As a School Social Worker, you are primarily responsible for helping students in Grades K- 12 succeed academically, socially, and emotionally, through individual and group counseling and by collaborating with educators, parents, and other professionals to create a safe, healthy, and supportive learning environment that strengthens connections between home and school.
‖ Responsibilities Include:
Providing individual and group counseling sessions to address interpersonal or family issues that interfere with school performance, including crisis intervention consultation and/or family therapy as needed.
Maintaining all required paperwork including but not limited to communication logs and meticulous notes regarding all services provided to students.
Conducting interviews with students, family members, and school personnel to gather information required to assess student social, emotional and behavioral adjustment to school and the community.
Maintaining regular contact with students' parents/guardians to inform of student's progress, areas of difficulties, and any changes or situations in home life that may affect school performance.
Developing and updating Individualized Education Plans (IEP) and Behavior Intervention Plans (BIP) as needed in adherence to the company, school, state and federal regulations and procedure.
Attending and participating in IEP team meeting, facilitating at the discretion of the school districts.
Actively participating in other team meetings when necessary to address specific student and parent concerns.
Collaborating with education and social services professionals (for example wrap- around, probation officers, court systems) to develop appropriate agency services for students and their families.
Providing case management functions for students, which include but are not limited to, coordination of parent meetings, emergency transportation requests, and coordination with therapists outside the school setting.
Collaborating and consulting with teachers, parents/guardians, district personnel, and service providers to find effective solutions to learning and behavior problems; always establishing and maintaining effective public relations and projecting a positive company image.
Ensuring the use of positive social skill teaching methods by conducting monthly direct observation of classroom staff's implementation of IEP goals and/or Behavior Intervention Plans/ Treatment Plans.
Providing guidance, consultation, and assistance to teachers in the development of classroom schedules, maintenance of student data systems and facilitation of student transition plans to assure appropriate schedules and services based on each student's Individual Education Plan (IEP) and/or other applicable goals.
Assessing student progress through consistent review of classroom data collection and recording systems and providing applicable training and guidance to classroom staff accordingly in addressing student behavioral needs.
Conducting crisis assessments and contacting the appropriate authorities if needed to ensure that emergency intervention is provided to students.
Keeping up to date with research-based practices and developments in subject area.
Responding constructively to formal and informal feedback.
Performing other duties as assigned.
‖ Qualifications Required:
Master's degree or higher in social work, educational counseling, therapy or a closely related field of study.
Licensed currently or in the process of obtaining an IL professional educator license (PEL) school support personnel endorsement.
Licensed currently or in the process of obtaining a licensed clinical social worker (LCSW) credential.
Ability to obtain and maintain certification in company approved crisis management (PCM) training.
Prior experience and/or knowledge in special education services and compliance, particularly for students with severe learning disabilities, emotional disturbance and/or behavioral disorders.
Well-versed in applied behavior analysis (ABA) including positive behavior interventions and supports (PBIS), functional behavior analysis (FBA) and behavior intervention plans (BIP's).
Proven success in developing and implementing effective intervention plans for students with diverse needs, preferably in an alternative, special education and/or behavioral health program setting.
Proficiency in providing motivation and having critical conversations with students that help move them toward realistic goals.
Ability to think and act quickly and calmly in an emergency and make independent decisions.
Advanced oral and written communication, instruction, curriculum, conflict resolution, multitasking, problem solving, and decision making ability.
Well-versed in the use of relevant technology including experience with computers, Microsoft Office Suite, database entry and basic office equipment.

The Crisis Recovery Center
The Crisis Recovery Center (CRC) is the critical next step in DuPage County Health Department's decades-long journey to build the infrastructure needed to support a best-in-class behavioral health system and ensures all DuPage County residents will have "someone to contact, someone to respond, and a safe place to get help" when experiencing a mental health or substance use crisis. The CRC will help residents de-escalate, stabilize, and connect to community resources catered to their individual mental health and substance use treatment needs. The CRC serves as an alternative to hospital emergency departments and simplifies access to crisis care for all individuals, families, and first responders. Job Details
This full-time position is scheduled to work Monday - Friday, 8:00 am - 4:00 pm. This position requires flexibility in hours to accommodate varying staff schedules. The role includes on-call responsibilities that rotate weekly among the team of Crisis Supervisors.

Position Summary

Clinicians manage their own caseloads with full administrative support, including scheduling, billing, credentialling, AI-enabled automated documentation (with client consent), and marketing.
Compensation
At Tailwinds, we believe every full-time licensed clinician deserves to earn at least six figures (plus benefits) on an annual basis within two years or less. Compensation is based on license type, licensure status, and tenure with the practice, and is calculated per billable clinical hour (i.e., hours billed to the client or insurance). Full-time clinicians are expected to maintain an average caseload of 25 clients per week and are eligible for benefits.

· Year 2: 55%

Base earnings range reflects good-faith estimate of gross annual earnings for a Year 1 clinician with an average caseload of at least 25 clients/week.

Qualifications
An Average Day:
(Includes, but not limited to)
Assess the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment and communicates findings to the registered nurse and other members of the interdisciplinary group
Provide an assessment in the patient's identified residence and assistance when this is not safe, and another plan is required
Carry out social evaluations, including family dynamics, caregiver abilities, communication patterns, high risks for suicide, neglect or abuse and plan intervention based on evaluation findings
Counsel patient and family/caregivers as needed in relationship to stress, and other identified coping difficulties; provide crisis intervention when necessary
Assess for, and educate interdisciplinary group, on any special needs related to the culture of the patient and family, including communication, role of family, space, and any special traditions or taboos
Maintain clinical records on all patients referred to social work
Educate patients and families on, and assist in, preparation of advanced directives
Provide information and referral services for organization patients and families/caregivers regarding practical and environmental needs
Provide information to patients and families/caregivers and community agencies
Serve as liaison between patients and families/caregivers and community agencies
Maintain collaborative relationships with organization personnel to support patient care
Maintain and develop contacts with public and private agencies as resources for patient and personnel
Participate in the development of the individualized plan of care, involving the patient and family, and attend regularly scheduled interdisciplinary group meetings, assist the team in recognizing the effects of the psychosocial stresses on symptoms of the terminal illness
Assist physician and other team members in understanding significant social and emotional factors related to health problems and death/dying issues
Actively participate in quality assessment performance improvement teams and activities
Assist family and patient in planning for funeral arrangements, financial, legal, and health care decision responsibilities
Perform other duties as delegated by the Clinical Supervisor
Requirements:
Must have a master's degree in social work from an accredited university.
Must have a LCSW
Must have minimum of two (2) years of documented supervised experience in health care, hospice experience preferred
Must understand hospice philosophy, and issues of death/dying.
Certified Hospice and Palliative Social Worker is desirable
Must be flexible in work hours and have the ability to travel throughout the assigned Bristol Hospice service area
Must demonstrate a willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs
