This role is essential in providing comfort-focused, patient-centered care to individuals in their homes during end-of-life care. The ideal candidate is caring, patient, reliable, and committed to maintaining the dignity and comfort of our hospice patients. Key Responsibilities: Provide personal care to hospice patients, including bathing, grooming, oral care, and dressing. Assist with mobility, transfers, ambulation, and positioning for comfort. Support patients with light housekeeping, and maintaining a safe home environment. Report changes in patient condition, behavior, or environment to the assigned Case Manager promptly. Follow and implement the patient's plan of care as directed by the RN. Maintain accurate, timely, and complete visit documentation in accordance with agency policy and hospice regulations. Adhere to all infection control, safety practices, HIPAA, and agency protocols. Participate in team meetings and in-service training as required. Qualifications: Active Home Health Aide (HHA) Certificate in the state of California (required). Hospice or home health experience preferred, but not required-training provided. Current CPR Certification. Valid California Driver's License, reliable vehicle, and proof of auto insurance. Ability to work independently in the field and follow care plans accurately. Strong communication skills and a compassionate, patient-centered approach.
