The NursePractitioner provides primary care to participants. Performs physical assessments of new PACE Enrollees, semi-annual reassessments of participants and develops and implements appropriate plans of care for program participants. SUMMARY OF JOB RESPONSIBILITIES:
The Hospice and Palliative Medicine NursePractitioner (HPMNP) is an advanced practice nurse, providing clinical care in accordance with state and federal practice law for patients in all hospice and palliative care settings. The HPCNP works in collaboration with the Hospice and Palliative Medicine Physician (HPMP) and the hospice interdisciplinary group (IDG) to ensure patients receive appropriate symptom control; facilitates discussions relative to the plan of care and advance directives; and provides critical regulatory and compliance requirements including face-to-face and billable/non-billable visits.
The HPCNP collaborates externally with attending physicians, specialists, and other providers as applicable for the purpose of coordination of care to effectively meet the medical, psychological, and spiritual needs of the patient.
QUALIFICATIONS:
Board Certified Advanced Practice Nurse licensed in the state of Colorado.
Prefer two years of experience in home care, palliative care, or oncology.
Seeks or maintains certification in Palliative Medicine.
Controlled substance license with current DEA license number.
Must have and maintain certification (or ACHPN) or obtain no later than the end of third year of employment.
Current CPR Certification.
Eligible or currently possess a Medicare/Medicaid UPIN number.
Eligible or currently credentialed at local hospitals.
Eligible to bill insurance for palliative care and symptom management visits.
Valid Colorado driver's license and unlimited access to reliable transportation.
Adequate automobile insurance.
Able to work in a variety of environments.
