What You Will Do to Change Lives
Under direct supervision, the Case Manager II serves as a primary support and case manager for individuals with mental illness and other co-occurring disorders by assisting them to live as independently as possible in their chosen community.
Shifts Available:
Full-Time | DAYS | Shifts: 8:00 AM - 4:30 PM | Days: Monday - Friday; Plus on-call rotation
Expected starting wage range is $21.00 - $22.52. Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.

a. Periodically serves as clinician-of-the-day by being available for on-call during scheduled work days for patient emergencies;

c. Conducts clinically sound suicide risk evaluations including clinical review, applying suicide risk assessment protocols, pertinent data/chart reviews, proper documentation and consultations as needed;
d. Meets with mentally ill patient-inmates/youths for crisis intervention, group psychotherapy, face-to-face interviews, etc. May be required to provide clinical, face-to face interviews at cell fronts in inmate housing units. In some institutions, multi-tiered housing units may require the ability to climb stairs and walk long distances;
e. Identifies the relevant signs and symptoms of psychiatric disorders in orders to comprehensively assess diagnose, and manage the presenting symptoms;
f. Writes clinical reports regarding diagnoses and prognoses and develops treatment plans that reflect the standard of practice;

Qualifications

The Clinical Social Worker (H/CF, Safety) is under the administration and clinical supervision of the Chief of Social Work Services through the Supervising Psychiatric Social Worker I at the Department of State Hospitals - Atascadero (ASH). The incumbent will be responsible for the following duties, but not limited to:
* Conduct or co-lead weekly treatment groups.
* Meet with and provide social work services for patients who have identified need, as defined by the Social Work Service.

* Applies and demonstrates knowledge of correct methods in Therapeutic Strategies and Interventions (TSI). * Participate in all Interdisciplinary Treatment Team reviews of each patient on assigned caseload; provide input on progress in treatment; report on family, social and community resources; identify current and ongoing treatment needs, and make recommendations.
* Write clinical progress notes describing and evaluating the patients' responses treatment, as defined by the Social Work Service.
* Collaborate with the Interdisciplinary Treatment Team, State's Conditional Release Program, Department of Corrections, Parole Agents, and/or representative from the dispositional setting to formulate an outpatient treatment contract/discharge plan and facilitate the patient discharge to the dispositional setting.

What You Will Do to Change Lives
The Clinician Unlicensed provides mentalhealth support services to members served and their families at the program. The Clinician also assists in the coordination of all mentalhealth support services provided by the Team. Qualifications

OTHER SKILLS AND ABILITIES:

Bilingual in Spanish required.
Strong organizational, communication and interpersonal skills.
Valid driver's license is required for transporting youths. Any changes that occurs on your driving record must be reported immediately to supervisor and Human Resources.

We are seeking a Licensed Clinical Social Worker (LCSW) to provide high-quality mentalhealth services to individuals, families, and groups. The ideal candidate will have a deep commitment to client care, cultural sensitivity, and clinical excellence.
Position Type: Full-Time
Compensation: $115,000 to $120,000 a year plus benefits
Location: San Luis Obispo, California (In-Person, Hybrid, or Flexible Scheduling Options Available)
Job Duties:
Conduct comprehensive assessments and develop individualized treatment plans
Provide evidence-based therapeutic services to a diverse client population
Maintain accurate and timely clinical documentation
Coordinate care with other providers and community resources
Participate in clinical team meetings and ongoing training
Requirements
Current and active LCSW license in the state of California
Master's degree in Social Work from an accredited program
Experience in providing mentalhealth services (minimum 1-2 years preferred)
Excellent communication, organizational, and problem-solving skills
Ability to work both independently and collaboratively
