Horizon Health has an exciting opportunity for a Licensed Therapist/ Social Worker on its behavioral health unit at Joint Township District Memorial Hospital in St. Mary's, Ohio. The Therapist/ Social Worker is responsible for providing direct high quality psychotherapy services to patients in assessments, diagnoses, treatment planning, group treatment, individual treatment, family treatment, and discharge planning. They also assists patients and their families in understanding and adjusting to diagnoses and potential treatments. Social Workers can also help identify resources during treatment and after discharge. The Social Worker effectively participates as a member of the multidisciplinary team and the unit community relations team.
Responsibilities:
Implements a treatment plan of care for patients based on a thorough assessment of the patient's symptom presentation, professional/referral input and all other available data.
Appropriately participates in referral development processes and activities as directed/requested
Engages in timely, efficient discharge planning. Works in close conjunction with patient family, nursing staff and attending physician. Displays thorough knowledge of referral/placement process and available community resources and makes effective contact with disposition sources
Participates as an active member of the interdisciplinary and treatment teams. Attend Unit and Hospital meetings as directed
As directed, conducts individual, group and education psychotherapy sessions in accordance with state/regulatory standards, displays sufficient theoretical knowledge of psychopathology and effective treatment techniques.
Conducts therapies in accordance with patient diagnosis and needs
Provides appropriate resources and support for patients before, during, and after treatment
Submits timely and accurate CQI+ outcome data on a weekly basis
Performs other duties as assigned/required by this position

The Physical Therapist Assistant assists the Physical Therapist in the provision of physical therapy. The incumbent shall perform physical therapy procedures and related tasks that have been selected and delegated by the supervising Physical Therapist. The Physical Therapist Assistant may modify a specific treatment procedure in accordance with changes in patient status within the scope of the Physical Therapist's established treatment plan. The incumbent documents patient progress notes. The incumbent shall safety and effectively provide physical therapy procedures as directed by their supervising Physical Therapist. The incumbent shall address appropriately the age specific needs for all patients. The incumbent will treat patients and families with respect and dignity. Patients pain will be assessed and treated according to the departments pain management policy. It is the philosophy of Mercy Hospital to optimize patients' comfort while they are in the hospital. The incumbent will identify and report any patient safety issue when identified. The Physical Therapist Assistant shall provide technical assistance and direction to the Physical Therapy aide and coordinates the workload in the department as directed.

* Graduate of an accredited school of massage
* Licensure for Massage Therapy through the State of Oklahoma
* Pass score on the Massage and Bodywork Licensing Examination (MBLEx) or grandfathered in through the State of Oklahoma
* Recommended CPR certification from the American Heart Association
* Ability to communicate effectively, both verbally and in writing
PHYSICAL REQUIREMENTS:
* Able to list, carry, push, and pull light to moderately heavy material, supplies or patients (up to or >100 lbs.) for most of the shift
* Able to lift, carry, push, and pull heavyweight materials (over 100 lbs.) occasionally

JOB SUMMARY:
Responsible for performing various therapy treatments and documentation under direction of a Licensed Occupational Therapist within the home setting for home care clients with disease/disability on an internittent basis in accordance with the plan of care.
QUALIFICATIONS:
* Graduate of Occupational Therapist Assistant Program.
* Current license to practice in Oklahoma
* Ability to communicate effectively, both verbally and in writing.
* Current CPR certification
* Having and maintaining a valid Oklahoma Driver's License and satisfactory driving record is a condition of employment
PHYSICAL REQUIREMENTS:
* Able to lift, carry, push, and pull light to moderately heavy material, supplies or patients (up to or > 100 lbs.) for most of the shift.
