Provides skilled treatments to patients as prescribed by the physician and directed by a licensed physicaltherapist as specified in their health care service setting.
Job Responsibilities
Implements treatment plan as developed by the licensed physicaltherapist.
Collaborates with the licensed physicaltherapist on an agreed upon frequency to assess and reassess patient's plan of care, treatment goals and interventions.
Documents patient progress in daily notes and weekly notes (as applicable), as well as contributes recommendations to patient's discharge note from physical therapy.
Assists with guidance for clinical fieldwork students under the supervision of the physicaltherapist.
Reports on interventions and patient progress during assigned meetings and contributes to the screening and evaluation process.
Boone Health has determined this is a safety-sensitive position. The ability to work in a constant state of alertness and in a safe manner is an essential function of this job.

The PhysicalTherapistAssistant - Journey Traveler must be able to travel nationwide for 13 week assignments. This position provides flexibility to travel the country and work in various settings. On assignment the PhysicalTherapistAssistant provides physical therapy treatments to patients to facilitate increased independence and functioning under the supervision of a Licensed PhysicalTherapist.
Attends and contributes to patient care, staffing conferences and other required meetings as a representative of physical therapy, under the direction of the physicaltherapist.
Responsible for the effective and efficient delivery of rehab services; adherence to an established plan of care, department and patient(s)' schedule, and timely documentation of services.
Works with other members of the rehabilitation team to implement programs and activities consistent with the needs and capabilities of each patient.
Acts as an assistant to the PhysicalTherapist when the PhysicalTherapist is performing tests, evaluations, and complex procedures.
Instructs patients, families and other caregivers in the skills and techniques of the physical therapy treatment program under the supervision of the PhysicalTherapist.
Maintains appropriate and timely documentation for all patients treated.
Cleans, maintains, identifies, and reports the need for repair of equipment, as necessary.
Puts Patient Service First: Ensures that patients and families receive the highest quality of service in a caring and compassionate atmosphere which recognizes the individuals' needs and rights.
Implements a patient's individualized treatment plan as established by the primary PhysicalTherapist.

Qualifications
* Graduate of an accredited PhysicalTherapistAssistant Program certified by the American Physical Therapy Association.

General Purpose The Staff Occupational TherapistAssistantassists the Staff Occupational Therapist with patient related activities and direct patient care. • Treat patients as directed by the Occupational Therapist. • Record daily treatment notes and weekly progress notes per OT Board.
• Treat patients per the physician treatment plan.
• Assist nursing department with training of Restorative Aides.
• Communicate with supervisor and other health team members regarding patient progress, problem and plans.
• Participate in in-services training program for other staff in the facility.
• Record evaluations, daily treatment notes, weekly progress notes, recertification documentation, per Occupational Therapy Board State Practice Act and governmental and third-party payer requirements.
• Record treatment changes per policy and procedures.
• Instruct patient's families or nursing staff in maintenance program and caregiver training in preparation for discharge from therapy services.
• Participate in discharge planning.
• Assist with cleaning and maintenance of treatment area.
• Report any problems with department equipment so that it is maintained in good working order.
• Ability to relate positively, effectively, and appropriately with patients/residents, families, staff and professional colleagues.

Qualification
Education and/or Experience
Licensed as a Staff Occupational Therapy Assistant Board of the State.
Proficient in computer skills such as inputting data into programs such as Casamba and Point Click Care.

Ability to effectively present information and respond to questions from managers, coworkers and families.
Mathematical Skills
Ability to apply concepts such as fractions, percentages, ratios and proportions to practical situations.
