Position Summary:
The Manager of Social Work & Care Coordination is responsible for providing programmatic and clinical direction, oversight, and guidance to diverse teams of Care Coordinators and Social Workers who serve populations of patients with complex psychosocial and medical conditions. Oversee implementation and refinement of designated program procedures, workflows, and measures to align with contract deliverables and business needs.
* Partner with external teams and organizations to strengthen core services and workflows such as transitions of care, complex behavioral health, and other processes impacting social drivers of health. Partner with marketing and business analytics to promote programs and services. Play a key role in alternative funding source procurement and sustainability such as grant writing contributions, funder reporting and project implementation.
* Identify, advocate for, develop and implement needed reports, data analytics solutions, documentation improvements and other program enhancements as needed. Continuously align work priorities with organizational goals, adjusting strategies and programs accordingly.
* Oversee the provision of Social Work and care coordination expertise for complex patient or care team scenarios especially for situations involving domestic violence, homelessness, trauma, substance use, crisis intervention, complex family dynamics, newly arrived refugees and other complex social or behavioral health situations. Promote patient self-management, self-determination, and person-centered care. Provide case consultation as needed.
* Provide clinical oversight and evaluation of various screening tools and Social Work interventions such as PRAPARE or other Social Determinant of Health screening, KATZ, GAD-7, PHQ-9, Motivational Interviewing, psychosocial assessments, and safety planning with patients. Supplement learning and expertise of all reporting staff and leadership within discipline, implementing changes when indicated.
* Collaborate with Supervisor to collect, analyze and present data regarding the performance of designated care coordination and Social Work programs including enrollment, capacity metrics and financial status to inform and expand services as needed. Run and maintain regular reports and utilize electronic resources to meet program, budget, and contractual requirements. Present data and information to internal or external groups as needed.
* Collaborate with Supervisor to lead teams and facilitate communication between staff, clinic leadership, care teams, and related care coordination programs aimed at improving understanding of Social Work and care coordination services including metrics and program alignment.
* Oversee Supervisor to ensure appropriate orientation and program training for assigned staff. Provide additional clinical training and support as needed.
* Provide coaching and guidance to maintain professional relationships and boundaries as staff support patients, families or caregivers with empathy, compassion and cultural congruence while maintaining respect for confidentiality, privacy, and mandated reporting.
* Maintain good attendance and punctuality.
* Demonstrate respectful, professional, and appropriate behavior that supports a team-oriented work environment.
* Demonstrate a commitment to the mission, core values and goals of HealthPoint and its healthcare delivery including the ability to integrate values of justice, respect, compassion, excellence and stewardship into appropriate programs and services.

Must have's you'll need to be successful:
* Master's Degree in Social Work (MSW) and at least five (5) years of experience with at least one (1) year of supervisory experience required.
* Previous experience in a clinic or hospital setting, working with vulnerable populations, behavioral health or community health required.
* Licensed Independent Clinical Social Worker (LICSW) required within 90 days of hire.
* Maintain licensure in good standing as a condition of employment.
* Valid Washington State Driver's License with an acceptable driving record determined by HealthPoint's insurance carrier.
* Possesses intermediate operating knowledge of computers.
* Advanced experience with Word, Excel, and Outlook required, willing to learn OTTO or any other electronic telehealth platform.
* At least two (2) years of Electronic Health Record (EHR) experience required.

POSITION SUMMARY:
Holt International finds and supports permanent, loving families for children who are orphaned, abandoned or at serious risk of separation from their family. The master's level social worker represents Holt in their community and ensures the successful adjustment of child and family by conducting an assessment of the family, and providing appropriate services throughout the adoption process. The master's level social worker works cooperatively with families, the Branch Office and Adoption Teams headquartered in Eugene, Oregon. Note: The Adoption Social Worker is primarily a home-based position. ESSENTIAL JOB FUNCTIONS (to be completed within timeframes outlined by the Branch Director):
1. Determines a family's ability and capacity to parent an adopted child by conducting pre-home study meetings as requested, reviewing and assessing agency-requested information, evaluating the family's knowledge and perceptions of adoption, teaching adoption education courses as needed, and conducting psycho-social assessments of family members.
2. Contributes to the delivery of high-quality adoption services by producing adoption home studies and post placement reports that comply with provided guidelines and meet current agency, state, country and USCIS requirements.
3. Provides support and case management of all assigned families by maintaining quarterly contacts throughout process, communicating information to the family, referring family to appropriate community support systems when necessary and documenting all correspondence in Holt's database.
4. Prepares family for travel and assists with post placement adjustment by providing training and identifying resources available to help meet the child's needs and aid in the child's adjustment to the family. Conduct home visits as required by state law and country program guidelines, assist in early identification of problematic issues, and make a determination regarding the stability and appropriateness of the placement.
5. Assures child's physical and emotional well-being and eases family's adjustment by providing counseling to the child and family and/or helping arrange for counseling with a clinician outside the agency if needed. In case of an adoption disruption or dissolution, maintain relationship with family to ensure transition plan is developed that meets the child's needs. Assist child and family with transition of child to new placement.
6. Contributes to public knowledge about Holt and supports Holt programs by participating in activities arranged by Holt including picnics or parent groups, representing Holt at adoption meetings, coalitions and organizations, and performing other outreach activities as appropriate.
7. Maintains professional knowledge by attending relevant seminars and course work, keeping informed about State regulations, legislation, adoption issues and child-related issues overseas; maintaining license, if required.
8. Contributes to a positive, productive work environment by meeting attendance and punctuality guidelines and pre-arranging time off with appropriate notice; treating all people with dignity and respect; and exhibiting good listening and comprehension skills including giving and welcoming feedback.
9. Contributes to team effort by working cooperatively with Branch and Eugene offices, respectfully responding to all communication from Branch and Eugene in a timely manner. Providing back-up to colleagues as needed and supporting branch office in meeting objectives.
