Responsibilities Perform comprehensive patient assessments, including physical examinations, medical histories, and diagnostic tests Collaborate with cardiologists to develop and implement treatment plans for patients with cardiovascular conditions Administer medications, monitor patient responses, and adjust treatment plans as necessary Conduct cardiac stress tests, electrocardiograms (EKGs), and other diagnostic procedures Educate patients and their families about cardiovascular health, preventive measures, and the importance of medication adherence Provide counseling and emotional support to patients and their families Medication Management: NPs/PAs ensure residents receive the appropriate medications, monitor for side effects, and adjust dosages as needed. Chronic Disease Management: They provide ongoing care for residents with chronic conditions such as diabetes, heart disease, and dementia, helping to manage symptoms and improve quality of life. Wound Care: NPs/PAs are skilled in wound assessment and treatment, promoting healing and preventing complications. Fall Prevention: They identify residents at risk for falls and implement strategies to reduce fall incidents. Knowledge of CMS Regulations and NCQA HEDIS Guidelines Document accurate medical records to ensure compliance with OSHA and Government regulations and perform duties in accordance with site quality and safety systems Collaborate with the interdisciplinary team Analyze and organize the care of critically complex patients Requirements Minimum 6 months of experience in long-term care or a skilled nursing facility Active and unrestricted Nurse Practitioner, Physician Assistant, or Physician license DEA License CPR BCLS/BLS certified Must hold a Master's Degree in Nursing Completed a degree from an accredited Nurse Practitioner program Preferred experience in cardiology: training provided Preferred experience in Documenting EMR (Sigma, Visual, PCC, etc.): training provided Must have great organizational and time management skills.
