What You'll Do:
As a RN Designee you will lead an interdisciplinary team to provide exceptional care and ensure the health, comfort, and overall well-being of our residents. Your responsibilities will include oversight of the facility operations, safety, cleanliness and compliance to nursing policies, procedures and regulations.
Key Responsibilities:
Supervising and guiding nursing staff to ensure high-quality care delivery
Overseeing daily operations, including staffing assignments and resource allocation
Collaborating with the interdisciplinary team to develop and implement care plans
Conducting regular assessments and audits to maintain quality standards
Providing mentorship, training, and support to nursing staff
Ensuring compliance with all regulatory standards and protocols

Qualifications of a RN Designee include:
Valid state nursing license
Advanced degree or certification preferred
Experience in a supervisory or leadership role in a Long-Term Care setting preferred

Weekend Field RN Role Overview
The Field RN/Coordinator of Care is a vital member of the home care team, managing all aspects of patient care. This full-time, permanent position follows a Monday through Friday schedule, offering stability and flexibility. Responsibilities include conducting home visits, developing and implementing care plans, coordinating with interdisciplinary teams, and ensuring accurate and timely documentation.
Weekend Field RN Key Responsibilities
Perform home care assessments to determine eligibility and develop care plans.
Provide skilled nursing care, including wound care, medication management, and disease education.
Coordinate and communicate with patients, families, physicians, and interdisciplinary teams.
Complete documentation in HCHB clinical software within required timeframes.
Supervise and orient home health aide personnel as needed.
Identify needs for additional services (e.g., PT, OT, MSW, speech therapy) and coordinate accordingly.
Monitor patient progress, adjust care plans as needed, and participate in case conferencing.
Ensure compliance with infection control standards and agency protocols.
Weekend Field RN Requirements
Registered Nurse (RN) licensed in New York State.
Minimum of 6 months clinical, hospital experience.
Vehicle required for most territories.
Familiarity with EMR systems; experience with HCHB is a plus.
Current physical examination.
Weekend Field RN Benefits:
$40,000 total tuition reimbursement, $10,000 yearly.
