilizing accepted practices per Agency policy and procedures
Reads, understands and accesses the program policy and procedures manual
Documents Safety Assessments in Client progress notes
Participates in regular staff meetings, ongoing training and staff development provided by the Agency
Demonstrates the ability to work in an electronic record system
Documents all case contact into the case record within 24 hours of service provision.
Utilizes appropriate measures to assess effectiveness of clinical work with youth enrolled in the program (Casey Life Skills Assessment, CANS & NC Assessment)
Demonstrates, promotes and practices cultural competency toward clients and staff through respect and understanding achieved through training and Agency cultural activities
Recognizes and embraces the diversity of teams, supports and develops the strengths of each individual youth and family
Assesses, develops and implements individualized goals and objectives identified by the family, foster/resource family, service provider and referral source
Documents all contacts with youth and family including goals, objectives, methods and maintains the clinical record and all other pertinent documentation in accordance with Agency standards
Conducts comprehensive strengths and needs assessment with each youth and family.
Completes all agency and funder required documentation related to opening and closing cases with in specified timelines
Utilizes flex funds for clients in a prudent manner, following required guidelines to obtain approval, and document use of such funds
Accurately documents mileage and completes reimbursement documentation
Assists management in the delivery of the Sanctuary Model aligning operational areas with values of the model
Provides individual and/or family therapy within a home-based setting
Assists the youth and family in managing and planning for safety needs and creates an individualized support plan when indicated
Ability to arrange schedule to be available to youth and families in their homes/communities
Acts as an advocate as appropriate, and coordinates linkage and referral to community resources for youth and family needs, including social, recreational, food housing etc.
Maintains ongoing contact with the referral source, school partners and preventive liaisons, through face to face, phone and electronic communication, for the purposes of service coordination
Serves as a positive representative of the Agency and of Preventive Services with youth and their family, community resource personnel and Monroe County representatives.
Perform other similar duties requiring comparable skills utilizing the Sanctuary Model Guiding Principles and Seven Commitment

Available to work morning, afternoon and evening hours Mon-Fri in the clients' homes/community
Comfortable with community based work with high risk youth and their families in high risk neighborhoods

Responsibilities:
The socialworker assigned to URMC Mobile Crisis Team functions as a crisis therapist and provides mental health assessment, and crisis intervention and treatment to complex mentally ill patients who are at risk in a variety of outreach settings, including group homes, shelters, patients' homes, and other sites across Monroe County. The Socialworker on the Mobile Crisis Team works closely with a Crisis Specialist. The socialworker is also an integral member of the Mobile Crisis Team, providing psychosocial assessment, crisis intervention, resource consultation, and brief treatment, minimizing psychosocial risk and decreasing Emergency Department utilization.
RESPONSIBILITIES
Provide psychosocial assessment to patients in outreach settings (i.e., group homes, shelters, patients' homes) who are at exceptional psychosocial risk as needed. Provides psychiatric crisis intervention and treatment in the community to help stabilize patients. Provides clinical leadership to Mobile Crisis Team in the field. Exercises considerable independent judgment with respect to assessment and treatment of psychiatric patients in outreach settings.
Responsible for ensuring assessment of safety of scene prior to deployment of Mobile Crisis Team and accessing need for law enforcement services to ensure safety of Mobile Crisis Team in the field. Involves family and other mental health treatment providers as necessary in interventions.
Provides clinical leadership to Mobile Crisis Team in the field. Exercises considerable independent judgment with respect to assessment and treatment of psychiatric patients in outreach settings.
Documents treatment in patient's electronic medical record in compliance with Division, Department, and Hospital Policy.
Attends Social Work Division, Social Work in Psychiatry and Program staff meetings. Participates in activities of the Social Work and Psychiatry Division Psychiatry Division including, but not limited to, ensuring policies and procedures are implemented with regard to professional practice, discharge planning and patient care, timely completion of time reporting and other Hospital or Division forms; Hospital and Division mandatory in-service training programs and UHS reviews. Participates in routine committee process of the Social Work Division, and facilitates Division and Hospital quality assurance and staff development activities. Subscribes to the departmental goal of creating a culturally competent environment by treating patient, families, trainees, and co-workers in a sensitive manner with appropriate attention to cultural differences. Participates in available cultural competency events and training appropriate to job duties
Ensures that proper treatment and referral protocols are followed with respect to child abuse, domestic violence, consultation with specialty clinics, and referral both within and outside of SMH.
Participates in on-call activities (rotational basis) of the Mobile Crisis Team, including after hours. Other duties as assigned.
Teaching: Based on experience and as determined by the social work supervisor, the socialworker may supervise social work or other master's level graduate students. Upon invitation from the Program Director and in consultation with the social work supervisor, he/she may offer learning experiences for psychiatric residence, medical students, and family medicine residence. Supervision relates to psychosocial assessment and intervention with patients and their families.
Research: Based on experience/interest, may participate in interdisciplinary research as determined by the Social Work Supervisor, in consultation with the Social Work Clinical Manager in Psychiatry.
QUALIFICATIONS
Required:
Master's degree from an accredited school of social work (CSWE)
Master's degree from an accredited school of social work. Licensure as a socialworker, as defined by the State Board for Social Work, State Education Department, Division of Professional Licensing Services is required. If licensure has not been obtained upon employment, the individual has 18 months within which to obtain it, unless duties will be performed under OMH then minimum of a limited permit (not to exceed 1 year) or NYS Licensure will be required upon employment start.
Must possess a valid New York State Driver's license and a satisfactory driving record.
Preferred:
2-3 years professional social work practice beyond the Master's degree, at least one of which must include psychiatric emergency, crisis or outreach work; or an equivalent combination of education and experience. and working with adult and/or pediatric and adolescent populations.
