• Assesses and implements discharge planning activities and makes appropriate recommendations regarding discharge needs, including home evaluations and equipment orders when necessary.
• Works with outside vendors to order patient specific equipment when funding has been verified.
• Provides staff training to promote patient progress re: equipment, transfers, mobility, etc.
4. Promotes education and participates in department tasks.
PERFORMANCE STANDARDS:
• Attends continuing education for licensure requirements and keeps abreast of current/new treatment approaches and/or equipment.
• Attends mandatory meetings, functions in multiple clinical areas and works at multiple location sites as needed.
• Participates in department management tasks; supervises and/or provides in-services and training for licensed assistants, rehab tech, students, hospital staff, and volunteers.
• Offers suggestions for process improvement; recommends to the supervisor/manager equipment and/or supplies that may enhance Rehab Program.
5. Performs other duties and special projects as required.

SPECIAL SKILL, LICENSE AND KNOWLEDGE REQUIREMENTS
Current Louisiana licensure as a Physical Therapist
Oral and written communication skills required.

JOB DESCRIPTION
This is a professional counseling position that involves the treatment of individuals with substance use disorders and other co-occurring conditions. Treatment involves facilitation of individual, group, and family counseling sessions, assessments, treatment planning, care coordination, case management services, discharge planning, and documentation. The clinician must possess expertise in counseling principles and techniques. Services are provided in accordance with recognized counseling principles and techniques and within the established policies, procedures and guidelines of the organization. The Clinician will work under the supervision of the Clinical Director and/or Clinical Supervisor.
RESPONSIBILITIES
Conduct group, individual, and family counseling sessions with clients, administer appropriate clinical assessments/ evaluations as needed, provide care coordination and case management as appropriate, develop treatment plans and discharge plans with the clinical team.
Complete intake documentation up client admission.
Document, review, and maintain detailed case records on all clients.
Participates in all staff conferences regarding the evaluation of client's progress and future treatment goals.

Adhere to the Policies and Procedures or of LHRC, agency Programs, CARF and Louisiana State Licensing Guidelines.

Provide addiction- prevention resources to the community and assist with organizing public outreach to educate people on risks associated with alcohol and drug abuse as needed.

REQUIRED QUALIFICATIONS:

About the Role
Charlie Health is looking for a dynamic individual to serve as Clinical Admissions Therapist. The Clinical Admissions Therapist will be responsible for meeting with clients upon admission, building rapport, completing biopsychosocial assessments, and establishing treatment needs.

This is an in-person position based in Baton Rouge, LA, with required in-office hours from 9:00 AM to 5:00 PM, Monday through Friday.
Responsibilities
Demonstrate personable and competent communication skills to develop rapport with clients and referral sources
Present programming information in a thorough and supportive manner to help clients gain access to care
Complete virtual biopsychosocial assessments for a wide range of ages, including children, teens, young adults, and adults with various mentalhealth concerns
Determine appropriateness and fit for virtual intensive outpatient program (IOP) level of care
Construct provisional mentalhealth diagnoses utilizing DSM-V criteria
Document client information in accordance with regulatory standards using electronic medical record
Assign treatment team members and group schedules while informing staff of new admissions
Interface with Admissions, Verification of Benefits, Utilization Review, and Clinical teams in facilitating intake
Collaborate with referral sources: hospitals, treatment centers, psychiatrists, therapists, and other behavioral health providers upon admission
Work authorized in the United States and native or bilingual English proficiency
Strong communication and interpersonal skills
Access to reliable technology resources to deliver uninterrupted, confidential, and compliant telehealth services
Ability to thrive in fast-paced environment while maintaining high attention to detail and quality of care
Requirements
Experience working with a wide range of ages, including children, teens, young adults, and adults preferred
Experience completing behavioral health assessments and admissions preferred
Independent licensure in a mentalhealth field required
An independent license is preferred, but we're happy to work with associate-level clinicians who are eager to grow and work toward full licensure.

The expected base pay for this role will be between $60,000 and $70,000 per year at the commencement of employment. However, base pay will be determined on an individualized basis and will be impacted by location and years of experience. Further, base pay is only part of the total compensation package, which, depending on the position, may also include incentive compensation, discretionary bonuses, other short and long-term incentive packages, and other Charlie Health-sponsored benefits.
