    We are seeking a Social Worker to join the Palliative Care team. The role of the PACT Social Worker is to support PACT inpatients and outpatients in partnership with the interdisciplinary palliative care team. The PACT social worker joins the Palliative Care IDT to engage in family decision-making and goals of care discussions, assist with developing an end of life care plan as appropriate, initiate / support hospice referrals and discharge coordination, and support transition to bereavement when indicated. The PACT social worker is responsible for maintaining EMR documentation, participates in division quality improvement activities, is able to participate in research activities if interested, and other duties as assigned.
